CONTINUING CONTRACT AGREEMENT FOR
PROFESSIONAL SERVICES
AMENDMENT #1 TO WORK AUTHORIZATION NO. 6 TO THE CONTINUING
CONTRACT AGREEMENT FOR PROFESSIONAL SERVICES BETWEEN KIMLEY-
HORN AND ASSOCIATES, INC. AND INDIAN RIVER COUNTY, FLORIDA.
This is an amendment to the existing Continuing Contract Agreement (AGREEMENT) dated
November 15th, 2011 between Kimley-Horn and Associates, Inc (ENGINEER) and Indian River
County (COUNTY) This work authorization addresses changes in 'Scope of Services" and
'Compensation" of the AGREEMENT.
Amendment #1 Work Authorization Description
This Work Authorization includes the following:
"SCOPE OF SERVICES" is being modified to incorporate the following:
A. Revisions to Gifford Neighborhood 45th Street Streetscape Design Plans
ENGINEER will provide professional services as specifically stated below:
1. Addition of Bike Lanes, Curbing of Street Intersections & H-3 Canal Culvert
Extensions at Street Intersections — Based on our meeting with County staff April
24, 2014 the COUNTY requested that the ENGINEER revise the landscape
construction plans to include 5 -ft paved shoulders along both sides of 45th Street
between 43`d Avenue and US Highway 1, provide for a westbound right turn lane
at 28th Court and provide for clear zone mitigation measures at the northern side
street connections along 45th Street by adding curbing.
In addition, the ENGINEER was requested to delineate future improvements to
the southern side street connections to 45th Street consisting of side drain
replacements within Indian River Farms Water Control District Lateral H-3 canal
to extend the canal culverts +/- 50' on either side of the cross streets to allow
more landscape opportunity.

2. Revisions to Final Landscape Design Plans — After COUNTY has reviewed and
approved the Revised Plans prepared in Task 1 above, the ENGINEER will revise
and prepare final bid -ready documents using CADD technology suitable for submittal
to COUNTY for permitting and bidding by Contractors, including planting details
specifications, and general notes plan sheet.
ENGINEER will provide three (3) hardcopy sets of plans.
"COMPENSATION" is being modified to incorporate the following:
COUNTY agrees to pay and ENGINEER agrees to accept for services rendered pursuant
to this Agreement fees inclusive of expenses in accordance with the following:

The basic compensation mutually agreed upon by ENGINEER and COUNTY is
as follows:

The AGREEMENT is hereby amended as specifically set forth here in. All other sections of the
AGREEMENT shall remain in full force and effect and are incorporated herein.
This Amendment #1 to Work Authorization No 6 to the AGREEMENT regardless of where
executed, shall be governed by and construed by the laws of the State of Florida.
In witness whereof the parties have executed this Amendment this 17 t h day of
