Oslo Road Bridge Replacement
And Roadway Widening
AMENDMENT NO . 3 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING
SERVICES AGREEMENT BETWEEN KIMLEY-HORN AND ASSOCIATES , INC . AND
INDIAN RIVER COUNTY, FLORIDA.
This is an amendment to the existing Engineering/Land Surveying Services Agreement (AGREEMENT)
dated March 24th, 1998 between Kimley-Horn and Associates, Inc . (ENGINEER) and Indian River
County (COUNTY) . This amendment addresses changes in " Section I - Project Limits and Description" ,
Section II - County Obligations, " Section III - Scope of Services " , " Section V - Compensation"
and
" Section XVI — Duration of Agreement " of the AGREEMENT .
Amendment Description
This Amendment includes the following :
" SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following :
The ENGINEER will revise the current Oslo Road construction documents, which detail the
construction of a 3 - lane quasi-urban roadway section, to define the construction of a 5 -lane urban
roadway section. These revisions will effect approximately 1 . 2 miles of the present construction
documents . The COUNTY also desires to extend the limits of the present roadway project
approximately 1 , 100 feet east to tie-in to the existing 5 -lane urban roadway section immediately
west of Old Dixie Highway and 1 , 000 feet west of the 20th Avenue/ Oslo Road intersection.
Roadway improvements west of the 20th Avenue intersection are required to facilitate the
construction of the 5 - lane urban roadway section and the ultimate build out of the intersection.
The COUNTY will provide the ENGINEER with the required topographic survey to enable design
of the additional 2 , 100 feet of roadway improvements .
Based upon the completed construction documents, this revision will require revisions to the
following plan sheets :
Plan and Profile — 11 Sheets
Pond Details — 2 Sheets
Cross Section — 15 Sheets
Signing and Marking — 5 Sheet
Construction Phasing — 1 Sheet
The extension of the eastern terminus will require the addition of the following plan sheets :
Plan and Profile — 4 Sheets
Intersection Plateau — 1 Sheet
Cross Section — 5 Sheets
Signing and Marking — 2 Sheets

Utility Design
The Engineer will develop construction documents to detail the relocation of the existing 20"
water main required to facilitate the proposed bridge replacement over the Lateral ` J ' Canal .
Utility Permitting
The Engineer will prepare and submit documentation to request a FDEP domestic wastewater and
public drinking water permit. The consultant will coordinate with FDEP in regards to obtaining
these permits . The county will be responsible for all permit application and agency review fees .
" SECTION V — COMPENSATION" is being modified to incorporate the following :
The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered pursuant
to this Agreement fees inclusive of expenses in accordance with the following :
A. Professional Services Fee
The basic compensation mutually agreed upon by the ENGINEER and the COUNTY is as
follows :

Oslo Road Revisions $41 ,400
Oslo Road Extension $219000
Utility Design $ 6 , 800
Utility Permitting $ 19600
" SECTION XVI — DURATION OF AGREEMENT " is being modified to incorporate the following :
The time for completion of the 90% design drawings and permit applications for the scope of
services detailed in this amendment will be five (5 ) months following receipt of the design survey
from the COUNTY. Once permits and 90% design comments are received from the COUNTY,
100% design drawings will be completed 30 days thereafter.
The AGREEMENT is hereby amended as specifically set forth here in. All other sections of the
AGREEMENT shall remain in full force and effect and are incorporated herein.

This Amendment No . 3 to the AGREEMENT regardless of where executed, shall be governed by and
construed by the laws of the State of Florida.
