FOR
PROFESSIONAL SURVEYING SERVICES AGREEMENT
FOR ANNUAL SURVEYING AND MAPPING SERVICES
WITH
KIMLEY-HORN & ASSOCIATES , INC .
This Work Order No . 2 is in accordance with the existing AGREEMENT dated April 16 , 20029
between Kimley-Horn & Associates , Inc . (SURVEYOR) and Indian River County (COUNTY) .
This Work Order No . 2 amends the agreement as follows :
SECTION I — PROJECT LIMITS
This Work Order No . 2 is for providing design survey of the Roseland convenient center area.
SECTION III - SCOPE OF SERVICES
Conduct a survey of the Roseland Convenient Center to include the following as a minimum :
1 . Existing right-of-way limits and/or easements within the limits of the Center.
2 . Existing parcels, tracts, and lot corner locations shown with front footage dimensions per
plat when platted. Also show back lots dimensions .
3 . Existing roadway edge of pavement or edge of dirt road .
4 . Existing utilities as located in field, surface evidence only, (water, sewer, telephone,
electric , etc .) . UNCLES to be contacted by SURVEYOR prior to survey locate.
5 . Existing utilities associated with Item 3 above (example : valves , meters, manhole, etc . ) .
6 . Existing curbs, driveways widths and types
7 . Existing drainage pipe crossings and driveway culverts (types, sizes and invert elevation) .
8 . Existing swales and/or ditches and elevations every 50 feet at top and bottom .
9 . Existing fences
10 . Existing trees and/or shrubbery (edge of vegetation line only) .
11 . All other non-movable items such as mailboxes , flag poles, etc . ) .
12 . All construction and/or route surveys shall meet the minimum requirements of Chapter
61 G17, FAC , Pursuant to Section 472 of the Florida Statutes ,
13 . All elevations shown shall be based an 1929 NGVD .
14 . All incoming route construction survey CAD drawing files shall be received on a CD
with a tie to state plane coordinates . (Note : prior to submitting the CAD drawing on CD,
one ( 1 ) copy of each survey shall be submitted for review and approval .

After all approvals a reproducible Mylar along with two (2) signed and sealed copies of
each survey shall be submitted .
SECTION IV — TIME FOR COMPLETION
The Work Order No . 2 shall be completed within 48 calendar days from Notice to Proceed.
SECTION V — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered
according to the "Scope of Services " identified in Section III of this Work Order No . 2 , a Lump
Sum of $4, 000 . 00 .
The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein .
IN WITNESS WHEREOF the partied hereto have executed these presents this 8th
daysof April 52003 .
