FOR
PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Masteller , Moler, Reed and Taylor, Inc.
In accordance with Contract No . 1032 2011 -2012
This Work Order No . 3 is in accordance with the existing AGREEMENT dated October 19 , 2010
between Masteller, Moler, Reed and Taylor, Inc . , (SURVEYOR) and Indian River County
(COUNTY) , renewed on October 19 , 2011 for 2011 -2012 . This Work Order No . 3 amends the
agreement as follows :
SECTION I — PROJECT LIMITS
This Work Order No . 3 is for the SURVEYOR to perform all related field and office Surveying
and Mapping services in connection with the Right-of-Way and Topographic Survey of 87th
Street from CR 510 to 101 st Avenue .
SECTION II - SCOPE OF SERVICES
As agreed upon between SURVEYOR and COUNTY, the SURVEYOR shall provide
Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 3 ;
specifically detailed in the attached proposal Exhibit A .
SECTION III — TIME FOR COMPLETION & DELIVERABLESIWORK PRODUCT
1 , Project shall be completed as follows :
a . 100 % " Paper" review submittal (final review prior to request for final deliverables )
shall be made within 90 calendar days of receipt of Notice to Proceed for review by
County Staff prior to preparing the final submittal package .
b . Time of FINAL project completion shall be within 10 calendar days of receipt of the
County' s review comments from the 100 % " Paper Submittal " .

2 . Deliverables -The SURVEYOR shall provide the COUNTY :
a . 100% Phase Submittal for COUNTY review and comment. All submittals shall
include one ( 1 ) paper " hardcopy" along with AutoCad Files and an ASCII file for all
survey points . COUNTY shall strive to provide review comments within 10 calendar
days of the preliminary submittal .
b . FINAL Deliverables , five (5 ) paper "hardcopy " signed and sealed sets and one ( 1 )
signed and sealed MYLAR set in adequate recording fashion ( I . E . Borders) . Sheeted
and model space ( as applicable) AutoCad drawing file in release 2004 through 2009 ,
PDF formats and an ASCII file for all survey points , all electronic files to be on CD .
c. Survey set/sheets shall include a cover sheet with location sketch , survey
certifications , related title and project number , survey notes , legend and
abbreviations and plan view sheets .
d . Work Product and digital versions are to be prepared and submitted so that the
COUNTY or other consultants can readily use it for the design and analysis of the
area , as defined . It shall contain all information necessary for third -party surveyor to
independently recreate and/or utilize the survey work . It is acknowledged all final
products become property of Indian River COUNTY and will be available for use by
the public at large .
e . The SURVEYOR ' S work product shall meet or exceed the minimum standards as
defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR ' S
request for payment .
SECTION IV — COMPENSATION
The COUNTY agrees to pay , and the SURVEYOR agrees to accept , for the above described
services rendered as identified in Sections I , II , and III of this Work Order No . 3 , for a total
lump
sum fee of $21 , 195. 00 .
All and/or any additional services not described hereon shall be pre -approved by the
COUNTY . Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year( s) of October 19 , 2011
through October 19 , 2012 .
All invoicing shall include Project Number 1205 , Work Order No . 3 (WO 3) , Contract Number
( 1032) , itemized man -hours and materials expended to complete the Scope of Services .
Payments shall be in accordance with the original Professional Surveying and Mapping/GIS
Services Agreement , Contract No : 1032 with the COUNTY and as stated in Section II , III and IV
hereon .

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREEMENT shall remain in full force and effect , and are incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this 22nd day
of May 2012 ,

87th Street Right of Way and Topographic Survev
Pursuant to our Annual Land Surveying and Mapping/GIS Services Contract 1032 , MMRT , Inc . is
pleased to provide Indian River County with this proposal for performance of a Specific Purpose Survey
to establish Right of Way and a Topographic Survey for property located in Indian River County and
lying in Section 27 , Township 031 South , Range 038 East and Section 28 , Township 031 South , Range
038 East described as and to include the following :
Proiect Outline and Limits :
The Survey limits are generally described as the 87th Street Right of Way from 101 st Avenue to County
Road 510 and 10 feet outside of the limits on each side and including side streets . The survey limits
shall also include County Road 510 , from west edge of pavement extending 10 feet outside of County
Road 510 west right of way line . The approximate length of the right of way limits is 7 , 500 linear feet
( 1 . 42 miles) .
Scope of Services :
Task 1 : Right of Way and Topographic Survey
1 ) Project initiation , including review of existing deeds and surveys , files and field notes , preparation of
work orders and field crew folders .
2 ) Find , recover and measure field ties into existing survey control along the route to establish the 87th
Street Right of Way and Baseline of Survey . Find , recover and measure field ties from Baseline of
Survey to existing GPS 23 and GPS 24 monuments as applicable . Horizontal Control shall be
based upon the State Plane Coordinate System , Florida East Zone , NAD 83 .
3 ) Find and recover existing vertical control . Elevations hall be based upon the National Geodetic
Vertical Datum of 1929 ( NGVD 29 ) with a conversion to the North American Vertical Datum of 1988
( NAVD 88 ) provided . Perform bench run and establish benchmarks along the survey corridor at
maximum 1000 foot intervals .
4 ) Field monument baseline control , including at a minimum , one control point for each existing side
street . Control points shall consist of iron rod and cap or nail and disk . Control points shall be
designated on survey with station/offset and NGVD ' 29 elevation .
5 ) Field locate and record vertical ground elevation cross sections within and 10 ' outside of right of
way at even 100 ' baseline stations and all distinct locations ( inverts , retention areas , swales ,
ditches etc . ) and changes in contour along the right of way corridor.
6) Field locate and record existing improvements and man made features within and 10 ' outside of
right of way , including but not limited to , roads , swales , driveways , culverts , signage and mailboxes ,
visible utilities , fences etc , within and 10 ' outside the existing Right of Way .
7 ) Field locate and identify limits of vegetation within the right of way limits . Field locate and identify
trees , 4 " or larger, outside the any limits of vegetation but within the right of way limits .

8 ) Contact Utility locating service and have all underground utilities , such as cable TV , phone , electric ,
water and sewer lines marked within Right of Way limits . Locate and identify all markings as
applicable .
9 ) Process field data and determine limits of existing 87th Street Right of Way . Prepare survey drawing
in accordance with Minimum Technical Standards including cover sheet with location sketch ,
certifications , related title and project number, notes , legend , abbreviations and plan view sheets .
Prepare final deliverables .
Time of Completion and Deliverables :
10) Time of final completion shall be 60 calendar days from notice to proceed .
11 ) Deliverables :
a ) Final review shall include one 100 % " Paper" copy be submitted prior to the Final Deliverables .
b ) Address comments as applicable .
c) FINAL — Five ( 5 ) paper " hardcopy" signed and sealed set and one ( 1 ) signed and sealed
MYLAR set in adequate recording fashion ( I . E . Borders) . Sheeted and model space (as
applicable) AutoCAD drawing file and PDF formats on CD .
• Survey set/sheets shall include a cover sheet with location sketch , survey certifications , and
related title and project number; survey notes/legend/abbreviation sheet , and right-of-way
plan view sheets .
Compensation .
Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services
Agreement , Contract No . 1032 with the Indian River County .
