SUPPLEMENTAL No. 2 to WORK ORDER NO. 7 (Engineering)
FOR
PROFESSIONAL SURVEYING SERVICES AGREEMENT
FOR ANNUAL SURVEYING AND MAPPING SERVICES
WITH
KINILEY-HORN & ASSOCIATES, INC.
In accordance with Contract No. 0390
This Supplemental No. 2 to Work Order No. 7 is in accordance with the existing
AGREEMENT dated September 23 , 2003 , and the extension thereof, between Kimley-Horn &
Associates , Inc. (SURVEYOR) and Indian River County (COUNTY) .
This Supplemental No. 2 to Work Order No . 7 amends the agreement as follows:
SECTION I — PROJECT LIMITS
Oslo Road Phase 1 Right of Way Map — the SURVEY LIMITS shall begin at the FDOT baseline
of US Highway No. 1 and continue westerly on Oslo Road to a point that is 100' west of 27th
Avenue. The limits of the right of way mapping also includes 27th Avenue 1 , 300' north of Oslo
Road and 27th Avenue 1 , 300' south of Oslo Road .
SECTION II — SCOPE OF SERVICES
The scope of services for this SUPPLEMENTAL No. 2 to Work Order No. 7 is to perform
additional office survey tasks required to add the recording information for the TIIF (Murphy)
reservations and releases as contained within the Ownership and Encumbrance Reports
furnished by Atlantic Coastal Title Corporation for those parcels lying north of Oslo Road .
Position the TIIF reservations and releases on the Special Purpose Survey based upon their
respective legal descriptions .
SECTION III - TIME FOR COMPLETION
Supplemental tasks shall be completed within 30 calendar days from Notice to Proceed .

SECTION IV — DELIVERABLES
a) Deliverables and format shall be in accordance with the original Work Order No. 7.
b) The SURVEYOR shall prepare its Work Product (digital and paper versions) so that
the COUNTY, consultants or the public can readily use it for design and analysis of
the area defined . It shall contain all information necessary for a third-party surveyor
to independently recreate and/or utilize the survey work. It is acknowledged that all
final products become property of Indian River County and will be available for use
by the public at large .
c) The SURVEYORS work product shall meet or exceed the minimum standards as
defined by ALL sections , or the COUNTY will not approve the SURVEYORS request
for payment.
SECTION V - COMPENSATION
The COUNTY agrees to pay , and the SURVEYOR agrees to accept, a fee for services
rendered according to the "Scope of Services" and further identified in all the Sections of
this Supplemental No. 2 to Work Order No . 7 , a Lump Sum of $4 , 115 . 00 .
All and/or any additional services not described hereon shall be pre-approved by the
COUNTY. Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the 2006 — 2007 contract year.
All invoicing and payments shall be in accordance with the original Professional Surveying
Services Agreement for Annual Surveying and Mapping Services , contract No : 0390 with the
COUNTY and as stated hereon . All invoicing shall be supported with documentation ( Man
hours) of time and materials spent and following information prior to payments . Invoices
shall include the work order, contract and project numbers . Invoice number related to this
work order, original contract amount, current billing amount and retainage amounts .

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this
