AMENDMENT NO , 1 TO WORK AUTHORIZATION DIRECTIVE 2007 =005
AND CONTRACT EXTENSION AGREEMENT
This Amendment No . 1 to Work Authorization Directive 2007 - 005 and Contract
Extension Agreement ( "Amendment" ) is entered into this 10th day of February , 2009 , nunc
pro tunc to February 4 , 2009 , pursuant to that certain Standard Form Agreement Between
Owner and Contractor on the Basis of a Stipulated Price entered into as of February 6 , 2007
( "Amendment" ) by and between Indian River County , a political subdivision of the State of
Florida ( " County " ) and Underground Utilities , Inc . ( " Contractor" ) .
1 . The COUNTY has selected the Contractor to perform work as set forth in Work
Authorization Directive 2007-005 dated as of June 19 , 2007 .
2 . The COUNTY and the Contractor desire to amend Work Authorization Directive
2007 - 005 by increasing the amount approved for labor by $ 198 , 194 . 84 for a total amount of
$ 721 , 735 . 00 , all in accordance with the terms and provisions set forth in the Agreement .
3 . The County and the Contractor agree to amend article 3 of the Agreement to extend
the term of the Agreement to May 15 , 2009 , The County and the Contractor acknowledge
and agree that the extension of the term of the Agreement is not a renewal , and will
allow
completion of previously authorized work in process and work authorized in this Amendment .
IN WITNESS WHEREOF , the parties hereto have executed this Work Order as of the
date first written above .
