In accordance with Contract No . 1032 2010-2011 This Work Order No . 1 is in accordance with the existing AGREEMENT dated October 19 , 2010 between Creech Engineers , Inc. , ( SURVEYOR) and Indian River County ( COUNTY) . This Work Order No . 1 amends the agreement as follows : SECTION I — PROJECT LIMITS This Work Order No . 1 is for the SURVEYOR to perform all related field and office Surveying and Mapping services in connection with the Vertical Control Network Densification , consisting of the establishment of twenty-six Benchmarks meeting 2nd Order, Class B Stability . SECTION II - SCOPE OF SERVICES As agreed upon between SURVEYOR and COUNTY, the SURVEYOR shall provide Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 1 ; specifically detailed in the attached proposal Exhibit A . SECTION III — TIME FOR COMPLETION & DELIVERABLESMORK PRODUCT 1 . Project shall be completed as follows : a . The SURVEYOR will complete all field monumentation and measurements within 130 calendar days from receipt of the Notice to Proceed from the COUNTY . b . The SURVEYOR will submit a preliminary submittal to the National Geodetic Survey ( NGS) , within 160 calendar days from receipt of the Notice to Proceed from the COUNTY c . Final Deliverables to the COUNTY are anticipated to be completed within two months after the submittal of the preliminary data as described in Item b above . • The scheduled review and acceptance by the NGS depends heavily on their work load at the time of submittal and may vary greatly. The COUNTY will perform a complete review of the Final Deliverables for accordance of contractual items prior to final acceptance by the COUNTY . 2 . Deliverables -The SURVEYOR shall provide the COUNTY : a . All copies of correspondence between the SURVEYOR and the NGS , including copies of all data submittals from project planning to final acceptance . b . FINAL Deliverables , five ( 5 ) paper " hardcopy" signed and sealed Surveyor Reports , photographs of each benchmark recovered or set , copies of NGS recovery sheets for each Benchmark recovered , NGS data sheets for each newly established Benchmark any project related digital files , on AutoCad shall be prepared and delivered in release 2004 through 2009 , PDF formats and an ASCII file for all survey points , all electronic files to be on CD . c . Surveyor Report shall include a cover sheet with location sketch , survey certifications , related title and project number, survey notes , legend and abbreviations and project data sheets . d . Work Product and digital versions are to prepared and submitted so that the COUNTY or other consultants can readily use it for the design and analysis of the area , as defined . It shall contain all information necessary for third - party surveyor to independently recreate and/or utilize the survey work . It is acknowledged all final products become property of Indian River COUNTY and will be available for use by the public at large . e . The SURVEYOR' S work product shall meet or exceed the minimum standards as defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR' S request for payment . SECTION IV — COMPENSATION The COUNTY agrees to pay, and the SURVEYOR agrees to accept , for the above described services rendered as identified in Sections I , II , and III of this Work Order No . 1 , for a total lump sum fee of $ 118 , 451 . 00 . All and/or any additional services not described hereon shall be pre -approved by the COUNTY . Approved additional services shall be invoiced at the rates disclosed in the approved fee schedule with the SURVEYOR for the contract year(s ) of October 19 , 2010 through October 19 , 2011 . All invoicing shall include Project Number ( 1050 ) , Work Order No . 1 (WO 1 ) , Contract Number ( 1032 ) , itemized man - hours and materials expended to complete the Scope of Services . Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services Agreement , Contract No . 1032 with the COUNTY and as stated in Section II , III and IV hereon . ( hereinafter referred to as " CONSULTANT" ) is pleased to submit this proposal for professional surveying services to Indian River County ( hereinafter referred to as " COUNTY" ) for the Vertical Densification Project 2011 located in Indian River County, Florida ( hereinafter referred to as " PROJECT" ) . It is the CONSULTANT' S understanding that the COUNTY is adding to , and strengthening the existing published vertical control in Indian River County for FEMA National Flood Insurance Program Community Rating System ( CRS ) credits . To accomplish this task , the COUNTY requested professional surveying and mapping services to level and Blue Book a maximum of twenty-six (26 ) monuments . All Twenty-six ( 26 ) of the monuments will be of aluminum alloy rod construction . Said monuments will be required to meet NGS Class " B" stability. The resulting vertical control network will be Blue Booked and submitted to the National Geodetic Survey ( NGS ) for inclusion into the National Spatial Reference System ( NSRS ) . SCOPE OF SERVICES Task 01 : Planning The CONSULTANT will prepare a map that will show the proposed level routes for all proposed benchmarks . Said level route map will be based upon the " NATIONAL FLOOD INSURANCE PROGRAM -COMMUNITY RATING SYSTEM ; 2011 BENCHMARK INVENTORY SHEET" provided by the COUNTY . Adequate coordination with Sunshine One Call - underground locates will be performed for each location assuring clearance of underground utilities of conflicts prior to any excavations . 10061 00-P P L-072511 .d oc Indian River County VERTICAL DENSIFICATION PROJECT 2011 July 25 , 2011 Page 2 of 6 Reconnaissance The CONSULTANT will recover published control and determine suitable locations for newly established benchmarks to be Blue Booked . The CONSULTANT will prepare field notes and take digital photography of existing stations . Adequate coordination with Call Sunshine - underground locates will be performed for each location assuring clearance of underground utilities of conflicts prior to any excavations . Monumentation The CONSULTANT will establish new monuments of sufficient stability to be Blue Booked in the vertical network . All new benchmarks will be referenced with three ( 3 ) or more witness monuments , and a flexible survey witness post . All Twenty-six (26 ) of the monuments will be of aluminum alloy rod construction . Said monuments will be required to meet NGS Class " B" stability. A maximum of twenty-six (26 ) newly constructed monuments are included in this contract . Task 02 : Vertical Measurements The CONSULTANT will use digital leveling methods to create a closed level network . This network will have sufficient redundancy to achieve the required project accuracy of a minimum of Second order, Class 2 as stated in the FGCS publication Specifications and Procedures to Incorporate Electronic Digital/ Bar-Code Leveling System version 4. 1 5/27/2004. Vertical measurements will be performed using a Trimble Dini 22 Digital Level and two ( 2 ) Invar type rods . Task 03 : Adjustments/ Reports/ Blue Book Submittal The CONSULTANT will perform computations and adjustments necessary for proper Quality Control / Quality Assurance as they relate to the required accuracies . The CONSULTANT will prepare Blue Book digital files , field documentation , and reports necessary for submittal to , and eventual publication by NGS according to the latest NGS Blue Book specifications : Input Formats and Specifications of the National Geodetic Survey Data Base VOL. I, dated 1994 and 1998 . The vertical values will be furnished to the COUNTY in U . S . Survey feet ; relative to the North American Vertical Datum of 1988 ( NAVD88 ) . The CONSULTANT will prepare a Blue Book project report relating the methods used during the survey and will submit survey for review and publication by NGS . EREECH Et`.dC;; iNEERS tlVC . Indian River County VERTICAL DENSIFICATION PROJECT 2011 July 25 , 2011 Page 3 of 6 Final Drawings and Deliverables The CONSULTANT will prepare a signed and sealed Survey Report relating the methods used during the survey, accuracy achieved , and a list of final coordinates in accordance with the " Minimum Technical Standards" for surveying and mapping in the State of Florida as set forth by the Florida Board of Professional Surveyor's and Mappers in Chapter 5J - 17 , Florida Administrative Code , pursuant to Section 5J - 17 . 052 Geodetic Control Surveys, Florida Statutes . DELIVERABLES : 1 . ) Supply five ( 5 ) signed and sealed Survey Reports . 2 . ) Raw data reports of all observations and adjustments pertinent to final product . 3 . ) Copy of all field notes pertinent to final product in PDF or Word format , provided on CD - ROM . 4 . ) The CONSULTANT shall prepare its Work Product so that it can be readily used by the COUNTY or other consultants for a variety of purposes . It shall contain all information necessary for an independent third - party CONSULTANT or entity to independently locate and utilize the vertical control network . All final products , drawings , files or information related to this project shall become property of Indian River County and remain so for the use of the County and Public at large . SCHEDULE The CONSULTANT shall complete the field Monumentation , vertical measurements and survey report within 130 calendar days of Notice to Proceed from the COUNTY. A preliminary submittal will be made to the National Geodetic Survey within 160 calendar days of Notice to Proceed . ADDITIONAL SERVICES The CONSULTANT will provide , for a fee , additional services that require analyses beyond those described in the above task( s ) . These additional services may include , but not limited to : A . Additional surveying services . B . Additional data formatting and submittal to agencies other than specifically stated herein . C . Access to restricted areas , or remote areas requiring special access and/or security clearance . FEES AND BILLING This proposal , as well as the attached General Contract Provisions , represents the entire understanding between the COUNTY and the CONSULTANT concerning the PROJECT . Billing will be invoiced on a monthly based percentage complete for Lump Sum Tasks and for services actually provided for Time and Material ( hourly) Tasks . Invoices are payable as per the original Professional Surveying and Mapping contractual agreement between the COUNTY and the CONSULTANT.
