Fire Station 13 , 4th Street and 43rd Avenue Site Work , Preparation and Improvements Emergency Services District WORK ORDER NO . 1 FOR DESIGN /BUILD AGREEMENT PHASE IV WITH Barth Construction , Inc . This Work Order No . 1 is in accordance with the existing DESIGN /BUILD AGREEMENT PHASE I dated December 6 , 2012 between Barth Construction , Inc ( Design/Builder) and Indian River County ( County) . This Work Order No . 1 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 1 is for the Design/Builder to perform work in connection with the Site Work , Preparation and Improvements to the proposed Fire Station 13 located at 4th Street and 43rd Avenue . i SECTION II - SCOPE OF SERVICES t; As agreed upon between Design/Builder County , the Design/Builder shall provide all engineering , architectural and construction services to complete all tasks as outlined in this Work Order No . 1 ; specifically detailed in the attached proposal Exhibit A . SECTION III — TIME FOR COMPLETION & DELIVERABLES/WORK PRODUCT 1 . Project shall be completed as follows : a . Time for substantial Completion shall be 90 calendar Days from notice to proceed . b . Time of FINAL project completion shall be within 15 calendar days of receipt of the County's review comments . 2 . The Design/Builder shall provide the County : a . Survey set/sheets shall include a cover sheet with location sketch , survey certifications , and related title and Project No ; survey notes/legend/abbreviation ry sheet and right-of-way plan view sheets . b . Work Product and digital versions are to prepared and submitted so that the County or other consultants can readily use it for the design and analysis of the area , as defined . It shall contain all information necessary for third-party surveyor to 1 �< IRCESD Work Order No. 1 Barth Construction , Inc Fire Station 13 August 20, 2013 Page 2 of 3 independently recreate and/or utilize the survey work . It is acknowledged all final products become property of the County . SECTION IV — COMPENSATION The County agrees to pay , and the Design/Builder agrees to accept , for the above described services rendered as identified in Sections I , II , and III of this Work Order No . 1 , for a total amount of $ 176 , 493 . 00 . Any additional services not described hereon shall be pre-approved by the County . All invoicing shall include Work Order No . (WO 1 ) , itemized costs and materials expended to complete the Work . Payments shall be in accordance with the original Agreement dated December 6 , 2012 . The AGREEMENT is hereby amended as specifically set forth herein . The scope of Work Order # 1 is as follows; • Coordinate with Local and State agencies to partial complete the site development. • Clear , grub and prep site for fill • Excavate wet retention pond in NE corner of site • Rough grade the site in preparation for future building and paving • Haul excess material to IRC Emergency Services site at 66d' avenue and 26`s street • Install outfall structure and piping, includes open road cut on 4''' Street • Install storm structure S-5 and piping into pond • Install new water service into site and connect the existing buildings, includes road cut on 40' street. • Install sod and landscape required along the pond banks . • Hydro seed the newly filled fire station site. i I We propose to perform all of the work described above including provide project management, coordination and supervision for $ 176, 493 .00.
