The COUNTY has selected the CONSULTANT to perform the professional services set forth on Attachment A, attached to this Work Order and made part hereof by this reference. The professional services will be performed by the CONSULTANT for the fee schedule set forth in the Attachment B, attached to this Work Order and made a part hereof by this reference. The CONSULTANT will perform the professional services within the Schedule more particularly set forth in Attachment A, attached to this Work Order and made a part hereof by this reference all in accordance with the terms and provisions set forth in the Agreement. Pursuant to paragraph 1.4 of the Agreement, nothing contained in any Work Order shall conflict with the terms of the Agreement and terms of the Agreement shall be deemed to be incorporated in each individual work Order as if fully set forth herein. COMPENSATION. The COUNTY agrees to pay, and the CONSULTANT agrees to accept, a fee for services rendered according to the "Scope of Services" identified in the attached EXHIBIT "B" Fee Schedule and Man Hours. The site assessment addendum activities are based on the Florida Department of Environmental Protection's (FDEP) review of the Supplemental Interim Source Removal Report, which recommended supplemental groundwater assessment activities to delineate the extent of the arsenic groundwater impacts with the submittal of a Site Assessment Report Addendum (SARA). The arsenic groundwater impacts must be delineated to the FDEP Groundwater Cleanup Target Level of 10 micrograms per liter (gg/L). The proposed scope of work for the services anticipated to delineate and monitor the arsenic groundwater impacts has been broken down into six separate tasks as described below. XR09305/CCR09108.DOC sAi-iOl_O('1s scjelihLIt ( 1P"mw( llojkk Mr. Mike Nixon 30 November 2009 Page 3 Task 1— Preliminary Activities Geospte& consultants Task 1 includes project management for the entire project, as well as all preliminary activities associated with securing project costs from subcontractors and updating the Site -Specific Health and Safety Plan to cover the additional assessment activities. Specific activities include: • Updating the existing OSHA -required, Site -Specific Health and Safety Plan; • scheduling a utility locate, acquiring field equipment (pumps, water quality meters, calibration solutions, sampling containers, etc.), subcontracting; and • project planning and management responsibilities, invoice review, project coordination, and project administration. Task 2 — Monitoring Well Installation Activities Task 2 consists of mobilizing to the site, installing and sampling monitoring wells, and collecting a soil core for lithologic characterization. Geosyntec proposes to install six monitoring wells using direct push technology (DPT) drilling techniques at the locations presented on Figure 1. The monitoring wells will be constructed of 1 -inch PVC and will be installed to a maximum depth of 15 feet below land surface (ft BLS) with 10 -ft of 0.010 -inch slotted screen with a 20/30 pre -packed sand filter. The monitoring wells will be completed with a 24 -inch by 24 -inch concrete pad with an 8 -inch manhole. For the monitoring wells located in the wooded area, the wells will be completed with the 24 -inch by 24 -inch concrete pad with a 4 -inch PVC casing that is approximately 3 -feet tall (instead of the 8 -inch manhole). Once installed, the monitoring wells will be developed until visibly free of particulate matter. Following installation, the newly installed monitoring wells and existing site monitoring wells will be purged and sampled according to FDEP Standard Operating Procedures (SOPS). The collected samples and one quality assurance sample (duplicate) will laboratory analyzed for arsenic by EPA Method 6010. In addition, water levels will be collected from the newly installed monitoring wells and the existing site monitoring wells to document the groundwater elevations and flow direction at the site. Surveying (location, top -of -casing and ground elevations) by a licensed professional land surveyor of the newly -installed monitoring wells and the current site monitoring wells will be also completed. XR09305/CCR09 ] 08.DOC 01'-Ic111t't CS scienlistti Mr. Mike Nixon 30 November 2009 Page 4 Geosyntec consultants The current cost breakdown of the monitoring well installation assumes only six monitoring wells will be required to document delineation of the arsenic groundwater impacts. An additional 100% contingency has been added to Task 2 to cover any additional monitoring well installations required to achieve arsenic groundwater delineation. Geosyntec will notify IRC and secure approval for any activities that may require access to contingency fees. Geosyntec's proposal assumes one day of oversight for the monitoring well installations, one and a half days of oversight for the monitoring well sampling, and a half a day for completing the "windshield survey" for the water supply well survey described below. A breakdown of estimated project costs is included as Attachment B. Task 3 — Water Supply Well Survey Geosyntec will perform a "water supply well survey" in an attempt to identify and obtain information on area water supply wells, as required under Chapter 62-780.600(3)(h) F.A.C. This survey will focus on locating public water supply wells located within 0.5 -miles of the facility boundary and private water supply wells (including irrigation wells) located within 0.25 -miles of the facility boundary. The goal of the survey is to ascertain whether or not the site is located within proximity to supply wells in the area which could be affected by the arsenic impacts and/or could affect area groundwater flow. The following subtasks will be performed: • coordinating with IRC to request and obtain information on County water supply wells located in the area; • contacting the St. Johns River Water Management District, the local health department, and IRC utilities, to request information on permitted supply wells located in the area; • performing a "windshield survey" from public roads to look for evidence of private water supply wells (to be completed during the monitoring well sampling event described above) within 0.25 -miles of the facility boundary; and • determining whether the Site is located within an established regulated wellhead protection zone of a public water supply well or well field. Additionally, Geosyntec will attempt to identify adjacent surface water bodies which could be potentially affected by or influence onsite contamination. XR09305/CCR09108. It is anticipated that the SARA will include the following: • brief description of field activities; • results of the well survey; • general lithologic characterization; • summary tables of the groundwater sampling results and groundwater elevations; • maps that show the locations of the monitoring well network, provide a summary of the groundwater sampling results and provide a summary of the groundwater elevations; and • recommendations. A draft copy of the report will be provided to IRC for review. Any comments from IRC will be incorporated into the SARA and a final version of the report will be prepared. One hard copy and one electronic copy of the final report will be submitted to the FDEP for review and two hard copies of the fmal report will be submitted to IRC. The final reports will be signed/sealed by the Florida Licensed Professional Engineer in responsible charge of the project. A breakdown of estimated project costs is included as Attachment B. Task 5 — Semi -Annual Monitoring Well Sampling Task 5 is included to cover one semi-annual sampling event for the monitoring well network, which will likely be required by the FDEP, assuming a Natural Attenuation Monitoring program is approved for the site by the FDEP. This event would occur after completion and submittal of the SARA to the FDEP and approximately six months after the monitoring well network was sampled previously existing wells and This event includes the sampling of the monitoring well network (three six newly installed wells). It also includes collecting a set of water level measurements from the monitoring well network. The monitoring wells will be sampled and purged according to the FDEP SOPS and the groundwater samples collected will be laboratory analyzed for arsenic by EPA Method 6010. Task 6 — Semi -Annual Sampling Letter Report Geosyntec will prepare a Semi -Annual Sampling Letter Report after completion of the semi- annual monitoring well sampling event. ASSUMPTIONS The following assumptions were made in developing this fee proposal: • All site work can be performed in Level D personal protective equipment. • Geosyntec will be able to work unimpeded Monday through Friday from 7 am to 7 pm. • Geosyntec has assumed that liquids generated during the monitoring well purging and sampling will be discharged directly onto the ground adjacent to the monitoring well (for percolation or evaporation). SCHEDULE Geosyntec anticipates that upon approval from IRC, site assessment addendum activities will be initiated within four weeks. It is anticipated that the monitoring well sampling results will be available approximately two weeks after completion of the sampling. The draft SARA will be provided to IRC within four weeks of completion of field activities. ESTIMATED BUDGET The fee proposal is provided as Attachment B. The total estimated budget to complete the work described in the fee proposal is $31,432 (which includes the 100% contingency on Task 2).
