INDIAN RIVER COUNTY PARKS MAINTENANCE COMPLEX
SUPPLEMENT No . 4 to WORK ORDER NO . 1
TO THE
PROFESSIONAL CIVIL ENGINEERING SERVICES AGREEMENT
WITH
CARTER ASSOCIATES , INC .
This Supplement No . 4 to Work Order No . 1 is in accordance with the existing
AGREEMENT dated January 9 , 2007 and the extension thereof, between Carter
Associates , Inc . (ENGINEER) and Indian River County ( COUNTY) .
This Supplement No . 4 to Work Order No . I amends the agreement as follows :
SECTION II — SCOPE OF SERVICES
The Scope of Services for this Supplement No . 4 to Work Order No . 1 is to perform the
following tasks as requested by the COUNTY :
• Attend additional site meetings to complete project .
Y Assist COUNTY in resolution of site fill discussions .
SECTION V - COMPENSATION
The COUNTY agrees to pay , and the ENGINEER agrees to accept , a fee for
construction related services rendered according to the " Scope of Services " and further
identified in all the Sections of this Supplement No . 4 to Work Order No . 1 , in
accordance with the hourly rate for a total not-to - exceed amount of $ 2 , 675 . 00 .
All and/or any additional services not described hereon shall be pre - approved by the
COUNTY . Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the ENGINEER, as outlined in Work Order No . 1 .

All invoicing and payments shall be in accordance with the original Professional Civil
Engineering Services Agreement with the COUNTY and as stated hereon . Invoices
shall include the work order and project number , invoice number related to this work
order, original contract amount , current billing amount and retainage amounts .
The AGREEMENT is hereby amended as specifically set forth herein . All remaining
sections of the AGREEMENT shall remain in full force and effect , and are incorporated
herein .
