( Universal ) is pleased to provide this proposal for Geotechnical Engineering and Construction Materials Testing ( CMT) services at the above. practices and the project minimum requirements . referenced project in accordance with the generally accepted soil and foundation engineering We are confident that our experience , quality of service , professional attitude and proximity to the site will be of value throughout the design and construction phases of the project . PROJECT DESCRIPTION We understand that the project will be constructed in three phases and will consist of various improvements along 66th Avenue and 16th Street in Indian River County, Florida . These improvements include : stormwater utility installation , new sidewalk installation , new roadway construction , widening and resurfacing . In addition , approximately 3 , 700 linear feet of water main addition is planned for a portion of 66th Avenue . Three drilled shafts are also planned for signalization improvements at the intersection of 66th Avenue and 16th Street . The signalization improvements will include the installation of three (3 ) traffic signal masts , one each at the northeast , northwest and southwest corners of the subject intersection . We assume that the mast poles will have a maximum height of 25 feet and the mast arms will extend horizontally 60 to 80 feet . GEOTECHNICAL ENGINEERING SCOPE OF SERVICE Based on the information provided , Universal proposes the following field services to obtain information regarding the subsurface conditions at the site . The exploration will be performed in general concordance with pertinent FDOT guidelines and procedures . The field services will include the following tasks : 661" Avenue and 16«' Street Improvements EXHIBITA Universal Proposal No. 1910. 0209. 00008 rev Page 2 Field Services • Drill three (3 ) SPT borings , one ( 1 ) adjacent to each of the proposed signal mast locations , to a depth of 25 feet bls . • Install a temporary piezometer at each boring location and obtain stabilized groundwater level readings . • Stake boring locations , provide utilities clearance , and obtain groundwater level readings . All SPT borings will be drilled in accordance with ASTM D 1586 ( Standard Method for Penetration Test and Split- Barrel Sampling of Soils ) procedures . The SPT borings will be sampled continuously at 18 - inch intervals from ground surface to a depth of 10 feet bls and at 5 feet intervals thereafter. The SPT borings will be performed using an ATV mounted drilling rig and/or a truck mounted drill rig using either rotary mud techniques or continuous flight augers to termination depth . Each recovered soil sample will be visually classified in the field and a geologic log prepared for each soil boring . Groundwater depths and any unusual subsurface conditions will be recorded on the boring logs . Representative portions of the recovered soil samples from each boring location will be sealed and packaged for transportation to our laboratory for a more detailed analysis , as required . All soil samples will be classified in general accordance with ASTM, D 2487 ( Standard Classification of Soils for Engineering Purposes ( Unified Soil Classification System ) ) guidelines . Analysis and Engineering Report Universal will prepare a geotechnical engineering report for 'the signalization portion of the project , which will respond to the proposed property development . The report will be prepared under the direction of a professional geotechnical engineer registered in the State of Florida . The report will summarize the field and laboratory services performed and provide an engineering analysis of the site soil and groundwater conditions with special attention to potential subsurface conditions that may hinder development . The final report will include the following : • Methodology and results of the field exploration . • Review of available data such as USGS Topographic maps , soil surveys and potentiometric maps . • Results of the laboratory testing program • Estimates of the typical wet season groundwater level at the boring locations • Recommendations concerning soil parameters for soil strata encountered for the design of a foundation for the proposed structures . Notes will be made as to whether the soils meet FDOT standard foundation criteria . 66" Avenue and 16" Street Improvements EXH , Universal Proposal No. 9996. 0209. 00008 rev BIT A Pa e 3 Scheduling And Cost Estimate The field work can be scheduled to begin , based on our Present schedule , within one week after receiving the written authorization to proceed . Universal expects the field work to require require approximately two to three days to prepare a2 working days to complete . Our laboratory testing analysis and report preparation will after completion of the field work . A final zation geotechnical report should be submitted within three weeks from receiving the authori to proceed . If this schedule needs to be amended or if special scheduling considera required please advice us . tion is The fee for the subsurface exploration and geotechnical engineering report is $ 2 , 593 . 00 . This estimate will not be exceeded without prior written authorization . For specific terms of the proposed work , please refer to our continuing services contract terms and conditions . CONSTRUCTION MATERIALS TESTING ( CMT) SCOPE OF SERVICES Based on review of the project documents and our experience in the area , provide the following : Universal will • On -call earthwork , asphalt and concrete testing • Engineering overview of our services The laboratory testing will be performed out of our Fort Pierce , Florida office with additional support from our Rockledge , Florida office , if required . During the duration of the project all technicians and personnel involved with the construction testing and inspection will be supervised by a Professional Engineer ( P . E . ) registered in the State of Florida . The following assumptions were made to develop the estimated scope of services : • Roadway construction will be phased into at least two separate operations , travel lane , and compaction tests will be performed every 500 linear feet . one per • Curb pad and stabilized shoulders will be constructed as a separate operation from the mainline roadway construction , and compaction tests will be performed every 500 linear feet . • Stabilized subgrade will be sampled and tested for Limerock Bearing Ratio ( LBR ) every 1 , 000 linear feet per width of roadway operation and density will be checked as a single 124ch lift . • Base course will be sampled and tested for Limerock Bearing Ratio (LBR ) every 2 , 000 linear feet per width of roadway operation and density will be checked as two 4 - inch lifts . • Utility pipe and structures will be tested for compaction for the full depth of fill in 6 - inch lifts at 100 % of a standard Proctor test (AASHTO T99 ) in accordance with the project plans . Tests will be performed at a minimum frequency of one per every 200 linear feet or one per pipe run . • A minimum of three feet ( vertical ) of roadway subgrade ( embankment) will be compacted and tested at a frequency of one per every 500 linear feet . • No compaction testing will be required for the perforated bituminous coated corrugated metal pipe ( BCCMP ) . • No density compaction testing will be required for the 4- inch underdrains . 6d ' Avenue and 16`h Street Improvements EX H , B ! Cost of Services The services listed herein will be invoiced on a unit rate basis per the estimated scope . Note that the budget is an estimate of our services and may change as the result of unforeseen events such as requested standby time , overtime , retesting , etc . Pick up charges are not included in soil or concrete testing rates . A minimum of four density tests are required per trip .
