Additional Services
This Work Order No . 4 is in accordance with the existing MASTER AGREEMENT
dated August 9th , 2005 between Nick Miller, Inc. (SURVEYOR) and Indian River
County (COUNTY). This Work Order No. 4 amends the agreement as follows :
Provide professional Surveying and Mapping services for the purpose of professional
consultation , planning , research , GIS/AutoCADD mapping , data sheet creations and other
related services as needed to complete anticipated tasks County Vertical Control Mapping as
outlined in Sections I , II , III and IV hereon .
SECTION I — PROJECT LIMITS
County wide.
SECTION II — SCOPE OF SERVICES
This Work Order No. 4 is for services to perform additional services to the original
scope of services outlined in Work order No. 3.
These additional services are to scan and create a PDF file of each Bench Mark
sheet as it appears in the completed IRCVCN Bench Mark book and GIS data base.
The PDF document will be linked to the GIS data base/map with a hyperlink function
within the ArcMap GIS map application .
Services shall include all related research , consultation , meetings, field and office
professional Surveying and Mapping procedures to accomplish all tasks as outlined in
the attached ( Exhibit "A") hereon .

SECTION III — DELIVERABLES
As outlined in the attached ( Exhibit "A") hereon . A preliminary submittal of the map
and data sheet formats shall be provided to the County for review and approval prior
to final applications .
SECTION IV - TIME FOR COMPLETION
Scope of Services as indicated in Section II hereon and Items 1 through 4 of the SURVEYORS
scope of services (exhibit A) shall be completed within 20 calendar days of the completion of the
original ; Work Order No. 3 task.
Note : Time for completion is reliant on the county review and comments of the preliminary
submittal .
SECTION V — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for
services rendered according to the all items as defined in Sections Il , III and IV of
this Work Order No . 4 , LUMP SUM of $2, 000. 00

The AGREEMENT is hereby amended as specifically set forth herein . All remaining
sections of the " MASTER AGREEMENT' dated the 9th day of August, 2005 by and
between INDIAN RIVER COUNTY, shall remain in full force and effect, and are
incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this

Indian River County Benchmark Database Update
Scope of Services
Nick Miller, Inc. will create a new field within the Database created under Work Order No. 3 . The new
field will contain a link to the Benchmark within in the Benchmark Inventory Book. The link will be
activated by the end-user turning on the Hyperlink function in ESRI's ArcMap and ArcReader
software suites . The Hyperlink function is turned on by clicking the mouse on the Hyperlink Icon .
Any Monument that the user then clicks on will open a PDF of the corresponding page in the
Benchmark Inventory Book.
Nick Miller, Inc. will complete the following work items :
1 . Create a PDF file of each page as it appears in the Benchmark Inventory Book created under
Work Order No. 3 .
2. Create a new field in the Database that will contain a link to the correct PDF file for each
Benchmark in the Database.
3. Create the links in the Database between the Benchmarks and the PDF files.
4 . Setup the Hyperlink function in ArcMap to work with the new field .
Nick Miller, Inc. will complete the described work items once the County has approved the Index and
the Datasheets created under Work Order No. 3 .
The county will provide the following items to Nick Miller, Inc. :
1 . Approval to proceed with task once the Index and Datasheets created under Work Order No .
3 have been finalized and approved .
The fee for these services shall be Two Thousand ($2,000) dollars .
