PROFESSIONAL SURVEYING SERVICES AGREEMENT
FOR ANNUAL SURVEYING AND MAPPING SERVICES
WITH
CARTER ASSOCIATES , INC .
Additional Services for the FPL Easement For Utility Line Relocation
This Work Order No . 2 is in accordance with the existing AGREEMENT dated
September 19 , 2006 , between Carter Associates , Inc . (SURVEYOR ) and Indian River
County (COUNTY) . This Work Order No . 2 amends the agreement as follows :
SECTTION I - PROJECT LIMITS
This Work Order 2 is for Survey for Utility Line easement at the West Regional WWTF .
The SURVEYOR'S work will be used by the COUNTY and/or the COUNTY' S Utility
Department for easement lay-out and legal description .
Limits :
Reference attached Exhibit A — Proposal from Carter Associates Inc.
1 ) Locate existing utility poles .
2 ) Locate new utility routing .
3 ) Prepare drawing and description of new overhead and
Underground easement.
SECTION II - DELIVERABLES :
SURVEYOR shall perform the additional services as
outlined .
SCTION III — TIME OF COMPLETION
a . Forty- Five (45) calendar days from notice to proceed .

SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services
rendered according to the "Scope of Services Provided by the SURVEYOR , " identified
in Section I , II and III of this WORK ORDER NO . 2 with a maximum charge under this
WORK ORDER of $2 , 740 . 00 . See proposal attached .
All invoicing shall be supported with documentation ( Man hours ) of time and
materials spent prior to payments

The AGREEMENT is hereby amended as specifically set forth herein . All remaining
sections of the AGREEMENT shall remain in full force and effect, and are incorporated
herein .

Locate existing overhead utility poles ;
Locate existing exterior limit of existing improvements near proposed plant expansion and
existing berm along west line of Tract 2;
Coordinate with and meet onsite with Florida Power and Light ( Dennis Pagano) onsite for new
pole locations and layout;
Review and utilize digital plans and North American Datum of 1983, Florida state plane
coordinates ( East zone-901 ) provided by PBSU for proposed easement routing ;
Prepare sketch and description of new proposed utility installation for FP&L.
