This Work Order Number 1 is entered into as of this 4'h day of September, 2007 ,
pursuant to that certain Contract Agreement for Professional Services entered into as of
October 19 , 2004 ("Agreement') , by and between INDIAN RIVER COUNTY, a political
subdivision of the State of Florida ("County") and DONADIO AND ASSOCIATES ,
ARCHITECTS , P . A. , a corporation existing under the laws of the State of Florida
("Consultant') .
The Consultant has agreed to extend the terms of the Agreement to the Indian River
County Emergency Services District (" District') . The District has selected the Consultant to
perform the professional services set forth in Exhibit 1 , attached to this Work Order and made
part hereof by this reference . The professional services will be performed by the Consultant
for the fee schedule and within the timeframe set forth in Exhibit 1 , all in accordance with the
terms and provisions set forth in the Agreement. Pursuant to paragraph 1 .4 of the
Agreement, nothing contained in any Work Order shall conflict with the terms of the
Agreement and the terms of the Agreement shall be deemed to be incorporated in each
individual Work Order as if fully set forth herein .
IN WITNESS WHEREOF , the parties hereto have executed this Work Order as of the
date first written above .

Dear Ed,
Within the content of this letter is a Proposal for Architectural/Engineering Services for a Site Plan,
Foundation and Concrete Slab to house an Owner supplied 3 Story Prefab Fire Training Tower located on the
current Indian River County Emergency Operation Center site at 4225 43`d Avenue in Indian River County,
Florida. The proposed Training Tower and Slab will be located at the North end of the stabilized "Training
Area" shown on the current site plan .
The Fee for Architectural/Engineering Services to develop Construction Documents for the 3-Story
Prefab Training Facility Site Plan, Foundation and Concrete Slab shall be a lump sum fee of $ 18,000.00
This Fee includes Services for Architectural, Civil, Structural and Electrical Engineering as follows :
A. Architectural services will be provided to coordinate with the Owner to design the conceptual
site plan , along with the coordination of Civil, Structural and Electrical Engineering for this
project.
B . Administrative Approval to modify the existing site plan of the Emergency Operation Center
to show the Training Tower and concrete slab, along with proposed grading, drainage and
landscaping associated with the proposed improvements . Since the addition of these
improvements will not cause an increase in impervious area, it shall be assumed that
modifications to the existing Indian River County Type "A" and St. Johns River Water
Management District (SJRWMD) ERP storm water permit will not be required.
C. Construction inspections shall be performed for certification of proper completion of the
project construction. Upon completion of the project the Owner must provide as-built field
survey information in AutoCAD format, acceptable to the permitting agency requirements .
Upon completion of the site improvements and review of the as-built and testing results to
confirm proper construction, Engineering certification shall be prepared to submit to the
Indian River Planning Department.

D. Structural engineering services will be provided to design the Tower foundation and ground
floor slab, along with the concrete apron around the Tower. The Owner shall provide CAD
backgrounds of the Fire Training Tower delineating the slab geometry, tower anchor bolt
locations and any other foundation related information. The Owner shall also provide the
design reactions from the pre-engineered fire-training tower.
E. Electrical engineering services scope is limited to the provision of power for the Fire Training
Tower. Owner shall provide the Fire Training Tower power requirements and from where the
power will be fed to the Tower.
SCOPE OF SERVICES
This proposal is based upon the provision of the following Scope of Services :
L SCHEMATIC DESIGN/DESIGN DEVELOPMENT PHASE
The Architect shall assist the Owner in establishing the program and ascertain the requirements of the
project and shall arrive at a mutual understanding of such requirements with the Owner. Based any
adjustments authorized by the Owner in the program, schedule or construction budget, the
Architect shall prepare, for approval by the Owner, Schematic Design/Design Development
Documents consisting of drawings and other documents to fix and describe the size, character of
the project, materials and such other elements as may be appropriate.
H. CONSTRUCTION DOCUMENTS PHASE
Based on the approved Schematic Design/Design Development Documents and any further
adjustments in the scope or quality of the project or in the construction budget authorized by the
Owner, the Architect shall prepare, for approval by the Owner, a permit set of Construction
Documents consisting of drawings and outline specifications setting forth in detail the
requirements for the Construction of the Project.
III. BIDDING OR NEGOTIATION PHASE — NOT REQUIRED
IV, CONSTRUCTION PHASE-ADMINISTRATION
OF THE CONSTRUCTION CONTRACT
The Architect' s responsibility to provide Basic Services for the Construction Phase under this
Agreement commences with the award of the contract for construction and terminates at the
issuance to the Owner of the final Certificate for Payment or 60 days after the date of Substantial
Completion of the Work, whichever comes first.
The Architect shall provide periodic Site Observations, Review of Contractor' s Applications for
Payment, Contractors request for information and/or clarifications, Shop Drawing Reviews,
Review of Change Order and decisions on any claims or disputes.

V, FEE PAYMENT SCHEDULE
A. The fee is based upon our understanding of the stated goals and upon completion of services
described above. The Fee Payment Schedule shall be as follows:
Schematic Design /Design Development Phase 40%
Construction Document Phase 40%
Construction Administration 20%
TOTAL 100 %
Reimbursable expenses are in addition to compensation for Basic Services and
include expenses incurred by the Architect, in the interest of the project. Such
expenses include, but are not necessarily limited to the cost of reproduction, postage ,
long-distance communication and facsimiles and photographs . Reimbursable expenses will
be billed at actual cost. Estimated Cost of Reimbursables is Not To Exceed Two
Thousand Five Hundred Dollars ($2,500.00).
B . Progress billings will be made monthly or upon completion of each phase of the project.
C. Should the scope of the project change substantially, the fee will be altered accordingly,
based upon Owner approval . Such work will be billed at the hourly rates specified below,
for the time involved. No additional work will be initiated without Owner authorization.
D. Donadio & Associates, Architects, P.A . Standard Hourly Billing Rates are as
follows :
Principal $200.00
Senior Project Manager $ 175.00
Project Manager $ 135.00
CADD Manager $ 135.00
CADD Technician I $ 100.00
CADD Technician II $ 90.00
CADD Technician III $ 85 .00
Intern Architect $ 100.00
Office Manager $ 85.00
Administrative Assistant $ 65.00
Receptionist S 60.00
Technical Support $ 55 .00
E. If the project is abandoned or terminated before completion, owner will be invoiced for the
services performed, together with the reimbursable expenses incurred .
F. Should any questions arise regarding an invoice or any portion there of, this office should be
contacted immediately. Payment for the amount in question may be withheld. However,
timely payment of the balance is required.
G. Payment for services performed is in no way based upon the receipt of regulatory approvals
or project financing.

understanding between Parties with respect to the referenced Project and may only be
modified in writing.
VI. GENERAL
A. Illustrative renderings or models are not part of the Basic Services.
B . Design and Construction of the 3-Story Prefab Training Tower will be the responsibility
of the Indian River County Fire District. Fire District shall provide the Architect with all
the required design and construction documents for the 3-Story Training Tower Facility.
C. Site Surveying, Traffic Impact Statement/Studies/Link Sheets, Geotechnical Reports, Off
Site Road Way Improvements , Irrigation Design, Consumptive Use Permitting,
Environmental Surveys, modifications to the existing Indian River County Type "A" and
St. Johns River Water Management District (SJRWMD) ERP storm water permit, Re-
Zoning, Site Lighting, Fire Protection, and Application Fees for Agency approvals are not
part of the Basic Services.
D. All drawings, specifications and other documents prepared for this project are for use
solely with this project and unless otherwise provided Donadio & Associates, Architects,
P.A. are the author and owner and retain all lawful copyrights to such documents.
This Proposal letter outlines our Architectural / Engineering Scope of Services.
