AMENDMENT TO LEASE AGREEMENNT° ly -0 y 8 1-!
Landlord and tenant do hereby agree to amend the Lease agreement dated July 7, 1992, with
amended annual rent due to an increase in property insurance of $71.87 per month as follows:
Monthly Rent Prorated Amount Revised Annual Rent
February 1, 2014 through September 30, 2014 $3.186.45 $25.491.60 $37.950.00
October 1, 2014 through September 30, 2015 $3.306.25 $39.675.00
October 1, 2015 through September 30, 2016 $3.426.04 $41.112.50
If the property taxes or insurance for the premises has any increases or decrease during the
term of this lease a prorate share of the additional expense shall be paid by Lessee to Landlord
as additional rent upon written demand by Landlord or result in a reduction paid by Tenant to
Landlord ._
IN WITNESS WHEREOF, Landlord and Tenant have executed this amendment to lease
agreement to Lease this _ day of June, 2014, nunc pro tunc February 1, 2014
