FOR PROFESSIONAL GEOTECHNICAL SERVICES AGREEMENT
FOR ANNUAL GEOTECHNICAL SERVICES CONTRACT NO . 0534
WITH
CTI — CONSTRUCTION TESTING & INSPECTION , INC .
All services performed under this contract shall be supervised and certified by a licensed
Professional Engineer (PE) registered with the State of Florida and staff employed directly with
your firm .
Scope of Services
All work shall be performed on a unit price basis in accordance with the attached Scope of
Services dated January 2 , 2007 and shall comply with Standards enumerated in the RENEWAL
OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT IRC PROJECT NO. 0534 WITH
CTI , INC . Effective Date: October 1 , 2006.
Minimum Specification Requirements as follows:
• In-place moisture density testing of embankment fill , pipe backfill , stabilized sub
grade , base course, and curb pads by Nuclear Method (AASHTO-T238) .
• Laboratory testing of soils and aggregates including Proctors, Limerock Bearing
Ratio (LBR) , soil classification , carbonate content (base) , and other such specified
QC Tests .
• Standard laboratory testing of concrete sidewalk and endwalls.
• Preparation and issuance of engineering reports presenting the test results and
their compliance with the project specifications .
All work shall also comply with the current Florida Minimum Technical Standards.

COMPENSATION .
The COUNTY agrees to pay, and the Geotechnical Professional agrees to accept , a fee for
services rendered according to the "Scope of Services" identified in the attached EXHIBIT "A"
Fee Schedule and Man Hours .
A Not-to-Exceed Amount of: $ 1 , 994 . 00 to be paid on a unit price basis for actual work
performed in accordance with the schedule of services and fees set forth in Exhibit A of
RENEWAL OF THE ANNUAL GEOTECHNICAL SERVICES CONTRACT, IRC PROJECT NO.
0534 WITH CTI , INC . Effective Date: October 1 , 2006.
TIME FOR COMPLETION
This Work Order No . 3 shall run concurrently with the construction work for the
Indian River Drive South Sidewalk Improvements
