FOR 39TH AVENUE IRC PROJECT # 0422
This Work Order No . 2 is entered into as of this 8th day of November , 2005 ,
pursuant to that certain Continuing Contract Agreement for Professional Services entered
into as of March 15 , 2005 ("Agreement") , by and between Indian River County, a
political subdivision of the State of Florida ("COUNTY") and G . K . Environmental , Inc .
("CONSULTANT") .
The COUNTY has selected the CONSULTANT to perform the professional
services set forth on Attachment 1 , attached to this Work Order and made part hereof by
this reference . The professional services will be performed by the CONSULTANT for
the fee schedule set forth in Attachment 2 , attached to this Work Order and made a part
hereof by this reference . The CONSULTANT will perform the professional services
within the timeframe more particularly set for in Attachment 3 , attached to the Work
Order and made a part hereof by this reference, all in accordance with the terms and
provisions set forth in the Agreement . Pursuant to paragraph 1 . 4 of the Agreement,
nothing contained in any Work Order shall conflict with the terms of the Agreement and
the terms of the Agreement shall be deemed to be incorporated in each individual Work
Order as if fully set forth herein .
IN WITNESS WHEREOF , the parties hereto have executed this Work Order as of
the date first written above .

Dear Michelle,
This letter is in response to the requested proposal for the above referenced
project in Indian River County, Florida, for environmental services as generally required
for State and Federal Environmental Resource / Individual Permits . G. K.
Environmental, Inc. (GKE) will be pleased to provide this service with tasks completed
under this contract and which are briefly outlined below:
SCOPE OF SERVICES
TASK I vvomo W111and Delineation
$ 1 ,650.00
TASK I : GKE will locate / flag State and Federal regulatory wetlands in
accordance with Florida Administrative Code (F .A . C . ) 62 -340 and the .
Army Corps of Engineers (ALOE) Wetland Delineation Manual /
Technical Report Y- 874 . Delineated wetlands will be field located on a
1 " - 200' scale county blue-luxe aerial or submitted survey drawing and
included in a written report of findings . Wetlands will be assessed per
current Federal regulations and current guidelines regarding isolation .

TASK 11 : GKE will coordinate on site inspections with Federal regulatory officials
to verify jurisdictional wetland lines / locations . Federal verification of

wetland line often coincides with State verification. If written agency
verification is required, the client will provide GKE a certified survey of
GKE flag locations for transmittal to agencies for approval .
TASK III - Permit AvRlication Submittals
8 hours - $880.00
(not to exceed without prior authorization)
TASK III : GKE to assist client and client' s engineer and surveyor to complete
required field work / meet / review work with client, review drawings, and
permit application completion.
TASK IV — Mitigation, Ply,
10 hours - $ 1 , 100.00
(not to exceed without prior authorization)
TASK IV : GKE will meet with client and permit agency staff to discuss mitigation
options and inspect potential mitigation sites for compatibility and prepare
a draft mitigation plan, based upon the ecological values of wetland
impacts .
TASK V - Wetland Assessment - U +SAM / WRAP
10 hours - $ 1 , 100.00
(not to exceed without prior authorization)
TASK V . GKE will conduct a Wetland Rapid Assessment Procedure (WRAP)
and/or a Uniform Mitigation Assessment Method (UMAM) of the
wetlands located within the boundaries of the project site . WRAP/UMAM
determine a numerical value for the ecological functions and ecological
values of the existing wetlands and mitigation site. The WRAP/UMAM
score can be used to determine what type / amount of mitigation will be
required for the project site . The WRAP/CJMAM score is based on
vegetation, wildlife usage, hydrology, soils, water quality, success factor,
and adjacent lands .
TASK ; Protected Species
8 hours - $880.00
(not to exceed without prior authorization)
TASK VI : GKE will complete a detailed protected species survey on the referenced
site . GKE ecologist will conduct a preliminary systematic survey for
protected species in accordance with Florida Fish and Wildlife
Conservation Commission (FFWCC ) Guidelines. GKE ecologist will
record all observations of listed protected species and features that may

indicate species presence. Physical features may include, but are not
limited to , tree markings, burrows, tracks, nests, scat, and cavity trees .
GKE will request that the U. S . Fish and Wildlife Service (FWS) and the
Florida Fish & Wildlife Commission (FFWCC) review their files for the
presence of any listed species that may be present onsite.
TASK VII - Vogetalion Survey (F'LUCCS)
2 hours - $220.00
TASK VII : GKE will conduct a systematic survey of vegetation using the Florida
Land Use, Cover and Classlr rcation System (FL UCCS), A Technical
Report. FLUCCS was developed by the State of Florida, Department of
Administration, Division of State Planning, Bureau of Comprehensive
Planning, April 1976 . GKE ecologist will base the vegetative community
descriptions upon field survey observations, SCS soil maps, and aerial
photograph interpretation.
TASK VIII - Ubtorieal / Archaeological Res urces
2 hours - $220.00
TASK VIII : If required, GKE will coordinate historical and/or archaeological
resources with the Florida Department of State, Division of Historical
Resources, State Historical Preservation Officer ( SHPO) and request the
state to review their files for historical and/or archaeological resources
related to this area.
The completion of the above tasks will be provided to the client in report form.
TASK IX — Response to Agency Requests for Additional Information (RAI)
Estimated $ 19760.00
(not to exceed without prior authorization)
TASK IX : Following agency permit application review, GKE will respond to all
environmental questions outlined in agency letters Requesting Additional
Information (RAI letters) , Not to exceed $2 ,000 . 00 without prior
authorization from Indian River County .
GKE Fees / Compensation
Work on TASKS I through VIII outlined above could begin within fifteen ( 15)
days of G. K. Environmental, Inc. ' s receipt of Notice to Proceed, design plans, and field
staked right-of--way, Receipt of one ( 1 ) fully executed copy of this contract shall serve as
our Notice to Proceed with Tasks I through VIII . Invoices will be submitted on a

monthly basis at GKE hourly rates. GICE hourly rates (2005) are $ 110.00 / hour for
Principal Ecologist and $ 30 . 00 / hour for administrative / staff assistance .
Work on TASK IX will begin following agency receipt of application and
receipt of permitting agency requests for additional information.
The above tasks (I - IX) are not to exceed $ 8 , 650 .00 without prior authorization
from Indian River County ,
Services Not Included
The following services are not included (if required) in this agreement, but will be
coordinated and reviewed by GKE :
1 ) water quality laboratory analysis
2) surveying / engineering
3 ) Auto CAD work
4) legal work
5 ) permit drawings
6) reasonable access to subject site
If the above is acceptable, please sign below and mail or fax a copy to our
office. Work will be scheduled upon receipt of sighed authorization.

ATTACHEMENT 2 to WORK ORDER NO . 2
L COMPENSATION
The COUNTY agrees to pay and the CONSULTANT agrees to accept , a fee for
services rendered according to Attachment 1 of this WORK ORDER, with a maximum
charge under this WORK ORDER of $ 8 , 610 . 00 .
2 . PARTIAL PAYMENTS
The COUNTY shall make monthly partial payments to the CONSULTANT for all
authorized work pertaining directly to this project that was performed during the previous
calendar month . Each month, the CONSULTANT shall submit an invoice for services
performed and expenses incurred pursuant to this Agreement during the previous calendar
month .
The CONSULTANT shall submit duly certified invoices in duplicate to the
Director of Public Works Department . For lump sum line items , the amount submitted
shall be the prorated amount due for all work performed to date , determined by applying
the percentage of the work completed to the total amount due . For time and material line
items , the amount submitted shall be based on the actual hours worked and expenses
incurred for the billing period .
The amount of the partial payment due the CONSULTANT for the work
performed to date shall be an amount calculated in accordance with the previous
paragraph, less ten percent ( 10 %) of the invoice amount thus determined , which shall be
withheld by the COUNTY as retainage , and less previous payments . Per F . S . 218 . 74 (2 ) ,
the COUNTY will pay approved invoices on or before the forty- fifth day after the
COUNTY receives the CONSULTANT ' s invoice .
Per F . S . 218 . 74(2), the 10 percent ( 10%) retainage withheld shall be paid in full to
the CONSULTANT by the COUNTY , on or before the forty- fifth day after the date of
final acceptance of the Work by the Public Works Director.
No payment shall be made unless the Public Works Director has received and
approved the work products required under the " Scope of Services Provided by the
CONSULTANT", herein .

1 . TIME FOR COMPLETION
Time is of the essence for this Project and therefore, all work shall be completed
within 45 calendar days from the date of the Notice-to -Proceed .
