FOR
PROFESSIONAL BRIDGE DESIGN SERVICES AGREEMENT
FOR ANNUAL BRIDGE DESIGN SERVICES CONTRACT
WITH
Bridge Design Associates , Inc .
Indian River County
This Work Order No . 22 is in accordance with the existing AGREEMENT dated December 1 ,
2009 between Bridge Design Associates , Inc . (ENGINEER) and Indian River County
( COUNTY) .
This Work Order Number 22 amends the agreement as follows :
SECTION I — PROJECT LIMITS
CR512 Bridge over Fellsmere Water Control District (F. W. C . D . ) Lateral " U" Canal
SECTION II — SCOPE OF SERVICES
This Work Order No . 22 is for providing structural engineering services for the above
mentioned project, including weight limit calculations and load rating analysis .
SECTION III - TIME FOR COMPLETION
Project shall be completed within thirty (30) business days from Notice to Proceed .
SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the ENGINEER agrees to accept , a fee for services
rendered according to the " Scope of Services " identified in Section II and III of this Work
Order Number 22 , in a lump sum amount of $ 1 , 960 . 00 .

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections
of the AGREEMENT shall remain in full force and effect , and are incorporated herein .

Pursuant to your request, Bridge Design Associates , Inc . can perform the required
structural engineering services at the above referenced project .
Our services will include weight limit calculations and load rating analysis per Florida
Department of Transportation requirements .
Our fees will be as follows :
4 . 0 hours Project Manager at $ 145 . 00 per hour = $ 580 . 00
12 . 0 hours Project Engineer at $ 115 . 00 per hour = $ 1 , 380 . 00
TOTAL $ 1 , 960 , 00
