HomeMy WebLinkAbout2006-188 5 WORK ORDER NUMBER CR512#I app G g FOR 101ST AVENUE EXTENSION NORTH OF CR512 IRC PROJECT # 0614 This Work Order No. CR512# 1 is entered into as of this 6th day of June , 2006, pursuant to that certain Continuing Contract Agreement for Professional Services entered into as of March 15 , 2005 ("Agreement"), by and between Indian River County, a political subdivision of the State of Florida ("COUNTY") and G. K. Environmental, Inc . ("CONSULTANT"). The COUNTY has selected the CONSULTANT to perform the professional services set forth on Exhibit "A," attached to this Work Order and made part hereof by this reference. The professional services will be performed by the CONSULTANT for the fee schedule set forth in Exhibit "A" attached to this Work Order and made a part hereof by this reference. The CONSULTANT will perform the professional services within the timeframe more particularly set for in Exhibit "A," attached to the Work Order and made a part hereof by this reference, all in accordance with the terms and provisions set forth in the Agreement. Pursuant to paragraph 1 .4 of the Agreement, nothing contained in any Work Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in each individual Work Order as if fully set forth herein. County Engineer Indian River County 1840 25h Street Vero Beach, FL 32960 RE: PROPOSAL / CONTRACT CR 512 / 101 st Ave. COUNTY PROJECT #.; 50f Acre Site; Sec. 21 , Twn. 31 , Rng. 3B Dear Mr. Kafer, This letter is in response to the requested proposal for the above referenced project in Indian River County, Florida, for environmental services as generally required for State and Federal Environmental Resource / Individual Permits. G. K. Environmental, Inc. (GKE) will be pleased to provide this service with tasks completed under this contract and which are briefly outlined below: SCOPE OF SERVICES TASK I — Wetland Delineation $2,900.00 TASK I: GKE . will locate / flag State and Federal regulatory wetlands in accordance with Florida Administrative Code (F.A. C.) 62-340 and the Army Corps of Engineers (ACOE) Wetland Delineation Manual / Technical Report . Y-87- 1 . Delineated wetlands will be field located on a V - 200' scale county blue-line aerial or submitted survey drawing and included in a .written report of findings. Wetlands will be assessed per current Federal regulations and current guidelines regarding isolation from Federal waters. TASK II - Agency Verification of Wetland Delineation $1 ,600.00 (not to exceed without prior authorization) TASK II : GKE will coordinate on site inspections with State and Federal regulatory officials to verify jurisdictional wetland lines / locations. Federal Office (772) 567-9129 E-mail grk@steds.org Fax (772) 567-6245 Printed on Recycled Paper TASK VI - Vegetation Survey (FLUCCS) $450.00 TASK VI: GKE will conduct a systematic survey of vegetation using the Florida Land Use, Cover and Classification System (FLUCCS), A Technical Report. FLUCCS was developed by the State of Florida, Department of Administration, Division of State Planning, Bureau of Comprehensive Planning, April 1976. GKE ecologist will base the vegetative community descriptions upon field survey observations, SCS soil maps, and aerial photograph interpretation. TASK VII - Permit Application Submittals 10 hours - $ 1 ,100.00 (not to exceed without prior authorization) TASK VII: GKE to assist client and client' s engineer and surveyor to complete required field work / meet / review work with client, review drawings, and permit application completion. The completion of the above tasks will be provided to the client in report form. TASK VIII — Response to Agency Requests for Additional Information (RAI) Estimated $3,000.00 (not to exceed without prior authorization) TASK VIII : Following agency permit application submittal and agency review, GKE will respond to all environmental questions outlined in agency letters Requesting Additional Information (RAI letters); Not to exceed $3 ,000.00 without prior authorization from client. GKE Fees / Compensation Work on TASKS I through VII outlined above could begin within fifteen ( 15) days of G. K. Environmental, Inc. 's receipt of Notice to Proceed, design plans, and field staked right-of-way if necessary. Receipt of one ( 1 ) fully executed copy of this contract shall serve as our Notice to Proceed with Tasks I through VIII. Invoices will be submitted on a monthly basis at GKE hourly rates. GKE Indian River County hourly rates (2006) are $ 110. 00 / hour for Principal Ecologist and $30. 00 / hour for administrative / staff assistance. Work on TASK VIII will begin following agency receipt of application and receipt of permitting agency requests for additional information. Page 3 of 4 GKE / Kafer, PE, IRC Proposal / IRC - CR 512 5- 15-06 verification of wetland line often coincides with State verification. If written agency verification is required, the client will provide GKE 5 copies of certified survey of GKE verified flag locations for transmittal to agencies for approval. ACOE verification my not be available until after permit application submittal due to agency workload. TASK III — Mitigation Plan $2,450.00 (not to exceed without prior authorization) TASK III : GKE will meet with client and permit agency staff to discuss mitigation options and inspect potential mitigation sites for compatibility and prepare a draft mitigation plan, based upon the ecological values of wetland impacts. TASK IV - Wetland Assessment - UMAM / WRAP $ 1 ,100.00 TASK IV: GKE will conduct a Wetland Rapid Assessment Procedure (WRAP) and/or a Uniform Mitigation Assessment Method (UMAM) of the wetlands located within the boundaries of the project site. WRAP/UMAM determine a numerical value for the ecological functions and ecological values of the existing wetlands and mitigation site. The WRAP/UMAM score can be used to determine what type / amount of mitigation will be required for the project site. The WRAP/UMAM score is based on vegetation, wildlife usage, hydrology, soils, water quality, success factor, and adjacent lands. TASK V — Protected Species $850.00 TASK V: GKE will complete a detailed protected species survey on the referenced site. GKE ecologist will conduct a preliminary systematic survey for protected species in accordance with Florida Fish and Wildlife Conservation Commission (FFWCC) Guidelines. GKE ecologist will record all observations of listed protected species and features that may indicate species presence. Physical features may include, but are not limited to, tree markings, burrows, tracks, nests, scat, and cavity trees. GKE may request that the U. S . Fish and Wildlife Service (FWS) and the Florida Fish & Wildlife Commission (FFWCC) review their files for the presence of any listed species that may be present onsite. Page 2 of 4 GKE / Kafer, PE, IRC Proposal / IRC - CR 512 5- 15-06 The above tasks (I - VIII) are not to exceed $ 13 ,450.00 without prior authorization from Indian River County. Payments are due and payable within 30 days of date of invoice. Services Not Included The following services are not included (if required) in this agreement, but will be coordinated and reviewed by GKE : 1 ) water quality laboratory analysis 2) surveying / engineering 3) Auto CAD work 4) legal work 5) permit drawings 6) reasonable access to subject site If the above is acceptable, please sign below and mail or fax a copy to our office.
