( "CONSULTANT" ) . The COUNTY has selected the Consultant to perform the professional services set forth on Exhibit A (Scope of Work) , attached to this Work Order and made part hereof by this reference . The professional services will be performed by the Consultant for the fee schedule set forth in Exhibit A ( Fee Schedule ) , attached to this Work Order and made a part hereof by this reference . Bridge Design Associates, Inc. can provide Professional Structural Engineering services for 41 st St. , 45"' St. , and 01 St. Bridges as follows. • Attendance at pre-construction meeting • Review of Shop Drawings • Monitor test pile driving • Analyze the test pile driving results, including a computer analysis to determine the production pile lengths and blow counts required for production piles. • Monitoring pile driving • Inspect and monitor sheet pile casting • Verify filter cloth installation and grouting of the sheet panel joints • Reinforcing steel inspection and casting of the end bents • Reinforcing steel inspection and casting of the superstructure deck units • Superstructure closure pours, reinforcing and casting • Post-tensioning of the deck slabs • Reinforcing steel inspection and casting of the sidewalks • Reinforcing steel inspection of the barrier walls • Reinforcing steel inspection and casting of the approach slabs • As-built drawings and certification package to governing agency (FDOT and County) • Load Rating Certification • PDA Testing 1402 Royal Palm Beach Boulevard • Building 200 • Royal Palm Beach, Florida 33411 (561 ) 686-3660 • Fax: (561 ) 791 -1995 66th Street Bridge Replacements Page 2 In addition , Bridge Design can provide construction services for 331 Street and 37t1 Street bridges as follows : • Attendance at pre-construction meeting • Shop Drawing Reviews • Response to Contractor RFI ' s • Load Rating Certifications 26th Street Headwall as follows : • Attendance at pre-construction meeting • Review Shop Drawings • Monitor pile installation • Monitor reinforcing and casting of new sidewalk and barrier • Final Inspection • Final Report & Certification Concrete and soil testing is not included in the scope of services for this project.
