WORK ORDER NO . 1 ( Engineering/Surveying )
FOR
PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Carter Associates , Inc.
In accordance with Contract No. 0725, 2007-2009
This Work Order No . 1 is in accordance with the existing AGREEMENT dated November 6th
2007 between Carter Associates, Inc. (SURVEYOR) and Indian River County (COUNTY) . This
Work Order No. 1 amends the agreement as follows :
SECTION I — PROJECT LIMITS
This Work Order No. 1 is for the SURVEYOR to perform a Boundary and Improvement Survey
for the existing Lennies Lounge site at 1959 43`d Avenue, Indian River County, Florida .
SECTION II - SCOPE OF SERVICES
As agreed upon between Carter Associates , Inc. and Indian River County, the SURVEYOR
shall provide Professional Land Surveying services to complete all tasks as outlined in this Work
Order No. 1 described as follows :
Part of Tract 9 , Section 4 , Township 33 South , Range 39 East and Part of Lots 1 and 26,
Pinewood Subdivision (plat Book 2 , Page 89 , Public Records of IRC , FL) less additional right-of-
ways for 43`d Avenue and State Road 60 . 'Parcel Identification Number 4-33-39-00001 -0090-
00001 . 0
Provide field and office services necessary to perform and prepare a certified Boundary Survey
and Map (of the properties described above and in provided title search report) in compliance
with the Florida Minimum Technical Standards , Chapter 61G17 , Florida Administrative Code,
pursuant to Chapter 472 . 027 Florida Statutes ,
Boundary Survey to also show all above ground (visible) improvements ; and delineate and
identify all furnished easements , additional rights-of-way and leases of record .

SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT
1 . Project shall be completed within thirty (30) business days from Notice to Proceed .
2. Deliverables to include electronic copies in AutoCAD 2007 and PDF formats, five (5)
signed and sealed 24"x36" hard copies and five (5) 8- 1 /2" x 11 " recordable hard
copies
SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as
identified in Sections I , II , and III of this Work Order No. 1 a lump sum fee of $5 ,850 . 00.
The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein.
