In accordance with Contract No . 0390 2005-2006 Project: Right-of-Way Mapping Project County Road 512 Roadway Improvements Phase III This Work Order No. 9 is in accordance with the existing AGREEMENT dated September 23rd, 2005 between Masteller, Moler, Reed & Taylor, Inc. (SURVEYOR) and Indian River County (COUNTY) . This Work Order No. 9 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No. 9 is for the SURVEYOR to prepare Right of Way Map for Phase III of the County Road 512 Roadway Improvements whose limits range from County Road 510 (Sta. 274+00) to Roseland Road (Sta. 346+00) , all lying in Indian River County , Florida. (7200 LF) SECTION 11 - SCOPE OF SERVICES As agreed upon between Masteller, Moler, Reed & Taylor Inc. and Indian River County , the Surveyor shall provide Professional Land Surveying services to complete all tasks as outlined in this Work Order No. 9 described as follows: • Right of Way Mapping: 1 . (At the SURVEYORS cost) Contract and coordinate with Title Company for research of the Public Records and obtain copies of all existing RNV acquisition deeds. 2 . Review and compile existing RNV acquisition parcel deeds for inclusion on Map. 3. Prepare Final Right of Way Map for County Road 512 Phase 111 . Map shall depict original right of way as shown on existing Route survey for same. Map shall also show current Right of Way as compiled from existing RNV acquisition parcels by the COUNTY. 4. Right of Way Map shall adhere to all requirements as listed in the Indian River County Right of Way checklist as modified and attached hereto as Exhibit "A" . SECTION III — TIME FOR COMPLETION Project shall be completed within one hundred twenty ( 120) business days from Notice to Proceed . Schedule shall include a 60-70% submittal within ninety (90) days for review and approvals by the COUNTY. Final submittal (Deliverables) shall be provided to the COUNTY upon completion . IRC Work Order #9E CR 512 PH3 R/W Map 9/6/2006 Page 2 of 5 SECTION IV — DELIVERABLES/WORK PRODUCT a) Survey shall be supplied as a right of way map in state plane coordinates and suitable for recording . b) Deliverables shall include a 60-70% submittal within ninety (90) days for review and approvals by the COUNTY prior to proceeding with the final product. c) Deliverables for the final (100%) submittal to the COUNTY will consist of a digital file (disk) . The digital file shall adhere to basic COUNTY AutoCAD layer structure as provided . d) The SURVEYOR shall provide the COUNTY five (5) certified copies and a digital file of the survey and prepare its Work Product digital versions so that the COUNTY or other consultants can readily use it for the design and analysis of the area, as defined . It shall contain all information necessary for third-party surveyor to independently recreate and/or utilize the survey work. It is acknowledged all final products become property of Indian River COUNTY and will be available for use by the public at large . e) The SURVEYOR'S work product shall meet or exceed the minimum standards as defined by Sections II , 111 and IV or the COUNTY will not approve the SURVEYOR'S request for payment. SECTION V — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for services rendered as identified in Sections It , 111 and IV of this Work Order No. 9 a fee at the standard hourly rates as agreed to by the Consultant and the COUNTY, and not to exceed $14, 700. 00. All invoicing shall be supported with documentation (Man hours) of time and materials spent and following information prior to payments. Invoices shall include the work order, contract and project numbers. Invoice number related to this work order, original contract amount, current billing amount and retainage amounts . All remaining sections of the AGREEMENT shall remain in full force and effect, and are incorporated herein . Cover sheet/Key sheet. 2 . Survey notes and report sheet (As applicable) 3 . Survey Control sheet (As applicable and or necessary) 4. Subsequent R/W map sheets. 5 . Supply a Box or area for COUNTY acceptance and/or certification. 6 . Title Box of each sheet must contain: a. COUNTY b. State Road Number c. Designed, drawn and checked by (use names or initials) & date d. Scale e. Field Book References f. Sheet Numbers to be in numerical order ( 1 , 2, 3 , 49 etc.) NOT ( 1 , IA, 1B, 2, etc.) 7. Any notes necessary to set out the type and/or purpose of the map shall be shown. 8 . Show pertinent dimensions and bearings in the following fashion as the case may be: a. Plat — (P) b. Computed — (C) c. Field — (F) d. Deed (D) 9. Table of Ownerships shall show: a. Parcel Number b. Owners Name c. Recording Data 10. Show stations and offsets to all Right of Way Breaks. 11 . Establish a baseline alignment for control and determine location of significant elements for future monumentation. 12. Verify that computed distances and bearings are supported by field measurements. 13 . Intentionally omitted — to be included in future contract. 14. Ensure that significant elements of all curves are shown and are correct. Also show curve data on all sheets where they appear. 15 . Begin and End R/W Project with Stations. 16. Baseline (equate to intersecting State or COUNTY Jobs by Station and job number when available) as available. 18 . Projects shall be in NAD83/90 State plane coordinate system and bearings reference shall be shown and must be clearly stated. Values shall be place on key baseline monumentation. 19. Certified Comer reports of all Section/ l /4 comers used in the course of survey shall be completed and/or updated. 25 . Subdivision name and limits 28 . Plat Book and page (COUNTY) 29. Lots and Blocks (key map — blocks only) 30 . Lot and Street dimensions (plat) . 31 . Show all original plat and R/W lines of record, if different from present time, with dashed lines 32. Show names of subdivision streets, plat book and page recording — also, show Lot and Block number and Dimensions, Exceptions, Easements, and Vacated portions. 33 . When property lines break the Right of Way on curve, show tangent bearings or chord bearings. 34. Show any replat of a subdivision — show old subdivision when necessary to clear up former acquisition or old title references — use system of dashed lines, dotted figures, etc. , with ties shown and name and plat book reference indicated 35 . Stations and Angle Ties to Existing Side Streets as surveyed and/or calculated (where applicable). 36. Show field located P. R. M. /major property comer or parcel ties by station/offset to baseline/control line (where applicable). 37. Current Street Names 38 . Show and identify all easements on Detail Maps by width, use and ownership (where applicable) .
