This Work Order Number 9 is entered into as of this i_tay of laLA 200 1
("Effective Date") pursuant to that certain Continuing Contract Agreement for Prkfessional Services
entered into as of April 6, 2004 ("Agreement), by and between Indian River County, a political
subdivision of the State of Florida ("COUNTY") and Masteller and Molex, Inc. ("CONSULTANT").
The COUNTY has selected the CONSULTANT to perform the professional services set forth
in Attachment 1 (Scope of Work), attached to this Work Order and made a part hereof by this
reference. The professional services will be performed by the CONSULTANT for the fee schedule set
forth in Attachment 2 (Fee Schedule), attached to this Work Order and made a part hereof by this
reference. The CONSULTANT will perform the professional services within the timeframe more
particularly set forth in Attachment 3 (Time Schedule), attached to this Work Order and made a part
hereof by this reference all in accordance with the terms and provisions set forth in the Agreement.
Pursuant to paragraph 1.4 of the Agreement, nothing contained in any Work Order shall conflict with
the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in
each individual Work Order as if fully set forth herein.
IN WITNESS WHEREOF, the parties hereto have executed this Work Order as of the date
first written above.

NON-EXCLUSIVE PROFESSIONAL ENGINEERING AGREEMENT
CONTINUING CONSULTING SERVICES
ATTACHMENT 1
• Protect Description
Work Order No. 9 provides for professional engineering services for the surveying, design, permitting,
bidding and services during construction of Phase Two arterial water main system to serve Vero Lake
Estates.
Upon completion of this Phase Two arterial potable water pipeline construction project, all of the Vero
Lake Estates community can be provided with potable water by the future construction of water lines,
valves, hydrants and fittings no larger than 6" in diameter.
Work Order No. 9 is being implemented at this time to provide professional engineering services by
Masteller & Moler, Inc. associated with the project described above. The work to be performed as part
of this Work Order will generally include providing a route survey and base plans; preparation of project
design for the proposed 8" and 12" pipelines, preparation of contract plans; preparation of specifications
and bid documents; preparation and submittal of permit applications, assistance during bidding and
contract administration/periodic inspection for the pipeline project.
The estimated project construction cost is $1,130,000.00
A brief description of the scope of services for Work Order No. 9 is provided below:
• Scope of Services
Task A — Route Survey and Base Plans
Masteller, Moler, Reed & Taylor, Inc will perform a Route Survey for proposed 12" waterlines located
in Vero Lake Estates described as follows:
1. Connection point of existing 12" water main on 87th Street at 98th Avenue west to proposed
waterline connection point per Schulke, Bittle & Stoddard plan at 104th Avenue. Length 3,910
feet.
2. Intersection of 87th Street and 99th Court south on 99th Court to proposed waterline connection
point per Schulke, Bittle & Stoddard plan at 79th Street. Length 5,300 feet.
3. Intersection of 87th Street and 95th Avenue at existing 12" water main, south on 95th Avenue to
proposed waterline connection point per Schulke, Bittle & Stoddard plan at 79th Street. Length
5,300 feet.

Route Survey includes:
1. Establish survey baseline as directed by the Engineer,
2. Locate all monumentation for establishment of existing rights-of-way;
3. Locate and identify all visible topographic features along the route (ie: power poles, mailboxes,
pavement, culverts, etc.);
4. Obtain elevations along the route sufficient for engineering design purposes;
5. Elevations will be based on NGVD 1929 unless otherwise directed;
6. Prepare and provide five (5) paper copies and one (1) digital file of route survey upon completion of
field work.
Task B — Construction Plans
Masteller & Moler, Inc will utilize the route survey and base plans prepared by Masteller, Moler, Reed
& Taylor, Inc. We shall use these base plans for design and preparation of construction plans for 12"
diameter water mains as follows:
1. Connection point of existing 12" water main on 87th Street at 98th Avenue west to proposed
waterline connection point per Schulke, Bittle & Stoddard plan at 104th Avenue. Length 3,910
feet of 12" diameter pipe.
2. Intersection of 87th Street and 99th Court south on 99th Court to proposed waterline connection
point per Schulke, Bittle & Stoddard plan at 79th Street. Length 5,300 feet of 8" diameter pipe.
3. Intersection of 87th Street and 95th Avenue at existing 12" water main, south on 95th Avenue to
proposed waterline connection point per Schulke, Bittle & Stoddard plan at 79111 Street. Length
5,300 feet of 8" diameter pipe.
The total estimated length of the proposed 12" water main is ± 3,910 feet and 8" water mains is ± 10,600
feet. Our plans will show the necessary valves, hydrants, air release valves, directional boring, and
construction details.
Task C - Permitting
Masteller & Moler, Inc. services include permitting from the Florida Department of Environmental
Protection (FDEP) In addition, an Indian River County Utility and Right-of-way permit and an ACOE
Nationwide 14 Permit will be needed. Masteller & Moler, Inc will prepare the permit applications and
supporting data and submit on behalf of the County. Permit application fees are not included in this
Proposal.
Task D - Construction Services
Masteller & Moler, Inc. will provide assistance during bidding and penodic inspection of the pipeline
system project on an as -needed basis to observe that construction is in reasonable compliance with the
Contract Plans and Permits. We will also attend meetings, review shop drawings and monthly pay
estimates from the contractor.

Method and Amount of Compensation
Masteller & Moler, Inc. proposes to provide the above Scope of Services based on the following lump
sum fees:
Tasks A, B, & C:
Provide for Surveying Services including Route Survey andBase Plans, Preparation;
Engineering Design, Preparation & Submittal of Permit Applications; Preparation of
Specifications & Bid Documents Utilizing Standard IRC Bid Documents. Forms,
Documents & Drawings will be in electronic Format MS Word
Fee for Tasks A, B & C $79,225.00
Task D•
Assistance during Bidding, Bid Evaluation, Contract Award & Construction
Administration; Periodic Administration and Periodic Construction/Field Inspection
Fee for Task D $12,300.00
Printing and Reproduction
The County shall make direct payment to our office for the cost of printing project plan sheets required
for utility coordination and for copies of reports, drawings, specifications, bidding and construction
contract documents, and other pertinent items required by federal, state and local agencies from whom
approval of the project must be obtained, prospective bidders, material suppliers, and other interested
parties, but may charge only for the actual cost of proving such copies based on the following tabulation:

Application Fees
The County shall make direct payment to our office for the actual cost of all construction permit
application fees paid to jurisdictional agencies, including but no limited to: FDEP, IRC Engineering
(Right -of -Way), IRFWCD, and ACOE.
