This Work Order No . 5 is in accordance with the existing AGREEMENT dated September 23`d
2006 between Creech Engineers , Inc. (SURVEYOR) and Indian River County (COUNTY). This
Work Order No. 5 amends the agreement as follows:
SECTION I — PROJECT LIMITS
Various locations throughout the entire County .
SECTION II — SCOPE OF SERVICES
This Work Order No . 5 is for providing a Professional Geodetic Surveying Services as
specifically outlined in the attached " Proposal for Professional Geodetic Surveying Services"
provided by Creech Engineers Inc. ( SURVEYOR ) dated July 30 , 2007 .
Services shall include all related field and office professional Surveying and Mapping
procedures to accomplish all tasks and COUNTY objectives as outlined in the attached
mentioned proposal and Scope-of-Services submitted to the COUNTY by Creech Engineers ,
Inc.
SECTION III - TIME FOR COMPLETION
As provided in attached scope of services.
SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, a fee for services
rendered according to all the items as defined in Sections I , II , III and IV of this Work Order
No . 5 , a Lump Sum of $2 , 050 . 00 per control monument. Fees are based on a minimum of
fifteen ( 15 = $31 , 750 . 00 ) and a maximum of twenty five (25 = $ 51 , 250 . 00) completed control
points . Total not to exceed $51 . 250 . 00

All invoicing shall be supported with documentation on a per monument basis and materials
spent and the following information prior to payments: invoices shall include the work order,
contract project number, invoice number related to this work order, original contract amount,
current billing amount and retainage amounts.
The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections
of the AGREEMENT shall remain in full force and effect, and are incorporated herein .

Proposal for Professional Geodetic Surveying Services
Dear Mr. O'Brien :
CREECH ENGINEERS, INC. (hereinafter referred to as "SURVEYOR") is pleased to submit
this proposal for professional surveying services to Indian River County (hereinafter referred to
as "COUNTY") for the GPS Densification Project 2007 located in Indian River County, Florida
(hereinafter referred to as "PROJECT").
It is the SURVEYOR'S understanding that the COUNTY is adding to, and strengthening the
existing published geodetic control in and around Indian River County for surveying and GIS
purposes. To accomplish this task, the COUNTY requested professional surveying and
mapping services to measure and Blue Book, using Global Positioning System (GPS)
technology, a minimum of fifteen monuments and a maximum of thirty (30) monuments. It is
anticipated that 50% of these monuments will be new monuments and 50% will be existing
unpublished stations . The resulting geodetic control network will be Blue Booked and submitted
to the National Geodetic Survey (NGS) for inclusion into the National Spatial Reference System
(NSRS) .
SCOPE OF SERVICES
Task 01 :
Research /Research / Planning
The SURVEYOR will research local published horizontal control near the project area, and
prepare a proposed GPS Network map for review by the COUNTY. Adequate coordination with
Call Sunshine - underground locates will be performed for each location assuring clearance of
underground utilities of conflicts prior to any excavations .

Reconnaissance
The SURVEYOR will reconnoiter published control, along with existing county stations to be
Blue Booked. The SURVEYOR will prepare field notes and take digital photography of network
stations.
Proposed GPS Network Submittal
The SURVEYOR will design a GPS Network and prepare a proposed GPS Network submittal
for review and approval by Ronnie Taylor and the NGS.
Monumentation
The SURVEYOR will establish new concrete bell-bottom monuments of sufficient stability to be
Blue Booked in the GPS network or perform GPS measurements on existing Indian River
County monuments. New monumentation shall be established as inter- visible "pairs" either as
a new pair or adding to an existing monument. All network stations will be referenced with three
(3) or more witness monuments, and a flexible survey witness post.
A minimum of fifteen (15) monuments and a maximum of twenty-five (25) monuments are
included in this contract.
GPS Measurements
The SURVEYOR will use static GPS measurement methods to create a closed geodetic
network. This network will have sufficient redundancy to achieve the required project accuracy
of First Order as stated in the FGCC publication Geometric Geodetic Accuracy Standards and
Specifications for Using GPS Relative Positioning Techniques version 5. 0 dated May 11 , 1988,
reprinted with corrections August 1 , 1989. GPS measurements will be performed using up to
four (4) Trimble® Dual Frequency GPS receivers. GPS observations will be documented by
either sketch , rubbing or photograph.
Blue Book Submittal
The SURVEYOR will perform computations and adjustments necessary for proper Quality
Control / Quality Assurance as they relate to the required accuracies. The SURVEYOR will
prepare Blue Book digital files, field documentation, and reports necessary for submittal to, and
eventual publication by NGS according to the latest NGS Blue Book specifications: Input
Formats and Specifications of the National Geodetic Survey Data Base VOL. 1, dated 1994 and
1998. The horizontal coordinates will be fumished to the COUNTY in U. S. Survey feet; Florida
State Plane East Zone (0901 ) relative to the North American Datum of 1983 (NGS adjustment
of 1990 or 1999) . The SURVEYOR will prepare a Blue Book project report relating the methods
used during the survey and will submit survey for review and publication by NGS.

The SURVEYOR will prepare a signed and sealed survey map relating the methods used
during the survey, accuracy achieved, and a list of final coordinates in accordance with the
"Minimum Technical Standards" for surveying and mapping in the State of Florida as set forth
by the Florida Board of Professional Surveyors and Mappers in Chapter 61G17-6, Florida
Administrative Code, pursuant to Section 472-027 , Florida Statutes.
DELIVERABLES:
1 . ) Supply one ( 1 ) signed and sealed 24" x 36" reproducible Mylar set and one ( 1 ) paper
version. Two (2) sets 11 " x 17" (ledger) bound and marked final drawings. One ( 1 )
CD-Rom containing all electronic files relating to this project (in AutoCAD release
2004 or newer version) and PDF format
2. ) Raw data reports of all observations and adjustments pertinent to final product.
3. ) Copy of all field notes pertinent to final product.
4. ) Sheet outline:
a. Survey cover sheet will be titled in a similar fashion — Indian River County,
GPS/Geodetic Re- Densification Survey 2007 . Drawing and information shall
depict monument positions in relationship to underlying County Map as
supplied or obtained , with the additional street identifications, landmarks,
rivers , etc. This sheet shall include the preparing Surveyor's Certifications
and an additional note prominently placed: "For reproductions of this survey
contact the Indian River County Surveyor . County Surveyor's name and
contact information shall be included .
b. An overall control sheet(s) map with a sketch of the control network which
consist of a graphic representation of the three-dimensional vector baseline
as established between controlling stations and newly established stations. A
tabular listing of coordinates obtained from the final fixed (constrained)
adjustments of all stations surveyed or used, as control shall be contained on
the control sheet or referenced to another sheet.
c. The listing shall include latitudes, longitudes; state plane coordinates
(S. P. C . ), Northing and Easting with S. P.C. zone and units clearly labeled (i .e.
"meters" or U . S . survey foot" , convergence angles and scale factors). An
additional column shall be accommodated for the insertion of the recorded
PID or other recorded identifiers. The values for the fixed control stations
shall be the published values and identified as such. The number of decimal
places displayed should reflect an appropriate level of precision for work
performed and the positional accuracy shall be stated.
d. The receiver make and model number as well as the software brand name
and version number used for baseline processing and geodetic adjustment
computation shall be identified. Dates representing the span of time during
which field observations were taken shall be listed and must be clearly stated
on the sheet(s) as to the order and class of survey in accordance with FGCC
standards and specifications. The control sheet(s) or referenced sheet(s)
shall contain any other pertinent survey data, survey notes, legend ,
abbreviations, bar scale, etc.
e. Individual station sheets (separate re-set and re-observations) depicting one
(or two) monuments with Go-To descriptions, individual data tabulation for

each point (as identified above), and sketch of adequate and sustainable
reference points (Min 3) as to allow for the re-establishment of the position
without instrumentation (distances and bearings) as set in the field. The
COUNTY has the option to modify the individual sheets as to enhance or
improve upon outline.
5. ) The COUNTY shall prepare its Work Product so that it can be readily used by the
COUNTY or other consultants for a variety of purposes. It shall contain all
information necessary for an independent third-party surveyor or entity to
independently locate and utilize the I . R. H.C. N. (GPS) control network. All final
products, drawings, files or information related to this project shall become property
of Indian River County and remain so for the use of the County and Public at large.
TIME OF COMPLETION
The SURVEYOR shall complete the field Monumentation and GPS monumentations within
eight (8) weeks of Notice to Proceed from the COUNTY. A preliminary submittal to the
COUNTY and the NGS will be completed the second week of December, 2007 . Final
acceptance by the NGS and final deliverables to the COUNTY are anticipated to be completed
by January 30, 2008.
ADDITIONAL SERVICES
The SURVEYOR will provide, as requested , additional services that require analyses beyond
those described in the above task(s) . These additional services may include, but not limited to:
A. Additional surveying services;
B. Additional data formatting and submittal to agencies other than specifically stated
herein ;
C. Access to restricted areas, or remote areas requiring special access and/or security
clearance.
INFORMATION AND /OR SERVICES PROVIDED BY THE COUNTY
The following information and/or services shall be provided to the SURVEYOR by the COUNTY
or his representative:
A. Florida Department of Environmental Protection (FDEP) "go to" files;
B. Map of monuments to be Blue Booked.
FEES AND BILLING
This proposal, as well as the attached General Contract Provisions , represents the entire
understanding between the COUNTY and the SURVEYOR concerning the PROJECT. The

Lump Sum amount will be determined based upon the amount of monuments submitted
to the NGS for Blue Book purposes. Each monument or station submitted will be Two
Thousand Fifty Dollars ($2,050.00). This contract is based upon a minimum of fifteen (15)
monuments and a maximum of twenty-five (25) monuments. Therefore, fifteen (15)
monuments would be a fee of Thirty Thousand Seven Hundred Fifty Dollars ($30, 750.00)
and twenty five (25) monuments would be Fifty One Thousand Two Hundred Fifty
Dollars ($51,250.00) and quantities between fifteen (15) and thirty (30) will be based upon
the aforementioned Two Thousand Fifty Dollars ($2,050.00) per monument Billing will be
invoiced PER MONUMENT and on a monthly based on percentage complete for Lump Sum
Tasks and for services actually provided for Time and Material (hourly) Tasks. Invoices are
payable as per the original Professional Surveying and Mapping contractual agreement between
the COUNTY and the SURVEYOR. All remittance should be sent to Creech Engineers, Inc. ,
P. O. Box 327, Stuart, Florida 34995-0327.

Cancellation of any individual lump sum task by COUNTY prior to completion shall be subject to
payment for time and expenses incurred up to point of cancellation.
It is understood and agreed that the amounts of money applicable for the performance
hereunder shall remain in full force and effect up to and including April 1 , 2008. After said
date, the SURVEYOR shall have the right, but not the obligation, to increase any and all fees
and expenses contained herein to such levels as may be currently established for similar
performances at that point in time.
SUMMARY AND AUTHORIZATION
The SURVEYOR strives to provide superior, personalized, professional service to our clients.
We appreciate this opportunity to present our proposal to the COUNTY.
General Contractual Provisions are as per the "Professional Surveying Services Agreement for
Annual Surveying and Mapping Services" dated September 16, 2003 and renewed September
19, 2006 between the COUNTY and the SURVEYOR.
