This Work Order No. 5 (Engineering) is in accordance with the existing AGREEMENT dated
September 23rd, 2005 between Carter Associates, Inc. (SURVEYOR) and Indian River County
(COUNTY).
This Work Order No. 5 (Engineering) amends the agreement as follows:
SECTION I — PROJECT LIMITS:
Boundary and surrounding features of: The County owned lands known as South County Oslo
Transfer Station Site. Bordered by the South Main Relief canal and Lateral "G". Project also includes
the County owned lands under the utilities department along the south side of the main site.
Reference attached exhibit "A" aerial photograph of site.
SECTION II — SCOPE OF SERVICES:
This Work Order No. 5 (Engineering) is for providing all Professional Survey services related to the task
and services as indicated in the attached exhibit `B" Proposal and scope of services supplied by Carter
Associates, Inc.

SECTION III — TIME FOR COMPLETION:
All tasks outlined shall be completed, in its entirety within 90 calendar days of notice to proceed.
* Reliant on aerial photography applications and acquisition of adequate photographs. *
The County shall be notified upon completion of tasks and/or of any delay beyond specified periods.

SECTION IV — COMPENSATION:
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services rendered
according to the "Scope of Services" identified in Section I, II and III of this Work Order No.5
(Engineering), a Lump Sum of $34,720.00.
All invoicing

AGREEMENT shall remain in full force and effect, and are incorporated herein.

As requested, Carter Associates, Inc. is pleased to submit the following proposal for the performance and
preparation of a Boundary and Topographic survey on the above referenced parcel. Said referenced parcels
being a portion of the southwest one-quarter of Section 13, Township 33 South, Range 39 East and a portion
of the northwest one-quarter of Section 24, Township 33 South, Range 39 East located south of the Indian
River Farms Water Control District "South Relief Canal" and east of Indian River Farms Water Control
Districts "Lateral J Canal" Indian River County, Florida.
Our firm is located in Vero Beach, Florida. and has been in existence since 1911.
Carter Associates, Inc. has a long standing history of competent; reliable, and professional land surveying
services within the area. With its many years of experience in the field of surveying, land development,
municipal and other government areas, Carter Associates, Inc. has kept abreast and amassed the technical
knowledge and expertise to perform and complete the tasks as set forth in this proposal. We have assembled
a professional team, each with thorough knowledge and experience in the field of surveying and mapping.
Over the years, Carter Associates, Inc. has performed extensive survey projects in and around the City of
Vero Beach and Indian River County.
Our surveying services included in this proposal will consist of the following tasks as outline in the request
for proposal. It is our intent to meet all of the County requirements as listed in the furnished scope of
services:
Boundary survey of two parcels (combined) being a portion of the southwest one-quarter of Section
13, Township 33 South, Range 39 East and a portion of the northwest one-quarter of Section 24,
Township 33 South, Range 39 East including surveying and mapping of the existing
tower/communications sites (2) lease lines within main parcel and monumentation of such. All
"exterior" boundary corners to be monumented with concrete monuments (disc to be supplied by
Indian River County). Project shall be projected and provided in Florida East (Zone 901) state plane

coordinates an be relative to the North American Datum of 1983. Adjustment of 1999. State piane
coordinates values will be provided on a minimum of three corner monuments.
Title Company (to be obtained by the consultant) to prepare and furnish 30 near title search report
with supporting documentation to Carter Associates. Inc. Review and plotting of relevant
information including but not limited to ownership and Easements/Encumbrances, from said 30 -near
title search report. Supporting documentation shall be furnished to Indian River County for these
parcels. Survey shall indicate title report identifying number, date and related scheduled "B" items
that affect property (As applicable).
Prepare and furnish current color ortho-rectified aerial photograph of the project site in glossy paper
and digital format. Aerial photography to be performed and prepared by Pickett Aerial Surreys, a
division of Pickett & Associates, Inc. (to be obtained by the consultant).
Vertical Control Survey and Topographic survey of the entire site with one -foot contours, including
spot elevations on a 100 foot grid and at all grade changes and unusual topographic features. Limits
of topographic data will extend 150 feet beyond the property lines (excepting subdivision
improvements along the westerly property line). Topography to be referenced to the National
Geodetic Vertical Datum of 1929 with benchmarks provided on a minimum of three corner
monuments.
Right-of-way positioning/mapping and Cross-sections in relationship to the boundary survey of the
Indian River Farms Water Control Districts "Lateral J" and "South Relief Canals" at 100 -foot
intervals, f/- from Right -of -Way to Right -of -Way line and/or as obtainable in open/non-vegetative
areas.
Locations, canal Right -of -Way alignment positioning/mapping and Cross sections of the project site
access road from the south Right-of-way of Old Dixie Highway to the east boundary of the project
site at 100 foot intervals
Locate all improvements within the two parcels, within five feet and/or along property lines and +,'-
150 feet of the exterior limits of overall parcel (excepting subdivision improvements not directly
along the westerly property line).
Locate all above ground visible utilities including researching the existing tower/communications
underground installations
Locate all protected hardwood tree, pine trees and cabbage palms with a diameter of 6 inches and
greater and the exterior limits of dense vegetation.
Deliverables:
Three hardcopy versions of the boundary and improvements survey alone.
Three hardcopy versions of the boundary/improvements/top graphic/contour data (Without aerial)
Three hardcopy versions of the boundary,/improvements/topographic/contour data (With aerial)
CD/electronic file of all mentioned versions in Autocad 2005 or compatible version to the county.
Our survey will be performed and prepared in accordance with the survey requirements of the Minimum
Technical Standards (M.T.S.) set forth by the Board of Professional Surveyors and Mappers, pursuant to
Section 472.027, Florida Statutes and Chapter 6 1 G 17, Florida Administrative Code.
Based on our review of the existing survey data, property appraiser's maps and the existing conditions of the
site, we estimate the cost to perform the required field tasks and prepare the necessary survey certificates to
be the following:
