In accordance with Contract No. 0390 2005-2006 Project: Laconia Street Extension Project, Topographic/Route/Aerial Mapping Survey From County Road 510 to Concha Avenue/ the City of Sebastian Indian River County Project No. 0523 March 14, 2006 This Work Order No. 3 is in accordance with the existing AGREEMENT dated September 23rd , 2005 between Creech Engineers, Inc. (SURVEYOR) and Indian River County (COUNTY). This Work Order No. 3 amends the agreement as follows: SECTION I — PROJECT LIMITS From the existing Intersection of Concha Avenue and Laconia Street in the City of Sebastian traveling south beyond the intersection of County Road 510. Total length plus or minus 3,400 feet. SECTION II — SCOPE OF SERVICES This Work Order No. 3 is for providing a Topographic/Route/Aerial mapping Survey for the purpose of planning and design of the proposed new corridor. Services shall include all related field and office professional Surveying and Mapping procedures to accomplish all tasks as outlined in the attached (Exhibit "A") project limits aerial photograph and Scope-of-Services submitted to the County by Creech Engineers, Inc. SECTION III - TIME FOR COMPLETION 90 Calendar days from Notice to Proceed . SECTION IV — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered according to the all items as defined in Sections 1 , II , III and IV of this Work Order No. 3, a Lump Sum of $36 . 000. 00 All invoicing shall be supported with documentation (Man hours) of time and materials spent and following information prior to payments. Invoices shall include the work order, contract project number, invoice number related to this work order, original contract amount, current billing amount and retainage amounts. (CEI ) is pleased to submit this revised proposal to Indian River County (COUNTY) for professional surveying and mapping services. PROJECT UNDERSTANDING It is our understanding that the COUNTY is extending Laconia Street south across the Saint Sebastian River (South Prong) to C. R. 510 (Wabasso Road) . To help accomplish this, the COUNTY is requesting professional surveying and mapping services to perform a Route / Design Survey. The project limits are from 250' south of the southern Right of Way of C . R. 510 to 150' north of the Laconia Street intersection with Concha Avenue, for an overall project length of approximately 3400 feet. SCOPE OF SERVICES We respectfully submit the following Scope of Services and Fee Schedule for your review: Task 1 — Research / Planning • CEI will research local published records provided by the COUNTY, along with in house records gathered during the course of recent projects performed in the area by CEI for the Florida Department of Transportation District 4 ( FDOT4). Laconia Street Route / Design Survey Project Specific No. 0523 Proposal for Professional Surveying Services Revision 2 23101 .02 (00) March 6, 2006 Page 2 of 4 Task 2 — Primary Control • Primary Horizontal Control — CEI will establish a horizontal control network using Static GPS methods. The network will meet the FDOT4 requirements for Primary Horizontal Control based on the FDOT4 Memorandum titled FDOT DISTRICT IV GLOBAL POSITIONING SYSTEM SURVEY STANDARDS FOR HORIZONTAL NETWORKS, dated April 11 , 2003. CEI Crews will reconnoiter published horizontal control surrounding the project corridor. CEI will recover existing FDOT4 horizontal control from the C. R. 510 project. • Primary Vertical Control - CEI will establish a Vertical Control Network using 3-wire differential leveling technique relative to the NGVD29 elevations for the benchmarks provided by the COUNTY. The network will also be adjusted relative to the NAVD88 based on published NGS and FDOT4 values for local control. The network will meet the FDOT4 requirements for Primary Vertical Control based on the current FDOT Location Survev Manual. Task 3 — Survey Baseline • CEI will establish a survey baseline the length of the corridor for the project relative to the west line of Section 25, Township 31 South Range 38 East. Task 4 — Locations • All structures and improvements will be located within 200' east of the east canal bank of the Lateral "C" Canal running along the west section line from the southern project limits north to the Saint Sebastian River ( South Prong ). • All structures and improvements will be located within the RNV of Laconia Street, within 80 feet west of the west RNV, and within 25 feet east of the east RNV of Laconia Street from the north side of the Saint Sebastian River (South Prong) north to the project limits and east +/- 150 feet along Concha Avenue within limits of RNV. Specifically this will include the finished floor elevations of houses along the corridor and improvements within Lot 1 Block 364 of Sebastian Highlands Unit 11 • CEI will locate only the limits of the orange groves within these limits. Ground measurements will be made within the grove rows sufficient only to provide a typical row profile. Task 5 — Cross Sections ■ Cross sections every 100' will be located within 200' east of the east canal bank of the Lateral "C" Canal running along the west section line from the southern project limits north to the Saint Sebastian River ( South Prong). Also cross sections every 100' will be located within the RNV of Laconia Street, within 80 feet west of the west R/W, and within 25 feet east of the east RNV of Laconia Street from the north side of Laconia Street Route / Design Survey Project Specific No. 0523 Proposal for Professional Surveying Services Revision 2 23101 .02 (00) March 6, 2006 Page 3 of 4 the Saint Sebastian River (South Prong) crossing north to the project limits and east +/- 150 feet along Concha Avenue within limits of R/W. Task 6 — Canal Cross Sections ■ CEI will measure cross sections of the Lateral `C" Canal every 500' running along the section line from the southern project limits north to its connection with the Saint Sebastian River ( South Prong). Task 7 — Areas of Environmental Concern • CEI personnel will locate the apparent upland vegetation lines and the apparent high water mark (debris line) within and crossing the corridor. These locations will only be approximate for purposes of estimating areas of environmental concern . Task 8 — Aerial Mapping Control • CEI will set approximately six panels in support of aerial mapping. CEI will establish horizontal and vertical control values on the five panels to accuracies sufficient to control aerial mapping and establishment of 1 foot contours. Task 9 — Photogrammetry • CEI shall contract with Pickett & Associates, Inc. to provide professional surveying and mapping services for this project. Please see attached contract for scope items. Task 10 — Deliverables ■ CEI will provide three (3) signed and sealed hardcopy survey maps in accordance with the proposal requirements , and with the "Minimum Technical Standards" for surveying and mapping in the State of Florida as set forth by the Florida Board of Professional Surveyors and Mappers in Chapter 61G17-6, Florida Administrative Code, pursuant to Section 472-027, Florida Statutes. ■ CEI will also provide the drawings in AutoCad® version 2001 or later electronic format. Information and/or Services NOT Included : This proposal does not include any fee for: a) Additional data formatting and submittal to agencies other than specifically stated herein. b) Underground utility location . c) Supplemental surveying of topographic features in obscured areas.
