Old Dixie Highway, CR 512 to US1 Maintenance Map Surveying and Mapping Services Project 0628 September 19th, 2006 This Work Order No . 7 ( Engineering ) is in accordance with the existing AGREEMENT dated September 23rd , 2005 between Carter Associates , Inc. (SURVEYOR) and Indian River County (COUNTY) . This Work Order No. 7 ( Engineering ) amends the agreement as follows : SECTION I — PROJECT LIMITS : Field survey and preperation of a Maintence Map OF : Existing Old Dixie Highway south of CR512 (eastbound ) through the existing FEC RR crossing and to US1 connection approximatly 1 . 5 miles . SECTION II — SCOPE OF SERVICES : This Work Order No. 7 (Engineering ) is for providing all Professional Survey services related to the task and services as indicated on in sections I , II , III and IV hereon , the attached exhibit "A" Proposal and scope of services supplied by Carter Associates , Inc. and attached Exhibit " B" Maintenance Map "check list' provided by the County . SECTION III — TIME FOR COMPLETION : All tasks outlined shall be completed , in its entirety within six weeks (45 calendar days) of notice to proceed . (The County shall be notified upon completion of tasks and/or of any delay beyond specified periods ) . SECTION IV — DELIVERABLES : An 80 % submittal shall be submitted for the County review and approvals . Final ( 100% ) submittal shall consist of Three (3) certified final survey '•hardcopies " in the form of ( 1 ) paper version and (2) Mylar versions , and an electronic file/disc in state plane geometry (AutoCAD 2005 or previous versions ) and PDF version shall be delivered in upon completion . All AutoCAD products shall adhere to the County's standard AutoCAD layer/file structure as supplied by the County . Pg2OF5 SECTION IV — COMPENSATION : The COUNTY agrees to pay , and the SURVEYOR agrees to accept , a fee for services rendered according to the "Scope of Services" identified in Section I , II , III and IV of this Work Order No . ( Engineering ) , a Lump Sum of $ 15 , 960 . 00 . All invoicing shall be supported with documentation ( Man hours ) of time and materials spent prior to payments . The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein . Projects shall be prepared/submitted in the form of a certified Maintenance Map will all applicable certifications and criteria met. 1 . Cover sheet/Key sheet/Location Sketch. 2 . Supply a Box for plat recording data, area for county acceptance and/or certification on cover sheet 3 . Survey notes and report sheet (As applicable) 4 . Any notes necessary to set out the type and/or purpose of the map shall be shown. 5 . Survey Control sheet (As applicable and or necessary) 6 . Subsequent Maintenance R/W map sheets . 7 . Title Box of each sheet must contain: a. County b. State/County Road Number or common name. c . Designed, drawn and checked by (use names or initials) & date d. Scale e. Field Book References f Sheet Numbers to be in numerical order ( 1 , 2, 3, 4, etc.) NOT ( 1 , 1A, 1B, 2, etc.) 8 . Show pertinent dimensions and bearings in the following fashion as the case may be: a. Plat — (P) b . Computed — (C) c . Field — (F) d. Deed (D) 9. Table of Ownerships shall show: a. Parcel Number b. Owners Name c. Recording Data 10. Survey Control lines and references shall be established with suitable material to perpetuate said control lines . 11 . Existing adjoining, abutting, intersecting Rights-of-Ways shall be depicted with information of record, sufficiently dimensioned and tied to control lines. 12 . The maintained lines shall be depicted as to allow for re-establishment in the field. 13 . Show stations and offsets to all existing and maintained Right of Way Breaks. 14 . Show monumented baselines and all references thereto. 15 . Verify that computed distances and bearings are supported by field measurements . 16. Verify and monument significant elements of Baseline alignment shown. (i.e . curves, bearings and P.I. Stations) 17. Ensure that significant elements of all curves are shown and are correct. Also show curve data on all sheets where they appear. 18 . Begin and End R/W Project with Stations. 19 . Stationing in field minimum 500 ' beyond beginning and ending of project or nearest major intersecting lines. Projects shall be in NAD83/99 State plane coordinate system and bearings reference shall be shown and must be clearly stated. Sections and '/ Sections (lines & comers) . Certified Comer reports of all Section/ 1 /4 comers used in the course of survey shall be completed and/or updated. 28 . Subdivision name and limits 31 . Plat Book and page (County) 32. Lots and Blocks (key map — blocks only) 33 . Lot and Street dimensions (plat or document of record). 34. Show all original plat and R/W lines of record, if different from present time, with dashed lines 35 . Show/plot all Murphy Reservations and/or release lines (including document of record) via graphic or dimension which ever is obtainable. 36. Show names of subdivision streets, plat book and page recording — also, show Lot and Block number and Dimensions, Exceptions, Easements, and Vacated portions. 37 . When property lines break the existing Right of Way on curve, show tangent bearings or chord bearings . 38. Show any replat of a subdivision — show old subdivision when necessary to clear up former acquisition or old title references — use system of dashed lines, dotted figures, etc. , with ties shown and name and plat book reference indicated 39. Stations and Angle Ties to ALL Existing Major and Minor Side Streets (Public Road R/W) as surveyed and/or calculated. 40. Show field located P. R. M. /major property comer or parcel ties by station/offset to baseline/control line. 41 . Current Street Names 42 . Any and all surface feature locations (including major trees or vegetation) within 25 feet of the maintained lines and buildings within 50 feet of maintained lines shall be dimensioned to the line or control line. Locations of buildings and large structures can be obtained from field and/or photographic / planimetric methods, note shall be placed on plan as to origin. 43 . Show and identify all easements of record (as supplied or obtained via title search) and any "apparent" easements or land uses that may indicate access or encumbrances on Detail Maps by width, use and ownership (where applicable).
