HomeMy WebLinkAbout2010-045 EXHIBIT ` 1 ' SCOPE OF WORK WORK ORDER NO , 2 The project consist of designing approximately 20+ water main conflict crossings between the existing water mains and proposed gravity stormwater system proposed for the 66 "' Avenue, Phase H constriction project . The conflicts are anticipated to be located on : 66 "' Avenue ; 4"' Street ; 8 "' Street ; and turnouts located on 4"' Place , 4"' Lane , 5t" Street, 5 "' Place , and 65th Avenue . In addition to the conflict crossings , several gravity stormwater crossings will be redesigned to prevent potential conflicts with existing water mains . The following describes our proposed scope of work and terms of agreement between Carter Associates , Inc . (CONSULTANT) and Indian River County Department of Utility Services ( COUNTY) : Task 1 - Design Survey : CONSULTANT will perforrn surveying services for the project site to include the following : 1 . The anticipated water main conflicts have previously been identified, were exposed and surveyed under a scope of services with IRC Public Works . However, since performance of those services , two additional cross drains on 66t" Avenue were requested by IRC Engineering staff. These present potential conflicts and will require exposure and location/elevation surveying . 2 . Based upon the current roadway design , all other design surveying has been completed . Task 2 - Coordination with Public Works Department : CONSULTANT will meet with Public Works staff to : 1 ) coordinate with county staff on an acceptable method of incorporating the new conflict plans into the roadway Bld/Constrrction plan set and 2) attend progress meetings with county staff as the Bid/Construction plan set is revised . Task 3 - Construction Plans and Specifications : CONSULTANT will prepare plan and profile drawings of each conflict crossing, detail sheets , and technical specifications for the proposed water main conflicts . Task 4 - Permitting : CONSULTANT intends to utilize the permitting exemption found in paragraph 62 - 555 . 520( 1 ) (c) 3 . of the Florida Administrative Code for relief from a Florida Department of Environmental Protection permit . Plans will be forwarded to the Department to obtain the exemption . CONSULTANT anticipates that no county Right- of-Way or Utility permits will be required for this project . Task 5- Construction Administration : CONSULTANT will attend pre-bid and pre- constriction meetings . CONSULTANT shall : act as liaison between the Contractor and Indian River County during construction of the water main conflicts ; review shop drawings and other Page 2 of 6 a Contractor submittals for the water main portion of the project ; and respond to "Requests for Information" from Contractor related to the water main portion of the project ; and review Contractor invoices and change order requests related to the water main portion of the project . Task 6- Water Conflict Construction Inspection : Provide part time project inspector for duration of time required to comply with FAC 62 - 555 requirements . This scope is based upon the provision of up to 80 hours (4 hours/conflict crossing) of " on- site " inspection . Task 7 - " As-Built" Drawing : Using "as-built" survey information provided by the Contractor ' s surveyor and the Engineer ' s inspection notes , revise construction plans to indicate "as-built" conditions . Complete and submit "as-builts" to authorities having jurisdiction . Reimbursable Expenses : Direct reproduction costs , communications , mileage , postage , express mail , etc . attributable to the completion of this project will be billed to the Client in accordance with Carter Associates , Inc . standard rate schedule , and shall not exceed the amount indicated for this Item unless prior approval is obtained from the COUNTY in writing . ENGINEERING ASSUMPTIONS : In developing this proposal , CONSULTANT represents to the COUNTY that the following assumptions have been made and that deviation from these assumptions may require additional expenditures on behalf of the COUNTY to CONSULTANT . 1 . No permitting, application , impact and/or testing .fees are included and will be paid for by the COUNTY or Contractor (as applicable) separately. 2 . The number of inspection hours is based upon the Contractor performing expedient work and providing the specified advance notice to the CONSULTANT and IRCUD prior to performing any water main conflict work . 3 . No constriction surveying or staking is included with this proposal . Page 3 of 6 EXHIBIT ' 2 " FEESCHEDULE WORK ORDER NO . 2 CONSULTANT will generally bill the COUNTY monthly on approximately the 101" day of each month for the previous month ' s charges . All tasks will be billed on a percentage complete/lump sum basis , including direct expenses ( See attached Rate Schedule) . Should additional services be required, a separate written agreement will be prepared and fees shall be negotiated based on the attached Rate Schedule . Page 5 of 6 f EXHIBIT 63 ' TIME SCHEDULE WORK ORDER NO . 2 The following timeline is projected after the Notice to Proceed is issued by the COUNTY : Task 1 Design Survey: (0- 15 days) Survey information downloaded within 15 days after Notice to Proceed . Task 2 Coordination with Public Works Department : ( 15 — 60 days) Coordination shall coincide with construction plans and specifications . Task 3 Construction Plans and Specifications : ( 15 — 60 days) Construction plans and specifications completed 45 days after completion of Survey. Task 4 Pennitting Exemption : ( 60- 90 days) Permit exemption request submitted after completion of Constriction Plans and Specifications . Permit exemption estimated to be issued within 30 days of submittal . Task 5 Construction Administration : (ongoing) Constriction administration shall be provided during conflict installation only. Schedule based on length of roadway constriction project . Task 6 Constriction Inspection : (ongoing) Construction inspection shall be provided during conflict installation only. Schedule based on length of roadway construction project . Task 7 As -Built Drawing : (upon completion of conflict installations) "As -built" drawing will be performed during and immediately after submission of water main conflict "as-built ' data . Page 6 of 6 WORD ORDER NUMBER 2 This Work Order Number 2 is entered into as of this 9th day of February , 2010 , pursuant to that certain Continuing Contract Agreement for Professional Services entered into as of May 1 , 2007 (the "Agreement ') , by and between INDIAN RIVER COUNTY , a political subdivision of the State of Florida (" COUNTY ') and CARTER ASSOCIATES , INC . ("CONSULTANT") . The COUNTY has selected the Consultant to perform the professional services set forth on Exhibit 1 ( Scope of Work) , attached to this Work Order and made a part hereof by this reference . The professional services will be performed by the Consultant for the fee schedule set forth in Exhibit 2 (Fee Schedule) , attached to this Work Order and made a part hereof by this reference . The Consultant will perform the professional services within the timeframe more particularly set forth in Exhibit 3 (Time Schedule) , attached to this Work Order and made a part hereof by this reference all in accordance with the terms and provisions set forth in the Agreement . Pursuant to paragraph 1 . 4 of the Agreement , nothing contained in any Work Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in each individual Work Order as if frilly set forth herein .
