ENGINEERING SERVICES AGREEMENT C
66 ' H AVENUE PAVING AND DRAINAGE IMPROVEMENTS , PHASE 2
BETWEEN 4TH STREET AND 16TH STREET
IRC PROJECT NO . 9810B
THIS is an amendment to the existing AGREEMENT dated NOVEMBER 17 , 1998 , including
AMENDMENTS NUMBER 1 through 17 , between CARTER ASSOCIATES , INC . (ENGINEER) and INDIAN
RIVER COUNTY (COUNTY) . The proposed scope of work includes Design for water main extensions that
includes : an additional 2 , 300 linear feet of 8 - inch diameter water main along the east side of 66th Avenue , south
of 12th Street , providing fire service to the adjacent residential properties including approximately 3 fire hydrants,
replacement of water services , inspections and final certification of completion . All provisions of the original
Professional Engineering Services Agreement including Amendment Nos . 1 through 17 shall remain in full force
and effect, unless otherwise specifically modified . This amendment addresses changes in " SCOPE OF
SERVICES ", "COMPENSATION" and "TIME OF COMPLETION " of the AGREEMENT .
SCOPE OF SERVICES
See attached letter from Carter Associates , Inc . dated August 24 , 2012 , (Exhibit A) which specifies
changes in the " SCOPE OF SERVICES " and " COMPENSATION" .
COMPENSATION
This section of the AGREEMENT entitled SECTION V - COMPENSATION shall be revised in response
to the Scope modifications set out herein . The charges associated with this AMENDMENT NO . 18 are
as
follows :
Design and drafting $ 11000 . 00
Quantity takeoff and drafting $ 340 . 00
Prepare and send deliverables $ 50 . 00
Permitting ( 1 new permit) $ 420 . 00
Permit application fee $ 650 . 00
Inspection ( 14 Hours) $ 910 . 00
FDEP Certifications $ 0 . 00
(Previously included in amendment No . 17)
Total (Not to exceed) $ 3 , 370 . 00

The charges associated with the AGREEMENT (which include changes from Amendments 1 through 17 and
these additional services included in proposed Amendment 18 ) , shall be increased from $ 703 , 555 . 00
to
$ 7065925 . 00
The AGREEMENT is hereby amended as specifically set forth herein . All sections of the AGREEMENT
shall remain in full force and effect, and are incorporated herein. This Amendment No . 18 to the AGREEMENT,
regardless of where executed, shall be governed by and construed by the laws of the State of Florida.
TIME OF COMPLETION
Carter Associates , Inc . shall have 30 calendar days from their receipt of a written Notice to Proceed to
complete the additional design services of this amendment . Inspection services to run concurrently with the
construction for the 66th Avenue Roadway Improvements, Phase 2 (4th Street to 16th Street) project .

Dear Mike :
We are pleased to present this proposed amendment for your review and consideration . This
proposal is for additional professional engineering services for new 8 - inch water main
improvements to be incorporated into the Phase 2 portion of the 66 °i Avenue paving and drainage
improvements project (41h Street to 16t11 Street) . All provisions of the original Professional
Engineering Services Agreement (dated May 1 , 2007) , shall remain in full force and effect , unless
otherwise specifically modified herein . The following further describes our proposed scope of
services and terms of agreement between Carter Associates , Inc . (CAI ) and Indian River County
( County ) :
Additional Setwvices :
In accordance with our meting at Indian River County Department of Utility Services yesterday , we
propose to provide the following services :
1 . Design and draft an 8 - inch water main extension of approximately 2 , 300 IS. in length , to be
located along 66th Avenue south of 12th Street . Provide fire service to the adjacent residential
properties with up to 3 fire hydrants . Replace two existing old service lines .
2 . Conduct simplified fire flow analysis of extended 8 - inch mainline to check for required
residential fire flow .
3 . Calculate and itemize additional Contract Construction Quantities on Quantity sheet .

4 . Produce and provide digital CD and hard copy of revised Plan/Profile and Quantity sheets to
County .
5 . Prepare FDEP Permit Application and Provide $ 650 Application Fee .
6 . Provide inspection of materials , conflict crossings , sample point locations and final pressure
test . The day to day inspection and assurance of construction complying with County
standards shall be performed by the County
7 . Perform FDEP certifications of completed , FDEP permitted facilities only .
Compensation .•
The estimated costs to provide the above stated additional services are as follows :

• Prepare and Send Deliverables $ 50 . 00
• Permitting ( 1 new permit) $ 420 . 00
• Permit Application Fee $ 6 .50 . 00

• FDEP Certifications (Prev . Included ) $ 0 . 00
Total = $ 3 ,370 . 00
Amendment No . l additional services shall be invoiced on a time and materials basis , with a not to
exceed total cost of $ 3 , 370 . 00 .
