In accordance with Contract No. 0390, 2006-2007 This Supplemental No. 1 to Work Order No. 5 is in accordance with the existing AGREEMENT dated September 19th , 2006 , and the extension thereof, between Carter Associates , Inc. (SURVEYOR) and Indian River County (COUNTY) . This Supplemental No. 1 to Work Order No . 5 amends the agreement as follows: SECTION I — PROJECT LIMITS The County owned lands known as South County Oslo Transfer Station Site . Bordered by the South Main Relief canal and Lateral " G " . Project also includes the County owned lands under the utilities department along the south side of the main site . SECTION II — SCOPE OF SERVICES The scope of services for this SUPPLEMENTAL No. 1 to Work Order No. 5 is to perform follwing additional office survey tasks as requested by the County: 1 . Develop a Legal Description of the 60' wide utility easement for existing power poles and utility lines. 2 . Revise survey to include (two) easements and new lease areas of the existing communication tower and facility sites. 3 . Revise/Update survey to include items 1 and 2 , supply additional signed and sealed hardcopies and electronic file . SECTION IV — DELIVERABLES a) Deliverables and format shall be in accordance with the original Work Order No. 5. b) The SURVEYOR shall prepare its Work Product (digital and paper versions) so that the COUNTY, consultants or the public can readily use it for design and analysis of the area defined . It shall contain all information necessary for a third-party surveyor to independently recreate and/or utilize the survey work. It is acknowledged that all final products become property of Indian River County and will be available for use by the public at large. c) The SURVEYORS work product shall meet or exceed the minimum standards as defined by ALL sections , or the COUNTY will not approve the SURVEYORS request for payment. SECTION V - COMPENSATION The COUNTY agrees to pay , and the SURVEYOR agrees to accept, a fee for services rendered according to the "Scope of Services" and further identified in all the Sections of this Supplemental No. 1 to Work Order No . 5 , a Lump Sum of $ 1 ,485 . 00 . All and/or any additional services not described hereon shall be pre-approved by the COUNTY. Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the SURVEYOR for the 2006 — 2007 contract year. All invoicing and payments shall be in accordance with the original Professional Surveying Services Agreement for Annual Surveying and Mapping Services, Contract #0390 with the COUNTY and as stated hereon . All invoicing shall be supported with documentation ( Man hours ) of time and materials spent and following information prior to payments . Invoices shall include the work order, contract and project numbers. Invoice number related to this work order, original contract amount , current billing amount and retainage amounts .
