PROFESSIONAL BRIDGE DESIGN SERVICES AGREEMENT
FOR ANNUAL BRIDGE DESIGN SERVICES CONTRACT
WITH
Bridge Design Associates , Inc .
Indian River County Project No . 0745
This Amendment no . 1 to Work Order No . 11 is in accordance with the existing
AGREEMENT dated October 1 , 2009 between Bridge Design Associates , Inc . (ENGINEER)
and Indian River County ( COUNTY) .
This Amendment No . 1 to Work Order Number 11 amends the agreement as follows :
SECTION II — SCOPE OF SERVICES
This Amendment No . l to Work Order No . 11 will include the processing and addressing
comments within the standard environmental resource application (40 - 061 - 122403 - 1 ) . This
will also include revising the construction plans .
SECTION IV — COMPENSATION
The COUNTY agrees to pay , and the ENGINEER agrees to accept , a fee for services
rendered according to the " Scope of Services " identified in Section II of this Amendment No .
1 to Work Order Number 11 , a Lump Sum Fee of $ 5 , 070 . 00 .

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections
of the AGREEMENT shall remain in full force and effect, and are incorporated herein .

Bridge Design Associates , Inc. can perform the necessary engineering services in connection with
environmental permit authorization processing for the above referenced project as outlined below :
1 . Review existing aerials , plans , surveys , permits and other materials as may be obtained for
the referenced project , and accomplish any necessary on-site reconnaissance .
2 . Process existing Standard Environmental Resource Permit Application No . 40-061 - 122403- 1
with the St . Johns River Water Management District .
Success and timing in permitting are not guaranteed . Proposed services do not include any other
authorizations such as local government approval or NPDES permitting . Other regulations should be
independently checked by others .
The above services will be performed for $5 , 070 . 00 .
