This Work Order No . 10 is in accordance with the existing AGREEMENT dated September 2"a 2003 between Masteller, Moler & Reed, Inc . (SURVEYOR) and Indian River County (COUNTY). This Work Order No . 10 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 10 is for the SURVEYOR to provide a route survey of 66th Avenue from 26th Street to 33rd Street and 58th Avenue from 41st Street to 45th Street as depicted on the "Vicinity Map" attached hereto and made part of this agreement. SECTION II - SCOPE OF SERVICES As agreed upon between Masteller, Moler & Reed, Inc . and Indian River County, Masteller, Moler & Reed, Inc . will perform Professional Land Surveying services for the completion and preparation of a route survey of 66th Avenue from 33rd Street to 41 s` Street and 58th Avenue from 41st Street to 45th Street to include the following: a) Establish survey baseline and tie to state plane coordinates. b) Locate and identify all monumentation for establishment of existing rights of way. c) Locate and identify all visible topographic features along the route of the proposed sewer system improvements including but not limited to the location of existing pavement, driveways, culverts, power poles, mailboxes, fences, trees, etc . d) Obtain elevations along the route of the proposed sewer improvements (as depicted on Figure 4 attached) sufficient for engineering design purposes . e) Elevations will be obtained at 100 foot intervals and will be based on NGVD 1929 unless otherwise directed. f) Survey shall identify all tracts, lot numbers, street addresses and parcel identification numbers for property along the limits of the route. Survey shall also identify the width of all driveways and the size and type of all culverts. g) Prepare and provide five paper copies and one digital file of the route survey upon completion of field work. 74 IRC-Work Order No. 10 66"i & 58" Avenue Route Survey 2/8/2006 Page 2 of 2 SECTION III — WORK PRODUCT a) The SURVEYOR shall prepare its Work Product so that the COUNTY or other consultants can readily use it for the design and analysis -of the area, as defined . It shall contain all information necessary for third-party surveyor to independently recreate and/or utilize the survey work. It is acknowledged all final products become property of Indian River County and will be available for use by the public at large. SECTION IV — TIME FOR COMPLETION Work Order No . 10 shall be completed within 45 business days from Notice to Proceed . SECTION V — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered according to the "Scope of Services" identified in Section II of this Work Order No . 10, a Lump Sum (not to exceed) fee of $ 6 ,250 . 00. The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein .
