WORK ORDER NUMBER _4 IQ — /G g , This Work Order Number _4_ is entered into as of this 18th day of September 2012_ , pursuant to that certain Continuing Contract Agreement for Professional Services entered into as of November 15 , 2011 , (the "Agreement" ) by and between INDIAN RIVER COUNTY , a political subdivision of the State of Florida ( " COUNTY " ) and KIMLEY- HORN AND ASSOCIATES , INC . ( " CONSULTANT " ) . The COUNTY has selected the Consultant to perform the professional services set forth on Exhibit 1 ( Scope of Work ) , attached to this Work Order and made part hereof by this reference . The professional services will be performed by the Consultant for the fee schedule set forth in Exhibit 1 ( Fee Schedule ) , attached to this Work Order and made a part hereof by this reference . The Consultant will perform the professional services within the timeframe more particularly set forth in Exhibit 1 (Time Schedule ) , attached to this Work Order and made a part hereof by this reference all in accordance with the terms and provisions set forth in the Agreement . Pursuant to paragraph 1 . 4 of the Agreement , nothing contained in any Work Order shall conflict with the terms of the Agreement and the terms of the Agreement shall be deemed to be incorporated in each individual Work Order as if fully set forth herein . Bair ,, County Administrator pproved as to for? al sufficiency : Alan Polackwich , County Attorney Exhibit # 1 Work Order Number 4 Stormwater Supply to Algal Turf Scrubber System at 5th Street SW Site South RO Plant Concentrate Disposal PROJECT DESCRIPTION This work order 94 includes additional survey , pot- holing , design , site planning support, permitting and construction phase services to account for a change in the proposed pipeline route and relocated Algal Turf Scrubber (ATS ) site to east of the Oslo Road water treatment plant, beyond what was included in the original Work Order #2 . In summary , the additional scope of work in general , consists of. Pump Station Site and 27th Avenue Bridge The pump station will be located west of the 27th Avenue bridge over the South Relief Canal and require additional survey and design to cross the canal and extend up to 100 ft west of the bridge . This work order includes the additional services required to extend the stormwater pipe to the selected location . It is anticipated that the stormwater pipe will be installed as aerial attached to the existing bridge crossing the South Relief Canal . • Provide additional survey , pot-holing , and design of pipe over the bridge and into the pump station site . • A specific purpose survey and document will be prepared for the pump station site , which will consist of locating encroachments , and topographical information at the pump station location . 27th Avenue Road Crossin ;? (EnOneering IRC Proiect #0391 ) The Indian River County Roadway Division ( IRCRD ) is planning a widening project for a portion of 27th Avenue and 5t" Street between July 2012 and January 2013 . In order to prevent potential damage to this new widened section from the proposed ATS pipeline construction , a phasing plan must be prepared for the construction of a portion of the ATS pipeline within this project . • Consultant will prepare the phasing plan and provide additional coordination with IRCRD during construction phase of this work. Environmental Resource Permitting The County is preparing an Environmental Resource Permit ( ERP) application to submit to the Florida Department of Environmental Protection ( FDEP) for the proposed stormwater pump station , ATS site and , and ATS stormwater pipe . It is our understanding that K : AWP [3_Civi1\044572021 - ATS Stormwater Pipe\ ScopeAATS Pipeline Project - Work Order 4 REV I . doc Page I ol' 9 Kimley - Horn is only responsible for permit documents related to the stormwater pipe and that others will be responsible for the preparation of the ERP application and all supporting documents related to the ATS site and pump station . • This work order includes additional permitting services to prepare documents for the ATS stormwater pipe that can be included in the County ' s ERP application to FDEP . ATS Site Support • Provide support in preparing a " submittable" site plan based on survey and design layout provided by IRC and include water treatment plant property . • Prepare landscape plan and perimeter sections . Prepare Clearing Plan and address status of native and protected trees . • Paving , Grading and Drainage plan for site driveway connection to 5th St SW and internal roadways . IRC to prepare ATS site specific paving , grading , drainage plan . • Prepare survey of sub- lateral E- 7 canal for drainage and peak stage analysis , conduct drainage calculations of canal capacity and coordination with Indian River Farms Water Control District ( IRFWCD) . • Prepare and design ATS outfall and improvements to sub - lateral E- 7 canal outfall structure into Lateral J canal . • Prepare utility plan of water treatment site , consisting of concentrate line , security , fencing , and access . • Technical Review Committee , Planning and Zoning , and Board of County Commissioners meeting attendance and preparation . The following scope of work under this work order consists of. SCOPE OF SERVICES Task 1 — SURVEY AND UTILITY LOCATES The South Relief Canal pump station is to be located in the South Relief Canal ' s north right- of- way approximately I OM west of 27th Avenue ' s west right of way . Topographic Route Survey will be performed to extend the limits of the survey included in Work Order # 2 to include the proposed pump station . The limits of the survey will be 50M west of the 27th Avenue ' s west right- of-way , 27th Avenue ' s east right- of-way , 25 - ft north of the north canal right-of- way , and 25 -ft south of the south canal top- of-bank . The topographic survey will include canal cross - sections every 50 ' beginning at the 27th Avenue western right-of- way , I - ft contours for the entire surveyed area, and observed physical features . Additional Topographic Route Survey will also be performed along sub- lateral E - 7 canal from station 254 + 50 to 288 + 75 ( Lateral J canal ) . Canal cross sections at 100 ft intervals along the route , including outfalls into the canal , or approximately 34 cross section locations . K :AW PB_Civil\044572021 - A FS Stormwater Pipe\Scope\ATS Pipeline Project - Work Order 4 REV I . doc Page 2 of9 Additional topographic and location survey at the water plant area west of the new ATS site survey with the limits being 100 tt west of the eastern property boundary , the north jog in the fence line , then south to 100 ft south of the driveway into the parking lot . Vertical underground locates of known existing utilities will be required at areas where potential conflicts exist and at tie - in locations . This information is necessary to collect location , depth , and pipe size/material that will be required for the design . It is expected that approximately three (3 ) holes in the South Relief Canal /271h Avenue right- of-- ways and drainage easement will be performed for the pipe extensions . Task 2 - DESIGN 27th Ave Bridge The Consultant will perform structural calculations to design the connections to the existing 27th Avenue Bridge over the South Relief Canal . The design of the bridge will also incorporate relocating the forcemain that is currently attached to the west side of the bridge . Since the FM is less than I OM in length , it is assumed that a FDEP permit will not be required . It is understood that the stormwater pump station will be designed by others and that the Consultant will be provided with information regarding the tie - in locations to the stormwater pipe route . ATS Site The Consultant will prepare design drawings of the stormwater piping to extend it to the property line of the ATS site on the east end . Existing utility information provided by IRCU and others will be used in conjunction with the survey and underground locate information as a basis for design of the piping alignment . The design drawings will be incorporated into the design drawings that are being prepared as part of Work Order #2 . Technical specifications will be prepared as described in Work Order #2 . It is anticipated that the following drawings will be updated to include the additional stormwater pipe . • Update one ( 1 ) horizontal plan views of alignment to include pipe extension to the proposed pump station . • Plan and profiles for canal crossing , one ( 1 ) sheet • Construction details for connections to the bridge 27th Avenue Road Crossing Coordination with Public Works Consultant will prepare a separate set of construction drawings for the 27th Avenue crossing . It is anticipated that this set will contain a cover sheet, a plan sheet of the crossing, and construction details . All notes and specifications will be included on the construction drawings as a separate set of technical specifications will not be prepared . Consultant will submit the design drawings as described in Work Order #2 . Task 3 —PERMITTING ERP Permitting Services Consultant will prepare an exhibit illustrating the route of the ATS stormwater pipe and a project description that can be included with the County ' s ERP submittal to FDEP . As requested by FDEP , the exhibit will include the approximate limits of the HDD staging areas and known wetlands that the ATS stormwater pipe will be crossing . It is assumed that the ATS stormwater pipe can be designed with either a minimum 15 - ft clearance from any known wetlands or as an HDD installation under known wetlands and therefore additional environmental or permitting services will not be required . It is our understanding that Kimley - Horn is only responsible for permit documents related to the stormwater pipe and that others will be responsible for the preparation of the ERP application and all supporting documents related to the ATS site and pump station . Additionally , it is our understanding that all coordination with FDEP including application submittals will be handled by others . Task 4 — CONSTRUCTION PHASE SERVICES 27th Ave Roadway Crossing The Consultant will provide additional construction phase services for the project related to the improvements included in this Work order. The additional construction phase services for this project will consist of limited shop drawing and pay application review , meeting attendance , and construction field observation for the construction of the 27th Avenue crossing that will be performed as part of the 5th Street S . W ./27th Avenue road widening project . In is anticipated that the Consultant will review one ( 1 ) set of shop drawing submittals . The shop drawings will be reviewed for conformance with the intent of the contract documents . The Consultant will review the submittal and return copies to the contractor and Indian River County Utilities and Indian River County Roadway Division . The Consultant will also review one ( 1 ) pay request and provide a recommendation for payment to Indian River County Utilities for the work . The Consultant will attend one ( 1 ) project kickoff status meeting with the Contractor and Owner. Construction observation will be provided by a field inspector and is anticipated to be for up to two (2 ) field visits . The Consultant' s visits will be for the purpose of endeavoring to K : AW11B_Civi1\044572021 - ATS Stortnwater Pipe\ScopeAATS Pipeline Project - Work Order 4 REV l . doc Page 4 of9 provide the Client a greater degree of confidence that the completed work of its contractors will generally conform to the construction documents prepared by the Consultant. The Consultant neither guarantees the performance of contractors , nor assumes responsibility for any contractor ' s failure to perform its work in accordance with the contract documents . Work that the Consultant believes does not comply with the contract documents will be brought to the attention of Indian River County Utilities and the contractor. The Consultant shall have no responsibility for any contractor' s means , methods , techniques , equipment choice and usage , sequence , schedule , safety programs , or safety practices , nor shall the Consultant have any authority or responsibility to stop or direct the work of any contractor. Record drawings will be prepared for the 27t" Street crossing in accordance with Work Order #2 . Task 5 — ATS SITE PLANNING Consultant will provide support in preparing a " submittable " site plan in accordance with the IRC site plan submittal standards based on survey and design layout furnished by IRC , and will show the water treatment plant property . Consultant will prepare landscape plan and perimeter sections ; a clearing plan which will address status of native and protected trees ; and paving , grading and drainage plan for the site driveway through the water plant site to the ATS site and connection to 5t St SW and internal roadways . Is it understood that IRC will prepare the ATS site specific paving, grading , drainage plan within the ATS site . Consultant will prepare and design ATS outfall and improvements to sub - lateral E- 7 canal outfall structure into lateral E canal . Consultant will meet and coordinate design criteria with IRFWCD . Consultant will prepare utility plan of water treatment site , consisting of concentrate line , security , fencing , and access to the ATS site . Consultant will prepare for and attend the two Technical Review Committee meetings and one Board of County Commissioners meeting . Consultant will perform a hydraulic analysis of the Lateral E- 7 canal to determine if it has sufficient capacity to handle treated effluent from the ATS site . It is assumed that the Lateral J canal has sufficient capacity and will not be included in the analysis . It is anticipated that the following tasks will be performed as part of the hydraulic analysis : • Determine the service area for the Lateral E- 7 canal based on information from IRFWCD and available aerials , St . John ' s River Water Management District permit information , and coordination with the Indian River Farms Water Control District . This information will be presented on an aerial exhibit. • Calculate the design capacity of the Lateral E- 7 canal based on the Indian River Farms Water Control District discharge limit of 2 "/acre during the 25 - year, 24 - hour storm event . This limit will be applied to the total service area of the E- 7 canal . K : AWPB_Civil\044572021 - ATS Stornmater PipeAScope\ATS Pipeline Project - Work Order 4 REV Ldoc Page 5 of9 • Calculate the hydraulic capacity of the Lateral E - 7 canal between the proposed ATS site and the Lateral J canal based on survey information and available peak stage information for the Lateral E- 7 and Lateral J canals . • Compare the design capacity of the E- 7 canal with the hydraulic capacity of the canal to determine if there if there is sufficient capacity in the Lateral E - 7 to handle the maximum design flow from the ATS site . • Perform headloss calculations between the proposed ATS site and the Lateral E- 7 canal to quantify how much the hydraulic grade line will increase with the addition of flows from the ATS site . Consultant will perform calculations for and design a velocity dissipater for the area where the Lateral E- 7 canal discharges into the Lateral J canal . It is anticipated that the velocity dissipater will consist of a section of rip rap that will be sized to reduce the scouring in the Lateral J canal . A plan sheet with notes and details will be added to the construction documents being prepared for ATS Stormwater FM project . Consultant will design a control structure from the proposed ATS site to the Lateral E- 7 canal for the ATS effluent discharge . The control structure will be added to the construction documents being prepared as part of the ATS Stormwater FM project and a new construction detail sheet will be prepared . Consultant will attend regularly scheduled bi - monthly progress meetings until January 2013 . ADDITIONAL SERVICES The following services are not included in the Scope of Services for this project , but may be required depending on circumstances that may arise during the execution of this project . Other items that may be considered additional services are mentioned herein . Additional services include , but may not be limited to the following : • Construction phase services beyond what is described in the Scope of Services . • Traffic studies • Building department coordination and structural design • Environmental studies , including scrub jay and gopher tortoise surveys , wetland delineation , and protected species determination • All coordination with FDEP including application submittals will be handled by others . INFORMATION AND SERVICES PROVIDED BY OWNER The Owner shall provide the following information related to the design of this construction project : • Stormwater pump station design , including site and mechanical by IRC public works • Boundary survey , base survey , tree survey , and topographical information of new ATS site by IRCU ( Masteller, Moller, and Reed) • Record drawings of water and sewer utilities along the determined route K : AWPB Civil\044572021 - ATS Stormwater Pipc\Scope\ATS Pipeline Project - Work Order 4 RLV I . doc Page 6 of 9 • Information including design drawings for the proposed tie - in location in the South Relief Canal north right- of- way and at the ATS property line . Consultant will work as expeditiously as practical and within a mutually agreed upon schedule . Services for these tasks are anticipated to be completed within 12 weeks of notice to proceed .
