RE : Closeout Report and Request for Modification 2 ( Closeout Budget Amendment)
# 08DB - D3 - 10-40 -01 -A 12 Indian River County Hurricane Wilma DRI CDBG
Dear Mr . Burgess :
Please accept the enclosed closeout report and request for modification number two (2) for the
above referenced Community Development Block Grant (CDBG) Agreement between Indian
River County and the Florida Department of Community Affairs (DCA) . The request for
modification is to revise the Program Budget and Scope of Work, Activity Work Plan, and
Change in the Number of Accomplishments and/or Beneficiaries .
The request does not reduce the number of accomplishments and/or beneficiaries , or score of
the project . In fact, the program has been quite successful . The County has served more
beneficiaries than previously anticipated and has done so without utilizing all of the County ' s
allocated DRI CDBG funds . Thus, the final modification and closeout reflects an increase in the
number of accomplishments and beneficiaries served and reflects a slight surplus of DRI
CDBG funds (to be de - obligated) .
Please process the County ' s request for modification two (2) , and thereafter the County ' s
closeout report . Please note that a final request for funds was submitted to DCA on January 3 ,
2011 . At the time that the grant modification and closeout forms were prepared , that request for
funds had not yet been processed .
