HomeMy WebLinkAbout2010-168 T 13 - 10 SWE . 4010 169 CONTRACT FOR EAP SERVICES Whereas Indian River County Government , herein referred to as Customer, desires to furnish its employees and their families with a comprehensive employee assistance program ( EAP) and Corporate Care Works , Inc . herein referred to as CCW , desires to provide these services , the undersigned parties do hereby mutually agree to the following : 1 . CORPORATE CARE WORKS , INC , HEREIN REFERRED TO AS CCW, WILL BE RESPONSIBLE UNDER THIS CONTRACT : a . To provide in - person EAP counseling sessions up to 6 in - person sessions per problem situation per year for each employee and eligible family members . Licensed counselors will provide these sessions by conducting an EAP assessment , short-term counseling , and/or linkage to community resources based on the employee or their family members ' needs and counselor recommendations . These services will be provided within a 30- miles drive from employee ' s work or home location where possible . CCW EAP will assist with stress , family problems , conflicts , alcohol/drug problems , and any other personal concern that can adversely affect productivity . CCW will provide referral to appropriate childcare , eldercare , legal , financial and community services for each employee and eligible family members requesting such assistance . Telephonic and web- based services are included . b . Referrals for ongoing services needed will be the employee/family member' s responsibility and will always be at a preferred discounted rate , covered through health insurance , based on what the employee/family member can afford to pay , or free based on the community resource that counselor determines to be the best match to the client ' s needs for problem resolution . c . To provide EAP policy and recommended procedures for program operation . d . To design and provide EAP training modules for supervisors and managers in addition to employee orientations . Additionally , CCW will conduct twelve ( 12) one- hour wellness seminars . e . To provide initial and ongoing promotional materials and publicity items e . g . posters , cards , brochures , etc . , and web based EAP awareness . f. To provide unlimited consultation to Customer regarding maximizing their return on their EAP investment . g . To provide unlimited consultation and coaching to Customer' s management and human resources staff to assist management in making effective referrals to EAP services , strategizing with management regarding recommended best practice approaches , and promoting optimal productivity and performance of employees . u b h . To staff a 24- hour helpline with experienced mental health professionals to respond immediately to calls for assistance . i . To provide statistical EAP Impact/Utilization Reports both quarterly and annually . j . To provide immediate crisis management consultation and response plan that includes Crisis Responder mobilization for same day onsite group debriefing services . These debriefings are typically 1 - hour in length and will be provided by CCW per incident . There is no limit to how many incidents to which CCW will respond . k . Includes mobilizing responders for same day onsite services based on customer need . I . To provide online services which include articles and resources in the areas of health , legal , financial , personal growth/online training and other tools to promote employee work/life balance . 2 . CUSTOMER WILL BE RESPONSIBLE UNDER THIS CONTRACT : a . To appoint an individual to coordinate program activities between CCW and Customer. b . To provide feedback concerning program operation . c . To coordinate benefit information with CCW staff. 3 . CCW SHALL BE COMPENSATED FOR THE AFOREMENTIONED DESCRIBED EAP SERVICES AS FOLLOWS : PER EMPLOYEE Rate : $ 1 . 65 per employee , per month Current Employee Count BOCC : 11118 Current Employee Count Sheriff' s Office : 533 Total Employee Count : 1651 Terms : Payable net 30 days from date of invoice ADDITIONAL SERVICES For additional training , onsite counseling and/or services outside the scope of EAP services described herein , CCW will be compensated at the rate of $225/hour, payable net 30 days from date of invoice . 4 . CONTRACT TERM LENGTH & TERMINATION CLAUSE : This contract is effective October 1St 2010 and will run continuously upon signing unless and until terminated or amended by either party in writing within the provisions of this contract . Customer or CCW may cancel this contract , without cause , with sixty (60 ) days written notice to the other party . 5 . HOLD HARMLESS CLAUSE : a . CCW will maintain a professional liability insurance policy with $3 , 000 , 000 aggregate limits and $ 1 , 000 , 000 limit per claim and will name Customer as an additional named insured . CCW is responsible for furnishing Customer with a copy . b . CCW, hereby agrees to and shall indemnify , defend and save harmless Customer, its officers , employees and agents from and against any and all liabilities , losses , claims , damages and expenses which may arise out of demands , claims , litigation and judgments ledged against them , jointly or separately as a result of negligence of CCW, its officers , employees and agents , in the performance contract , excluding negligence of Customer, or their officers , employees and agents . than Ri er County Governmen Da e r orat Care Works , Inc .
