('allow up to FDEP 4- log Virus Treatment Request for Additional Information
North and South RO Water Treatment Plants
Project Understanding
Florida Department of Environmental Protection has requested additional information to
demonstrate 4-log virus removal that requires recalculation of the way 4- log credit for virus
reduction is considered . Due to this and what is believed to be unreasonable requests for some
additional information, additional clarification is needed which is beyond the scope of Work
Order # 19 .
SCOPE OF SERVICES
Consultant will recalculate 4 - log credit methodology considering free chlorine as the primary
mode of virus removal and achieving 99 . 99% virus removal/inactivation . Consultant will prepare
revised calculation, prepare additional process flow diagrams, and submit a response to FDEP ' s
request dated July G , 2010 , for conditional approval of 4-log credit .
SCHEDULE
Consultant will prepare a draft demonstration of 4- log removal with attachments for Owner
review within two (2) weeks of notice to proceed .
FEES
We will provide these services in accordance with our Continuing Services Contract Agreement
for Professional Services dated April 7 , 2007 with Indian River County.
"I'lic Consultant will provide professional services on a lump sum fee basis of $ 3 , 100 .
Additional Services .
The fallowing services are not included in the Scope of Set-vices for this project, but may be
required depending on circumstances that may arise during the execution of this project .
Additional services include , but may not be limited to the following :
C Design drawings of any addition to the facility as requested by MET .
o Pennit applications for any plant component additions which may be necessary.

AMENDMENT NUMBER 1 TO WORK ORDER 19
This Amendment Number 1 to Work Order 19 ( "Amendment" ) is entered into as of
this day of August 17 , 2010 ( " Effective Date" ) pursuant to that certain Continuing
Contract Agreement for Professional Services entered into as of April 71 2007
( "Agreement" ) , by and between Indian River County , a political subdivision of the State of
Florida ( "COUNTY" ) and Kimley- Hom and Associates Inc . ("CONSULTANT' ) .
1 . The COUNTY has selected the Consultant to perform the professional
services set forth in Work Order Number 19 dated January 12 , 2010 .
2 . The COUNTY and the Consultant desire to amend Work Order Number 19
as set forth in Attachment 1 attached to this Amendment and made a part hereof by this
reference , all in accordance with the terms and provisions set forth in the Agreement .
3 . From and after the Effective Date of this Amendment , Work Order Number
19 is amended as set forth in this Amendment . Pursuant to paragraph 1 . 4 of the
Agreement, nothing contained in any Work Order shall conflict with the terms of the
Agreement and the terms of the Agreement shall be deemed to be incorporated in each
individual Work Order as if fully set forth herein .
IN WITNESS WHEREOF , the parties hereto have executed this Amendment as of
the Effective Date first written above .
