As requested, enclosed herewith please find a draft of the proposed Work
Authorization 419 to complete a boundary and topographic survey of the
existing utility easement for the referenced Life Station # 58.
We agree with your conclusion that additional easement area will need to be
obtained from HealthSouth to accommodate the existing encroachments . Our
proposal includes the preparation of a legal description for additional easement
as necessary. The exact limits can be determined after the survey is completed .
We thank you for this opportunity and look forward to working with you on this
project .

"Lift Station 58 Survey"
FOR
PROFESSIONAL SURVEYING SERVICES AGREEMENT
FOR ANNUAL SURVEYING AND MAPPING SERVICES CONTRACT NO : 0390
WITH
Masteller, Moler, Reed & Taylor, Inc,
This Work Order No. 19 is in accordance with the existing AGREEMENT dated September 19th,
2006 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County
(COUNTY). This Work Order No. 19 amends the agreement as follows :
SECTION 1 — PROJECT LIMITS
Perform Boundary and Topographic Survey for Lift Station 58 on easement portion of property
located in Indian River County and lying in Section 26, Township 032 South, Range 039 East and
identified as Tax Parcel 32392600000700000001 .4.
SECTION II - SCOPE OF SERVICES
As agreed upon between Masteller, Moler, Reed & Taylor, Inc. and Indian River County, Masteller,
Moler, Reed & Taylor, Inc. will perform Professional Land Surveying services to include the
following :
a. Perform limited boundary survey of Healthsouth property and identify location of existing
Utility Easement per Bill of Sale recorded in Official Record Book 762, Page 350. Boundary survey
shall be prepared in accordance with the Minimum Technical Standards for surveying as adopted by
the State of Florida.
b. Perform Bench Run using NGVD 1929 elevations to the site and establish benchmarks on site.
c . Obtain elevations on site and 25 feet outside of the perimeter of the easement, sufficient to
generate one foot contours.
d . Locate and obtain elevations of all visible improvements on and adjacent to the site. (i. e . :
utilities, culverts, pavement, mailboxes, etc.)
e. Measure and dimensions existing improvements, including wet well, valve box, generator etc .
f. Prepare legal description and sketch of additional easement to be obtained from property owner.
g. Provide five copies and one digital file of the boundary and topographic survey upon
completion.
SECTION III — WORK PRODUCT
a) The SURVEYOR shall prepare its Work Product so that the COUNTY or other consultants
can readily use it for the design and analysis of the area, as defined. It shall contain all
information necessary for third-parry surveyor to independently recreate and/or utilize the
survey work. It is acknowledged all final products become property of Indian River County
and will be available for use by the public at large .

SECTION IV — TIME FOR COMPLETION
Work Order No . 19 shall be completed within 30 business days from Notice to Proceed.
SECTION V — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered
according to the "Scope of Services" identified in Section II of this Work Order No . 19, a Lump
Sum (not to exceed) fee of $5 ,400 .00.
The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein.
