This Work Order No . 3A and 3B is in accordance with the existing AGREEMENT dated April 16 , 2002 , between Carter Associates, Inc . (SURVEYOR) and Indian River County (COUNTY) . This Work Order No . 3A and 3B amends the agreement as follows : WORK ORDER NO. 3A - SECTION I — PROJECT LIMITS This Work Order is for a topographic survey for the LANDSIDE of the proposed Oslo Road Boat Ramp Facility. The SURVEYOR ' S work will include but may not be limited to, Topographic Survey. The SURVEYOR' S work will be used by the COUNTY and/or the COUNTY' S Stormwater Engineering Consulting Team for design and permitting of proposed boat ramp facilities . 200 ' Corridor — 100 ' north and south of physical centerline of existing graded road. Begin at 500 ' + west of each end of existing pavement to 100 '± beyond Mean High Water line of Indian River (4, 100 ' ±) WORK ORDER NO , 3A - SECTION II — SCOPE OF SERVICES PROVIDED BY THE SURVEYOR A. The SURVEYOR shall obtain and map as a "Topographic Survey" the projects existing horizontal and vertical datum and features as outlined below of the corridor for design and permitting purposes . 1 . Establish permanent survey Horizontal Baseline/Control and Benchmarks (NAVD99/NGVD29) at 1 , 000 ' + intervals . Begin control with a physical intersection at US # 1 . Baseline control as established by FDOT . 2 . Recover previous (IRC) or set two tidal Bench Mark monuments in the vicinity of existing boat ramp and the Indian River. 1 FAEngineering\Capital Projects\0138-Annual Survey Contract 2002-2003\0138 Work Order No. 3 , Carter-cjk.dot 3 . Horizontal Control shall be tied into State Plan coordinate system utilizing either IRC or City of Vero Beach GPS monuments . Coordinate and Elevations shall be shown on all major control points with a reference to monuments used. 4 . Locations of all surface features and underground utilities (as visible or per information supplied by IRC and other sources) . 5 . Location of established Mean High Water Line elevations (Tidal Waters), as it exist surrounding the entire corridor. 6 . +/- Locations of Mangrove outlines or areas of environmental concern. Depict type and +/- height of mangroves . 7 . Cross- section at 100 ' stations, offset 100 ' left and right of existing centerline of graded road. 8 . Soundings — extend 200 ' corridor beyond existing boat ramp +/- 100 ' easterly of MHW line. 9 . Topographic survey at 100 ' stations and intervals of the Indian River County parcel located on the north side of Oslo Road adjacent to the western shore of the Indian River (Part of Section 19 and 20, Township 33 South, Range 40 East — Parcel Number 33401900000700000002 . 0) . 10 . Deliverables : a. Preparation and submittal shall be in survey feet. b . 24" X 36" sheets with accompanying electronic disc . In AutoCAD 2000 format. c . Key sheet depicting the projects location and overall information. d. Sheet depicting Horizontal and Vertical Control network with assigned values . e . Individual sheets with related topographic/Hydrographic/Environmental Survey information. The submitted survey shall be certified to Indian River County. B . SURVEYOR' S WORK PRODUCT The SURVEYOR ' S work product must meet or exceed the following minimum standards, or else the COUNTY will not approve the SURVEYOR' S Request for Payment: 1 . Drawing Requirements a. Minimum drafting standards for all Drawings are : ( 1 ) Prepare all Drawings using AutoCAD 2000 or later. (2) Drawing Scale — Unless otherwise approved in writing by the COUNTY, the horizontal scale shall be 1 "= 100 ' or larger (e . g. 1 "=60 ' , 1 "=50 ' , etc . ) . Use match lines as appropriate . (3 ) Text Size - No text (including numbers) shall be smaller than 1 /8 -inch high. 2 FAEngineering\Capital Projects\0138-Annual Survey Contract 2002-2003\0138 Work Order No. 3 , Carter-cjk,dot (4) Drawing Sheet Size - Drawing sheet size shall be 24-inch by 36- inch. (5) Existing Utilities (a) Call out each existing utility in the following order: 1 . Type of utility ("high pressure gas main," high voltage electric lines, " "water, " "sewer, " "drainage culvert," etc .) 2 . Diameter 3 . Type of material 4 . Number of pipes/conduits , as applicable . (d) All existing utilities shall have the system-abbreviated text written into the line. (e . g . ------FM------ for a force main.) (e) Tie down the horizontal location of each utility. (f) Provide top-of-pipe (or invert) elevations at appropriate locations . (g) Screen existing utilities 60 percent. WORK ORDER NO. 3A - SECTION IV — TIME FOR COMPLETION The Work Order No . 2 shall be completed within 70 calendar days from Notice to Proceed. WORK ORDER NO. 3A - SECTION V — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered according to the " Scope of Services Provided by the SURVEYOR," identified in Section III of this WORK ORDER NO . 3A, with a maximum charge under this WORK ORDER of $ 13 , 820. 00 . The SURVEYOR shall obtain and map as a "Topographic Survey" the projects existing horizontal and vertical datum and features as outlined below of the corridor for design and permitting purposes : 1 . Baseline/control and benchmarks are to reference previously established landside control. Horizontal Control shall be tied into State Plan coordinate system utilizing either Indian River County of City of Vero Beach GPS monuments . Coordinates shall be shown on all major intersections with rights of way and or property lines . 2 . Cross- Section (Soundings) at 100 ' stations, offset 100 ' left and right of existing centerline of channel. Locations of all existing spoil mounds , sand bards, channel markers or buoys indicating the channel or navigable waterway (or obstruction of) . 3 . Channel Markers shall depict horizontal position (Coordinates) . 4 . Location /Mapping of existing Sea Grass areas or areas of environmental concern from the MHW line of Right-of-Way of Inter- coastal Water-way. Indian River County and/or a Sub- Consultant shall supply delineation of environmental area (Sea Grass) within the Indian River. (To be established prior to commencement with phase two) . B . ' Proiect No. 3 Phase One and Two e Dear Mr. O 'Brien: As requested, Carter Associates, Inc. is pleased to submit the following proposal for the performance and preparation of a Topographic survey on the proposed Oslo Road Boat Ramp Facility. Said project area being a portion of Oslo Road right-of-way (9`b Street S .W.), lying easterly of U. S . Highway Number 1 and the existing Oslo Road boat ramp channel easterly to the west right-of-way line of the Inter-Coastal Waterway, all being in Indian River County, Florida. Our firm is located in Vero Beach, Florida, and has been in existence since 1911 . Carter Associates, Inc. has a long standing history of competent, reliable, and professional land surveying services within the area. With its many years of experience in the field of surveying, land development, municipal and other government areas, Carter Associates, Inc. has kept abreast and amassed the technical knowledge and expertise to perform and complete the tasks as set forth in this proposal. We have assembled a professional team, each with thorough knowledge and experience m the field of surveying and mapping. Over the years, Carter Associates, Inc. has performed extensive survey projects in and around the City of Vero Beach and Indian River County area. It is our intent to meet all County requirements as listed in the furnished scope of services: Minimum Specification Requirements : Purpose: The purpose of this project is to obtain and map as a "Topographic Survey" the projects existing Horizontal and Vertical datum and features as outlined in scope of services below of the corridor for design and permitting purposes. Reference Datum : Horizontal reference datum must be adjusted to NAD 1983/99 and derived (and/or tied to) from the existing Indian River County Horizontal Control (I.R.C.H. C .) or City of Vero ' s State Plane coordinate system in use at time of this project. A report of Indian River Counties G.P. S . control used shall be included in final submittal. (Include point status, condition and or comments benefiting the maintenance of said points) Vertical Datum, NAVD88 with a relative "Vertcon" conversion to NGVD29 placed on the cover sheet and each additional sheet. D . O .T .). Carter Associates, Inc . estimates the time frame to complete Phases One and Two to be eight to ten weeks from the notice to proceed (depending on the weather) . Should the county decides to proceed with both phases , we have the manpower to perform the above described tasks concurrently. Dear Mr. Luethje: On July 1 , 2003 the Board of County Commissioners approved the above subject Work Orders at their regular meeting. The Chairman executed an agreement outlining the project limits, scope of services , etc . . There is a typographical error on page no . 3 of the abovementioned agreement. Paragraph 2 "Work Order No . 3A — Section IV — Time for Completion"; first sentence "The Work Order No. 2 shall be completed within 70 calendar days" . No . 2 in this sentence should read "No . 3A11 . 70 calendar days should read "5 weeks ". The same applies to page no . 5 .
