No . 9321 ) DATE: January 8, 2007 DESCRIPTION AND CONDITIONS On October 4, 2005 the Board of County Commissioners approved an annual agreement for Professional Geotechnical Services , with CTI -Construction Testing & Inspection , Inc. for various services including but not limited to Field and Laboratory Construction Materials Testing in Indian River County under IRC Project No. 0534. The purpose of this Work Order is to provide testing services for the construction of the subject projects. CTI-Construction Testing & Inspection , Inc. submitted a proposal outlined in the attached Work Order No. 1 , (Engineering) and scope of work (Exhibit A) for these projects for a fee not to exceed $ 8 , 540 . 54. RECOMMENDATION Staff recommends approval of the attached Work Order No. 1 (Engineering) authorizing the above-mentioned project as outlined in the attached scope of services and requests the Board to authorize the Chairman to execute the attached Work Order on their behalf for fee not-to- exceed $ 83540. 54. All services performed under this contract shall be supervised and certified by a licensed Professional Engineer ( PE) registered with the State of Florida and staff employed directly with your firm . Scope of Services All work shall be performed on a unit price basis in accordance with the attached Scope of Services dated January 2 , 2007 and shall comply with Standards enumerated in the RENEWAL OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT, IRC PROJECT NO. 0534 WITH CTI , INC . Effective Date : October 1 , 2006 . Minimum Specification Requirements as follows: Roadway Construction • In-place moisture density testing of embankment fill , pipe backfill , stabilized sub grade , base course, and curb pads by Nuclear Method (AASHTO-T238) . • Laboratory testing of soils and aggregates including Proctors , Limerock Bearing Ratio (LBR) , soil classification , carbonate content (base) , and other such specified QC tests . • Standard laboratory testing of SP 9 . 5, SP 12 . 5 asphalt • Standard laboratory testing of concrete flume , curbing , sidewalk and endwalls . • Preparation and issuance of engineering reports presenting the test results and their compliance with the project specifications . All work shall also comply with the current Florida Minimum Technical Standards. COMPENSATION . The COUNTY agrees to pay, and the Geotechnical Professional agrees to accept , a fee for services rendered according to the "Scope of Services" identified in the attached EXHIBIT "A" Fee Schedule and Man Hours.
