Survey) WORK ORDER NO. 12 (Engineering) PROFESSIONAL SURVEYING SERVICES AGREEMENT FOR ANNUAL SURVEYING AND MAPPING SERVICES WITH Masteller, Molar, Reed & Taylor, Inc. In accordance with Contract No. 0390 This Work Order No. 12 is in accordance with the existing AGREEMENT dated September 23RD, 2006 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County (COUNTY) . This Work Order No. 12 amends the agreement as follows: SECTION 1 — PROJECT LIMITS This Work Order No. 12 is for the SURVEYOR to perform additional services for the proposed 39th Avenue Corridor between 69"' Street and 71 " Street, Indian River County, Florida . SECTION 11 - SCOPE OF SERVICES As agreed upon between Masteller, Moler, Reed & Taylor, Inc. and Indian River County, Masteller, Moler, Reed & Taylor, Inc. shall provide Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 12 to update the topographic survey as follows : TASK 1 : • Recover and/or reset and cage horizontal and vertical control as shown on existing MMRT Survey #5396 .01 . • Recover and flag right-of-way and property corners along 39"' Avenue, between 69' & 71st Streets. • Update existing topographic survey as necessary. TASK 2 : • Locate and identify all native trees within the 39' Avenue Corridor that are 4" dbh or greater with the exception of those trees noted in Chapter 927 . 06 of the County code. Integrate tree survey as a separate sheet into existing MMRT Survey #5396 . 01 . TASK 3 : • Coordinate with environmental consultant on location of turtle nests, if any. • Locate and identify turtle nests and depict on existing MMRT Survey #5396 . 01 . Work Order # 12E 39'h Avenue Corridor — Tree and Update Topo Page 2 of 3 SECTION III — TIME FOR COMPLETION Project shall be completed within 60 calendar days from Notice to Proceed . Final submittal (Deliverables) will be provided to the County upon completion . SECTION IV — DELIVERABLESMORK PRODUCT a) Survey shall be supplied as a topographic base map file including tree locations (as a separate sheet) , tortoise nests (if any) and the original wetland flag locations. b) Said base map file shall include survey horizontal and vertical control and subsequent topographic, tree and environmental data sufficient for all anticipated design and planning needs. c) Deliverables for the final ( 100%) submittal to the COUNTY will consist of a digital file (disk) . d ) The SURVEYOR shall provide the COUNTY five (5) certified copies and a digital file of the survey and prepare its Work Product digital versions so that the COUNTY or other consultants can readily use it for the design and analysis of the area , as defined . It shall contain all information necessary for third-party surveyor to independently recreate and/or utilize the survey work. It is acknowledged all final products become property of Indian River County and will be available for use by the public at large. e) The SURVEYOR'S work product shall meet or exceed the minimum standards as defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR'S request for payment. SECTION V — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept , for services rendered as identified in Sections Il , III and IV of this Work Order No. 12 a fee at the standard hourly rates as agreed to by the Consultant and the COUNTY, as follows: TASK 1 : Not to Exceed $2 , 500 . 00 TASK 2 : Not to Exceed $3 , 200 . 00 TASK 3 : Minimum $ 100 . 00 for 2 locations , $50. 00 each additional location . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein . Funds for all tasks as outline in the amount of $6 , 000 . 00 shall come from the annual Professional Surveying and Mapping Services Contract #0390 for 2005-2006 (account number 10921441 -033490-05007) .
