FOR ANNUAL SURVEYING AND MAPPING SERVICES

This Work Order No . 13 is in accordance with the existing AGREEMENT dated September 19th ,
2006 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County
(COUNTY). This Work Order No . 13 amends the agreement as follows:
SECTION I — PROJECT LIMITS
This Work Order No . 13 is for the SURVEYOR to prepare a Right of Way Map for Phase Two of
the existing 66 foot wide Right of Way for Indian River Drive from a Right of Way Survey
performed as a part of Phase One of the project. The overall project limits range from the
southern intersection of Indian River Drive with US 1 to the northern intersection of Indian River
Drive with US 1 (approx. 3. 5 miles) . Phase Two of the project ranges from the southern overall
project limits at US1 to the northern city limits of the City of Sebastian (approx . 2 . 0 miles ). All
lying in Indian River County, Florida .
SECTION II - SCOPE OF SERVICES
As agreed upon between SURVEYOR and COUNTY, the SURVEYOR shall provide
Professional Land Surveying services to complete all tasks as outlined in this Work Order No.
13 described as follows :
• Right of Way Survey (Performed as a part of Phase One) :
1 . Establish and field locate the centerline of the existing 66 foot Right of Way for Indian
River Drive from US1 to US1 as described in Deed Book 538, Page 541 .
2 . Find or establish sectional data as necessary for establishing the Indian River Drive
centerline and certify as required . Locate and identify existing Right of Way
monumentation along the overall project limits . Prepare and record Certified Corner
Reports as applicable to Florida Statutes

3 . Monument Permanent Control Points (PCP's) with referenced points along the
centerline at all changes in direction , including all Points of Curvature and Tangency.
Provide State Plane Coordinate values on all baseline control points and reference
points. PCP's and reference points shall be a nail and tab stamped "PCP LB #4644"
and shall be placed at a maximum of 1000 feet apart.
4 . The product shall include underlying topographic information from prior work
performed by Masteller, Moler, Reed & Taylor, Inc. and Indian River County.
5 . Locate and depict all Indian River County G . P. S . control monuments.
• Right of Way Mapping:
1 . Prepare final Right of Way Map for Phase Two as described above and performed
as a part of Phase One. Map shall depict limits of 66 foot Right of Way for Phase
Two and the centerline of right of way for overall project limits including ties to
sectional data as necessary.
2 . Perform research for deeds , right of way maps , etc. as necessary.
3 . Map shall also depict limits of existing Rights-of-Ways and/or Boundaries as per
apparent physical evidence, deeds as provided by the County and the limits of Right
of Way as shown on existing recorded plats along the route.
4. Right of Way Map shall meet or exceed all requirements as listed in the Indian River
County Right of Way checklist as modified and attached hereto as Exhibit "A" .
SECTION III — COUNTY RESPONSIBILITIES
It is the COUNTY's responsibility to provide an abbreviated title search and all other information
of record for properties along the proposed route for Phase Two .
SECTION IV — CONSULTANTS RESPONSIBILITIES
It is the Consultants responsibility to obtain legal access to properties along the route prior to
performing any work. Proper safety precautions and protocol shall be maintained at all times .
SECTION V — TIME FOR COMPLETION
Project shall be completed within ninety (90) calendar days from Notice to Proceed . Schedule
shall include a 60-70% submittal within sixty (60) calendar days for review and approvals by the
COUNTY. Final submittal ( Deliverables) shall be provided to the COUNTY upon completion .

SECTION VI — DELIVERABLESIWORK PRODUCT
1 . Survey shall be supplied as a right of way map in state plane coordinates and
suitable for recording ; in the form of a cover sheet, Control sheet, key sheet and
detailed Right-of Way sheets as applicable.
2 . Deliverables shall include a 60-70% submittal within sixty (60) calendar days for
review and approvals by the COUNTY prior to proceeding with the final product.
3. Deliverables for the final ( 100%) submittal within ninety (90) calendar days to the
COUNTY will consist of a digital file (disk). The digital file shall adhere to basic
COUNTY AutoCAD layer structure as provided .
4. The SURVEYOR shall provide the COUNTY one ( 1 ) executed mylar in recordable
format and a digital file in . pdf format of the survey and prepare its Work Product
digital versions so that the COUNTY or other consultants can readily use it for the
design and analysis of the area , as defined . It shall contain all information necessary
for third-party surveyor to independently recreate and/or utilize the survey work. It is
acknowledged all final products become property of COUNTY and will be available
for use by the public at large .
5. The SURVEYOR'S work product shall meet or exceed the minimum standards as
defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR'S
request for payment.
6. Final product will be provided to the COUNTY in a recordable format.
SECTION VII — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as
identified in Sections II , III and IV of this Work Order No. 13 a lump sum fee of $ 27, 260. 00.
All invoicing shall be supported with documentation ( Man hours ) of time and materials spent
and following information prior to payments . Invoices shall include the work order, contract
and project numbers . Invoice number related to this work order, original contract amount ,
current billing amount and retainage amounts .

The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein .

EXHIBIT "A"
INDIAN RIVER COUNTY
RIGHT OF WAY MAP CHECKLIST
All Right-of-way projects shall adhere to Chapter 177 & 472 , F. S. and
Chapter 61G17, F.A.C. as minimum and be certified as such.
1 . Cover sheet/Key sheet.
2 . Survey notes and report sheet (As applicable).
3 . Survey control sheet (As applicable and or necessary).
4. Subsequent R/W map sheets .
5 . Supply a box or area for COUNTY acceptance and/or certification.
6. Title Box of each sheet must contain :
a. COUNTY
b. State Road Number
c. Designed, drawn and checked by (use names or initials) & date
d. Scale
e. Field Book References
f Sheet Numbers to be in numerical order ( 1 , 2, 3 , 4, etc.) NOT ( 1 , 1 A, I B,
2, etc.)
7. Any notes necessary to set out the type and/or purpose of the map shall be shown.
8 . Show pertinent dimensions and bearings in the following fashion as the case may
be:
a. Plat — (P)
b. Computed — (C)
c. Field — (F)
d. Deed (D)
9. Table of Ownerships shall show :
a. Parcel Number
b. Owners Name
c. Recording Data
10. Show stations and offsets to all Right of Way Breaks .
11 . Establish a baseline alignment for control and determine location of significant
elements for future monumentation .
12. Verify that computed distances and bearings are supported by field
measurements.
13 . Intentionally omitted — to be included in future contract.
14. Ensure that significant elements of all curves are shown and are correct. Also
show curve data on all sheets where they appear.
15 . Begin and end R/W Project with Stations .
16. Stationing in field minimum 500' beyond beginning and ending of project.

17 . Baseline (equate to intersecting STATE or COUNTY Jobs by Station and job
number when available) as available.
18. Projects shall be in NAD83/90 State plane coordinate system and bearings
reference shall be shown and must be clearly stated. Values shall be placed on key
baseline monumentation.

21 . Township.
22 . Range.
23 . Government Lots, Grants, etc.
24. Sections and '/4 Sections (lines & comers). Certified Comer reports of all Section
and '/4 Section corners used in the course of survey shall be completed and/or
updated.
25 . City Limits by Name & Symbol ; Name of City, ifjob entirely within city limits
but does not cross-city limits .
26. Land and Subdivision Ties (angles and/or, bearings & distances).
27. Subdivision name and limits.
28 . Plat Book and page (COUNTY).
29. Lots and Blocks (key map — blocks only).
30. Lot and Street dimensions (plat).
31 . Show all original plat and R/W lines of record, if different from present time, with
dashed lines.
32 . Show names of subdivision streets, plat book and page recording — also, show Lot
and Block number and Dimensions, Exceptions, Easements, and Vacated
portions.
33 . When property lines break the Right of Way on curve, show tangent bearings or
chord bearings.
34 . Show any replat of a subdivision — show old subdivision when necessary to clear
up former acquisition or old title references — use system of dashed lines, dotted
figures, etc . , with ties shown and name and plat book reference indicated.
35 . Stations and Angle Ties to Existing Side Streets as surveyed and/or calculated
(where applicable) .
36. Show field located P. R. M. /major property corner or parcel ties by station/offset
to baseline/control line (where applicable) .
37 . Current Street Names .
38. Show and identify all easements on Detail Maps by width, use and ownership
(where applicable).
