WORK ORDER NO. 4 ( Engineering/Surveying )
FOR
PROFESSIONAL SERVICES AGREEMENT
FOR ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
KIMLEY-HORN and ASSOCIATES , INC .
In accordance with Contract No. 0725
This Work Order No . 4 is in accordance with the existing AGREEMENT dated November 6 ,
2007 , between Kimley-Horn and Associates, Inc. (SURVEYOR) and Indian River County
(COUNTY) . This Work Order No . 4 amends the agreement as follows :
SECTION I — PROJECT LIMITS
( Reference attached map)
The SURVEY LIMITS are as follows : The existing +/- 1 . 67 acres situated along the south side of
CR510, east of the Indian River surrounded by the proposed Ocean Sands Development,
(tax parcel 31392600000002000001 . 0) Indian River County, Florida.
SECTION II — SCOPE OF SERVICES
The scope of services for this Work Order No. 4 is the preparation of a Boundary Survey ,
updating the previous Boundary Survey by Kimley-Horn and Associates , Inc. Job Number
047371001 .
The survey shall depict the boundary as per deed of record (supplied by the County) , all above
and obvious below ground improvements and monitoring wells in the near proximity of the
existing cattle trough along the northern 40 feet of the parcel .
Task does not include topographic features , trees or monitoring wells lying outside the areas
mentioned above.
SECTION III - TIME FOR COMPLETION/DELIVERABLES
a) Project shall be completed within thirty (30) business days from Notice to Proceed .
b) Deliverables shall be certified to Indian River County, include three (3) signed and
sealed 24" x 36" AND three (3) reduced 81 /2" x 11 " — " legible" versions (for recording) ,
electronic copies in AutoCAD 2007 and PDF formats.
c) The SURVEYOR' S work product shall meet or exceed the minimum standards as
defined by Section It and made a part of this agreement , or the COUNTY will not
approve the SURVEYOR' S request for payment.

SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept , a fee for services
rendered according to the "Scope of Services" identified in Section I , II and III of this
Work Order No . 4, a Lump Sum of $ 1 , 500 . 00 .
All and/or any additional services not described hereon shall be pre-approved by the
COUNTY. Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year(s ) of November 6`h , 2007
through November 5" , 2009 . All invoicing and payments shall be in accordance with the
original Professional Surveying Services Agreement for Annual Surveying and Mapping
Services , Contract No . 0725 with the COUNTY and as stated in Section III and IV hereon .
The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREEMENT shall remain in full force and effect, and are incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this 18th day
of Dec . 2007 .
