Home My WebLink About2006-249 WORK ORDER NUMBER 4 �' y a q This Work Order Number 4 is entered into as of this 111 day of July 200 6 ("Effective Date") pursuant to that certain Continuing Contract Agreement for Professional Services entered into as of April 6, 2004 ("Agreement"), by and between Indian River County, a political subdivision of the State of Florida ("COUNTY") and Masteller and Moler, Inc. ("CONSULTANT") . The COUNTY has selected the CONSULTANT to perform the professional services set forth in Attachment 1 (Scope of Work), attached to this Work Order and made a part hereof by this reference. The professional services will be performed by the CONSULTANT for the fee schedule set forth in Attachment 2 (Fee Schedule), attached to this Work Order and made a part hereof by this reference. The CONSULTANT will perform the professional services within the timeframe more particularly set forth in Attachment 3 (Time Schedule), attached to this Work Order and made a part hereof by this reference all in accordance with the terms and provisions set forth in the Agreement. Pursuant to paragraph 1 .4 of the Agreement, nothing contained in any Work Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in each individual Work Order as if fully set forth herein. Fe ssis ant oun Attorney Our File #0627LA June 16, 2006 NON-EXCLUSIVE PROFESSIONAL ENGINEERING AGREEMENT CONTINUING CONSULTING SERVICES ATTACHMENT 1 • Project Description Work Authorization #4 provides for professional engineering services for preparation of a plan of action and tasks to convert an 8" wastewater force main into a reuse main. Masteller & Moler, Inc. will utilize its expertise on wastewater force main conversion into reuse mains relative to the 8" pipeline between the North and Central WWTP along Lateral G Canal. We will conduct site visit(s); prepare plans, specifications and bid documents to transform the force main into a reuse main including: cleaning, disinfecting, leak detection, valve checking and hydrostatic testing. Included in our services will be detail plans showing the piping connection to the 12 " reuse line at the North County WWTP site and the 16" reuse line at the Central County WWTP site. We will also prepare the FDEP permit application as well as bidding and services during construction. The Estimated Cost of the Project is $275,000. 00 A brief description of the scope of services and deliverables for Work Authorization 94 is provided below: • Scope of Services Task A — Contract Plans/Specifications Masteller & Moler, Inc. will meet with County staff, conduct site visits along the existing 8" force main route; prepare construction plans; project specifications; prepare a plan of action; and prepare bid documents to convert the existing 8" force main into a reuse main. Contract plans will show the method of connection to the existing reuse system piping on the North Regional and Central Regional Wastewater Treatment Plant ends of the pipeline. Our services will include preparation of permit applications. Observations of pipe cleaning, disinfection, leak detection, valve checking and hydrostatic testing will be conducted by our staff to assure the County Utilities is getting a good job.
