PROFESSIONAL BRIDGE DESIGN SERVICES AGREEMENT
FOR ANNUAL BRIDGE DESIGN SERVICES CONTRACT
WITH
Bridge Design Associates, Inc.
Indian River County Project No. 0107-B
This Work Order No. 5 is in accordance with the existing AGREEMENT dated October 3 ,
2006 between Bridge Design Associates, Inc. (ENGINEER) and Indian River County
(COUNTY).
This Work Order No. 5 amends the agreement as follows:
SECTION I - PROJECT LIMITS
53'd Street Bridge over I.R.F.W.C .D. Lateral H Canal
SECTION H - SCOPE OF SERVICES
This Work Order No. 5 is for providing Construction Phase Service (Construction Engineering
and Inspection) for the above mentioned project.
SECTION III - TIME FOR COMPLETION
During Bidding and Construction Phases.
SECTION IV - COMPENSATION
The COUNTY agrees to pay, and the ENGINEER agrees to accept, a fee for services
rendered according to the "Scope of Services" identified in Section II and III of this Work
Order No. 5, Hourly Not To Exceed $30,883 .00.

The AGREEMENT is hereby amended as specifically set forth herein. All remaining sections
of the AGREEMENT shall remain in full force and effect, and are incorporated herein.

Pursuant to your request, Bridge Design Associates, Inc. can provide the necessary
construction phase services at the above referenced project site. We propose to provide
these services on an hourly, not to exceed basis_
The attached Exhibit "A" is a manhour breakdown of tasks with commensurate fees .
