This Work Order Number 5 is entered into as of this 20t1 av of March 200 7
("Effective Date") pursuant to that certain Continuing Contract Agreement for Professional Services
entered into as of April 6, 2004 ("Agreement"), by and between Indian River County, a political
subdivision of the State of Florida ("COUNTY'S and Masteller and Moler, Inc
("CONSULTANT") .
The COUNTY has selected the CONSULTANT to perform the professional services set forth
in Attachment 1 (Scope of Work), attached to this Work Order and made part hereof by this
reference. The professional services will be performed by the CONSULTANT for the fee schedule set
forth in Attachment 2 (Fee Schedule), attached to this Work Order and made a part hereof by this
reference. The CONSULTANT will perform the professional services within the timeframe more
particularly set forth in Attachment 3 (Time Schedule), attached to this Work Order and made a part
hereof by this reference all in accordance with the terms and provisions set forth in the Agreement.
Pursuant to paragraph 1 . 4 of the Agreement, nothing contained in any Work Order shall conflict with
the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in
each individual Work Order as if fully set forth herein .
IN WITNESS WHEREOF, the parties hereto have executed this Work Order as of the date
first written above.

CONTINUING CONSULTING SERVICES

• Project Description
Work Order No. 5 provides for professional engineering services for the surveying, design, permitting,
bidding and services during construction of a 12" reuse main for conveyance of reuse water in 43rd
Avenue between 13th Street SW & 25' Street SW.
Work Order No. 5 is being implemented at this time to provide professional engineering services by
Masteller & Moler, Inc. associated with the project described above. The work to be performed as part
of this Work Order will generally include preparation of project design for the proposed 12" reuse
pipeline, preparation of contract plans; preparation of specifications and bid documents; preparation and
submittal of permit applications, assistance during bidding and contract administration/periodic
inspection for the pipeline project.
The estimated project construction cost is $500,000.00
A brief description of the scope of services for Work Order No. 5 is provided below:
• Scone of Services
Task A — Route Survey and Base Plans
Masteller, Moler, Reed & Taylor, Inc. will perform Professional Land Surveying services for the
completion and preparation of a route survey and base plans of 43`d Avenue from the intersection of 250'
Street SW north to 13d' Street SW to include the following:
1 . Establish survey baseline and tie to state plane coordinates.
2. Locate and identify all monumentation for establishment of existing rights-of-way.
3 . Locate and identify all visible topographic features along the route. Improvements including but
not limited to the location of existing pavement, driveways, culverts, power poles, mailboxes,
fences, trees, etc.
4. Obtain elevations along the route of improvements sufficient for engineering design purposes.
5 . Elevations will be based on NGVD 1929 unless otherwise directed. Cross sections for
directional boring locations will be obtained.
6. Prepare and provide five (5) paper copies and one (1 ) digital file of the route survey upon
completion of field work.
The total length of the Route Survey is approximately 8,200 feet.

Task B — Construction Plans
Masteller & Moler, Inc. will utilize the route survey and base plans provided and design and prepare
construction plans for a 12" diameter reuse main between 25`h Street SW and 130' Street SW. This
project will connect to the existing 12" reuse main on 25th Street SW and provide for construction of a
12" reuse main north along the east side of 43rd Avenue with a directional bore under canals and
terminating at 13th Street SW. The estimated length of the proposed 12" reuse main is 8,200' t. Our
plans will show the necessary gate valves, air release valves, directional boring, and construction details.
Task C - Permitting
Masteller & Moler, Inc. services include permitting from the Indian River Farms Water Control District
(IRFWCD). h1 addition, an Indian River County Utility and Right-of-way permit, FDEP Construction
Permit and the ACOE Nationwide 14 Permit will be needed. Masteller & Moler, hic. will prepare the
permit applications and supporting data and submit on behalf of the County. Permit application fees are
not included in this Proposal.
Task D - Construction Services
Masteller & Moler, Inc. will provide assistance during bidding and periodic inspection of the pipeline
system project on an as-needed basis to observe that construction is in reasonable compliance with the
Contract Plans and Permits. We will also attend meetings, review shop drawings and monthly pay
estimates from the contractor.
