Home My WebLink About2003-088 y o, INDIAN RIVER COUNTY Oslo Road Bridge Replacement And Roadway Widening AMENDMENT NO . 1 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING SERVICES AGREEMENT BETWEEN KIMLEY-HORN AND ASSOCIATES , INC . AND INDIAN RIVER COUNTY, FLORIDA. This is an amendment to the existing Engineering/Land Surveying Services Agreement (AGREEMENT) dated March 24th, 1998 between Kimley-Horn and Associates , Inc . (ENGINEER) and Indian River County (COUNTY) . This amendment addresses changes in " Section I - Project Limits and Description " , Section II - County Obligations , " Section III - Scope of Services " , " Section V - Compensation " and " Section XVI — Duration of Agreement" of the AGREEMENT . Amendment Description This Amendment includes the following : " SECTION I - PROJECT LIMITS AND DESCRIPTION " is being modified to include plan revisions for shifting 43rd Avenue west of proposed alignment , incorporate limited bridges and roadway construction observation assistance services for the Oslo Road/43 ' d Avenue construction. " SECTION II — COUNTY OBLIGATIONS " is being modified to incorporate the following : It is understood that during the Construction Phase of the Oslo Road/43rd Avenue Intersection improvements , the COUNTY will undertake the day to day task of construction observation, which includes but is not limited to the following : • COUNTY representatives will be on site during construction and responsible for construction observation. • Conduct the weekly progress meetings . • Work with the Contractor to develop and process monthly pay request . • Review shop drawings associated with the bridge to include manufacturer specification certifications for reinforcing steel, bearing piles, cut sheets for toppings and sealants . Indian River County, Amendment No . 1 Oslo Road Bridge Replacements and Roadway Widening • Monitor the removal of the existing structure . • Monitor pile installations . • Provide monitoring of the utility attachments to the bridges . " SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following : The ENGINEER will revise the Oslo Road and 43rd Avenue roadway plans such that 43rd Avenue is positioned 12 feet west of the proposed location. Work shall include revisions of 43rd Avenue plan and profile sheets , adjustment of roadway transitions, revision of signing and marking plans , revision of plateau sheet, relocation of drainage structures , elimination of guardrail where appropriate, modification of signalization plans , and adjustment of quantities . The ENGINEER will provide limited construction services assistance for structural, roadway, and utility construction elements of the Oslo Road/43rd Avenue construction project . The following provides descriptions of the tasks that the ENGINEER will perform . The general scope of work will be conducted under a lump sum budget . However, should the COUNTY request additional services other than those described below, assistance may be authorized under an hourly budget . Structural Observation Assistance • The ENGINEER will attend the Pre- Construction meeting and address the questions that pertain to the structure . • The ENGINEER will review structural shop drawings to include : 1 . concrete grout and mortar mixes 2 . precast slabs and connection details 3 . sheet piles 4 . utilities saddles and culverts 5 . resubmittals (maximum of one each) . • Observation of the bridge construction is established to provide limited observation during the installation of the structure . The ENGINEER effort will include : 1 . one site visit to observe bent cap forms and rebar placement Page 2 of 6 Indian River County, Amendment No . 1 Oslo Road Bridge Replacements and Roadway Widening • The ENGINEER will also perform a pre-final walk through, review the record drawings, assist in establishing a punchlist, and perform a final walk through and certification. • The ENGINEER will assist with shop drawing review and field observation assistance beyond that described above on an hourly basis , when requested by The COUNTY . Roadway Construction Observation Assistance • The ENGINEER will attend the Pre- Construction meeting with regard to roadway construction for the Oslo Road/43rd Avenue construction project. • The ENGINEER will attend the bi-weekly progress meetings for up to 36 weeks . • The ENGINEER will respond to COUNTY and Contractor Requests for Information (RFI) • The ENGINEER will review the contractor submitted shop drawings . • The ENGINEER will review as -built drawings provided by the Contractor ' s licensed surveyor. • The ENGINEER will assist the COUNTY in conducting two , walk through inspections . One inspection as a final inspection and a second inspection as a follow up to clear punch list items . • When requested by the COUNTY, the ENGINEER will review Contractor proposed plan alternatives . This work will be undertaken as an additional service and the fee will be calculated at an hourly rate along with expenses . • At the request of the COUNTY, the ENGINEER will provide CADD assistance to input Contractor supplied As-Built data into the project electronic files . The ENGINEER will undertake this effort at an hourly rate . Utility Construction Observation Assistance • The ENGINEER will review the Contractor submitted shop drawings and material specification sheets . " SECTION V — COMPENSATION " is being modified to incorporate the following : The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered pursuant to this Agreement fees inclusive of expenses in accordance with the following : A . Professional Services Fee The basic compensation mutually agreed upon by the ENGINEER and the COUNTY is as follows : Task Labor Fee Lump Sum Components 43rd Avenue revisions $ 10 , 100 Structural observation assistance $ 129000 Roadway observation assistance $ 22 , 000 Utility observation assistance $ 4 , 000 Hourly Rate Components Labor Feel Structural on call assistance $ 45000 Roadway on call assistance $ 4 , 000 As-Built CADD assistance $45000 'Not to exceed without written authorization. Additional Services Fee The COUNTY agrees to pay for additional services , which are outside or beyond the lump sum scope of services identified above in accordance with the revised hourly rate schedule . Principal $ 150 Senior Professional $ 120 Professional $ 100 Engineer Analyst $ 85 Technician $ 75 Draftsman $ 65 Support Staff $48 2 -Man Survey Crew $ 90 " SECTION XVI — DURATION OF AGREEMENT " is being modified to incorporate the following : This Agreement shall remain in full force and effect until January 1 , 2005 or until completion of project phases as specified by the Public Works Director, or unless otherwise terminated by mutual consent of the parties hereto or pursuant to Section XII of this AGREEMENT .
