SPECIFIC PURPOSE SURVEY FOR HERON CAY MOBILE HOME PARK Topographic surveying related to mobile home sites and site drainage Indian River County , Florida All services performed under this contract shall be supervised and certified by a licensed Surveyor and Mapper registered with the State of Florida and staff employed directly with your firm . All survey procedures and final submittals shall meet or exceed Minimum Technical Standards for Specific Purpose as set forth in Florida Statues Chapter 61G17- 6 Florida Administrative code pursuant to section 472 . 027 , Florida Statues , Indian River County bases the following specifications on minimum requirements for this particular project . If the consultant so desires to modify or "enhance" these requirements all modifications shall be submitted in writing to the County Surveyor for approval prior to proceeding with task . SCOPE OF SERVICES Project Limits/understanding : 1 . Various mobile home site/lots (29) within the park boundary as outlined in exhibit "A" attached . 2 . Various drainage features as outlined in exhibit A . 3 . It is understood proper coordination with all entities involved ( I . E . County staff , park owners , mobile home owners ) is part of the tasks and shall be sufficiently completed prior to entering/accessing any properties . 4 . It is understood county staff and/or assigns shall utilize the information for the purpose of creating a report on the current conditions verses previously permitted and/or design plans . 5 . It is understood the estimated number of survey sites ( units ) is 29 and may be subject to change as the project proceeds . All invoicing shall be as per unit and Not to Exceed basis of $478 . 00 per unit/lot survey . * If site/unit surveys fall below 20 , fees shall be subject to negotiations . 6 . If "apron" openings are not made available at the agreed upon time , the return trip to obtain the elevation beneath the home will be invoiced on an hourly basis . Any delays resulting from homeowner issues will be reported to the County Surveyor, and any subsequent delays will be invoiced on an hourly basis . Specific Purpose Survey Work Order No: 2 Burdett & Assoc SECTION I : SPECIFIC PURPOSE (TOPOGRAPHIC) SURVEY : 1 . Establish adequate vertical ( NGVD 29 ) control on site to perform all tasks . 2 . Obtain sufficient elevation and measurement data on all mobile home sites and drainage features as outlined in exhibit "A" . The consultant shall use due diligence in accessing and obtaining elevations under mobile homes and drainage features , providing specific information regarding conditions , pumps and/or features 3 . The Consultant shall prepare a Specific Purpose Survey ( Sketch ) as depicted in exhibit " B" . Drawing to be prepared at a scale and sheet amounts to be determined , delineating lot number, address and topographic details of each individual mobile home site/lot . 4 . As an attachment to the drawing sheets indicated in exhibit "A" , the consultant shall prepare an informational "tabulation " of the drainage features as it relates to previous design plans . 5 . Project Deliverables shall consist of three signed and sealed 24" x 36" and three reduced 11 " x 17" versions "hardcopy" plots , reproducible PDF versions of the final product and AutoCAD 2005 format on CD . Prior to final delivery the Consultant will deliver one copy of the survey to the County Surveyor for review and approval . SECTION II : COUNTY OBLIGATIONS . 1 . The County shall organize and distribute letters of notification to the individual site owners advising them of the project and proposed tasks , obtain access approvals and confirm that individual owners shall be responsible for providing a minimum 3 foot opening along the bottom of mobile homes for proper access to terrain under the structure . 2 . The County shall advise and obtain approval from the park ownership/ management for property access prior to all tasks . 3 . The County shall supply vertical control ( Bench Marks) in the vicinity of the main entrance at time of notice to proceed . SECTION III : ADDITIONAL SERVICES . 1 . As per this work order additional services pertaining to the project in the form of meetings and additional filed or office survey task shall be performed upon prior approval of county staff and based on the accepted fee schedule for the annual survey services contract for 2005-2006 as established on September 31 , 2005 . Fee schedule attached Exhibit "C" . 2 . Furthermore if required ; Professional Surveyor and Mapper ( Marty Burdette P . S . M . ) shall provide Expert Witness testimony services (as approved prior to commencement) and shall be based on an hourly rate of $ 165 . 00 . 3 . All additional services shall be on a NOT TO EXCEED basis in the amount of $2 , 000 . 00 . SECTION IV : COMPENSATION . The COUNTY agrees to pay, and the SURVEYOR agrees to accept , a fee for services rendered according to the "Scope of Services" identified in the attached EXHIBIT "A" Fee outline . Specific Purpose Survey Work Order No: 2 Burdett & Assoc SECTION V — TIME FOR COMPLETION The Work Order No . 2 shall be completed within 26 (County) business days from Notice to Proceed . Setting time of final submittals on or before January 13th , 2006 . * . * Reliant on property and mobile home access coordination and verifications . The original AGREEMENT for annual surveying and mapping services contract No . 0390 is hereby amended as specifically set forth herein . All remaining sections of the AGREEMENT shall remain in full force and effect , and are incorporated herein . SCOPE : Burdette & Associates , Inc will obtain elevations , in the NGVD ' 29 datum , based on existing benchmarks near the entrance to Heron Cay . The homes to be surveyed are based upon a letter to Commissioner Thomas S . Lowther , from Mr. Alfred N . Andersen , dated August 31 , 2005 . The homes are lots 1 , 3 , 50 91 127 13 , 142 21 , 23 , 24 , 26 , 29 , 30 , 34 , 37 , 55 , 56 , 66 , 74 , 98 , 1279 165 , 169 , 2069 2082 226 , 229 , 178 . Unit 193 is not listed with the above , but is include in an attachment to the letter, entitled , " Heron Cay Drainage Deficiencies" . Based on our earlier discussions , we propose the following as a Schedule of Events . • Notice to Proceed • Kick Off ( Obtain Benchmark Information ) • Establish Vertical Control Network — During this phase we want to coordinate with the homeowners the location for the "apron " opening . If "apron" openings are not made available at the agreed upon time , the return trip to obtain the elevation beneath the home will be invoiced on an hourly basis .
