NON-EXCLUSIVE PROFESSIONAL ENGINEERING AGREEMENT
CONTINUING CONSULTING SERVICES
WORK AUTHORIZATION #4
ORCHID ISLAND / ORCHID ISLE ESTATES
WATER/SEWER ASSESSMENT PROJECT
• Scope of Services
Masteller & Moler, Inc . will provide for a route survey for engineering design purposes for a
proposed utility pipelines along the following route . Live Oak Drive, Seacrest Drive, Bayview
Court, Riverview Court, Lagoon Court and Orchid Lane. The route survey will locate all visible
objects within the right-of-ways (IE : mailboxes, pavements, poles, etc . ) . X-sections will be
obtained every 300 feet along the project route. Elevations will be based on NGVD 1929 . Upon
completion (5 ) copies and ( 1 ) digital file will be provided.
• Consulting Engineer Insurance Requirements
B . Work Orders Greater Than OR Equal to $25 ,000 and Less Than $ 500,000
The ENGINEER shall , at least ten ( 10) days prior to the commencement of any work covered by
a specific approved Work Order, provide to the COUNTY a certificate of commercial general
liability insurance with a reputable insurance company authorized to do business in the State of
Florida, subject to approval by the COUNTY ' s risk manager, in an amount not less than
$ 1 ,000,000 combined single limit for bodily injury and property damage, including coverage for
premises/operations, products/completed operations, contractual liability, and independent
contractors, in accordance with the COUNTY ' s Administrative Policy Manual , The
ENGINEER shall, at least ten ( 10) days prior to the commencement of any work covered by a
specific approved Work Order, provide to the COUNTY a certificate of professional liability
insurance with a reputable insurance company authorized to do business in the State of Florida,
subject to approval by the COUNTY' s risk manager, in an amount not less than $ 1 ,000,000 . The
ENGINEER shall, at least ten ( 10) days prior to commencement of any work covered by a
specific approved Work Order, provide to the COUNTY a certificate of business auto liability
insurance authorized to do business in the State of Florida, subject to approval by the
COUNTY' S risk manager, in an amount not less than $ 1 , 000,000 per occurrence combined
single limit for bodily injury and property damage, including coverage for owned autos and other
vehicles, hired autos and other vehicles, non-owned autos and other vehicles, in accordance with
the COUNTY' s Administrative Policy Manual . The commercial general liability and auto
liability insurance policies shall name Indian River County, a political subdivision of the State of
Florida, as an additional insured. In addition, the ENGINEER shall , at least ten ( 10) days prior

to the commencement of any work covered by a specific Work Order, provide to the COUNTY a
certificate of worker ' s compensation insurance, including employer' s liability, with a reputable
insurance company authorized to do business in the State of Florida, subject to approval by the
COUNTY' s risk manager, with a limit of $ 100, 000 per accident, $ 500, 000 disease (policy limit),
and a $ 100,000 disease (each employee) in compliance with all state and federal laws, and in
accordance with the COUNTY ' s Administrative Policy Manual , The ENGINEER shall provide
to the COUNTY at least thirty (30) days written notice by registered mail, return receipt
requested, addressed to the COUNTY ' s risk manager, prior to cancellation or modification of
any required insurance . ENGINEER shall cause any subcontractor performing any work to
provide to COUNTY certificates of insurance under the same conditions and with the same
policy limits as required of the ENGINEER.
• Method and Amount of Compensation
Masteller & Moler, Inc . proposes to provide for the above Scope of Services based on the
following lump sum fee :
Task 1 . Preparation of survey base plans for the purpose of
Engineering design of utility pipeline
To be constructed at Orchid Isle Estates $21 , 560 . 00
• Printing and Reproduction
The County shall make direct payment to our office for the cost of printing project plan sheets
required for utility coordination and for copies of reports, drawings, specifications, bidding and
construction contract documents, and other pertinent items required by federal , state and local
agencies from whom approval of the project must be obtained, prospective bidders, material
suppliers, and other interested parties, but may charge only for the actual cost of providing such
copies based on the following tabulation :

• Time of Performance
The estimated time frame for completion of services from the approval of this Work
Authorization is 60 days from formal authorization :
