PROFESSIONAL SERVICES AGREEMENT
ANNUAL SURVEYING and MAPPING/GIS SERVICES
WITH
Masteller, Moler, Reed and Taylor, Inc .
In accordance with Contract No . 1032 2010-2011
This Work Order No . 2 is in accordance with the existing AGREEMENT dated October 19 , 2010
between Masteller, Moler, Reed and Taylor, Inc . ( SURVEYOR ) and Indian River County
( COUNTY) . This Work Order No . 2 amends the agreement as follows :
SECTION I — PROJECT LIMITS
This Work Order No . 2 is for the SURVEYOR to perform all related field and office Surveying
and Mapping services in connection with the Topographic Survey of Old Dixie Highway from
691h Street to North of CR 510 . Limits include all side street intersections for a minimum 100 feet
and all properties associated with the project .
SECTION II - SCOPE OF SERVICES
As agreed upon between SURVEYOR and COUNTY , the SURVEYOR shall provide
Professional Land Surveying services to complete all tasks as outlined in this Work Order No . 2 ;
specifically detailed in the attached proposal Exhibit A .

SECTION III — TIME FOR COMPLETION & DELIVERABLESJ'WORK PRODUCT
1 . Project shall be completed as follows :
a . Time of 100 % " Paper Submittal " review (final review prior to request for final
deliverables ) submittal shall be 120 calendar Days from notice to proceed .
b . Time of FINAL project completion shall be within 15 calendar days of receipt of the
County' s review comments .
2 . Deliverables -The SURVEYOR shall provide the COUNTY :
a . 100 % Phase Submittals for COUNTY review and comments . All submittals shall
include one ( 1 ) paper " hardcopy" along with AutoCad Files and an ASCII file for all
survey points . COUNTY shall strive to provide review comments within 10 calendar
days of preliminary submittal .
b . FINAL five ( 5 ) paper " hardcopy" signed and sealed sets and one ( 1 ) signed and
sealed MYLAR set in adequate recording fashion ( I . E . Borders ) . Sheeted and model
space ( as applicable ) AutoCad drawing file in release 2004 through 2009 and PDF
formats on CD .
c . Survey set/sheets shall include a cover sheet with location sketch , survey
certifications , and related title and Project No ; survey notes/legend/abbreviation
sheet and right-of-way plan view sheets .
d . Work Product and digital versions are to prepared and submitted so that the
COUNTY or other consultants can readily use it for the design and analysis of the
area , as defined . It shall contain all information necessary for third - party surveyor to
independently recreate and/or utilize the survey work . It is acknowledged all final
products become property of Indian River COUNTY and will be available for use by
the public at large .
e . The SURVEYOR ' S work product shall meet or exceed the minimum standards as
defined by Sections II , III and IV or the COUNTY will not approve the SURVEYOR ' S
request for payment .
SECTION IV — COMPENSATION
The COUNTY agrees to pay, and the SURVEYOR agrees to accept, for the above described
services rendered as identified in Sections I , II , and III of this Work Order No . 2 , for a total lump
sum fee of $28 , 550 . 00 .
All and /or any additional services not described hereon shall be pre -approved by the
COUNTY . Approved additional services shall be invoiced at the rates disclosed in the
approved fee schedule with the SURVEYOR for the contract year( s ) of October 19 , 2010
through October 19 , 2011 .
All invoicing shall include Project Number ( 0934A ) , Work Order No . 2 (WO 2 ) , Contract Number
( 1032 ) , itemized man - hours and materials expended to complete the Scope of Services .
Payments shall be in accordance with the original Professional Surveying and Mapping /GIS
Services Agreement , Contract No : 1032 with the COUNTY and as stated in Section II , III and IV
hereon .

The AGREEMENT is hereby amended as specifically set forth herein . All remaining sections of
the AGREEMENT shall remain in full force and effect , and are incorporated herein .
IN WITNESS WHEREOF the parties hereto have executed these presents this 24th day
of May 2011 .

Topographic Survey
Pursuant to our Annual Land Surveying and Mapping/GIS Services Contract 1032 , MMRT , Inc . is
pleased to provide Indian River County with this proposal for performance of a Topographic Survey for
property located in Indian River County and lying in Section 03 , Township 032 South , Range 039 East
and Section 33 , Township 031 South , Range 039 East described as and to include the following :
Project Outline and Limits :
The Topographic survey limits are generally described as the Old Dixie Highway Right of Way from 691h
Street to 1 00 ' feet north of County Road 510 and 25 feet outside of the limits on each side and 100 feet
each way along all side streets . The survey limits shall also include County Road 510 , 200 feet each
way of Old Dixie Highway, extending 25 feet outside of County Road 510 right of way. The
approximate length of the right of way limits is 11 , 500 linear feet ( 2 . 17 miles ) .
Scope of Services :
Task 1 : Topographic Survey
1 ) Project initiation , including review of existing deeds and surveys , files and field notes , preparation of
work orders and field crew folders .
2 ) Find , recover and tie into existing survey control based upon existing Right of Way map as
prepared by Carter & Associates to be provided by the County . Horizontal Control shall be based
upon the coordinate system on said existing Right of Way map .
3 ) Find and recover existing vertical control . Elevations hall be based upon the National Geodetic
Vertical Datum of 1929 ( NGVD 29 ) with a conversion to the North American Vertical Datum of 1988
( NAVD 88 ) provided . Perform bench run and establish benchmarks along the survey corridor at
maximum 1000 foot intervals .
4 ) Field locate and record vertical ground elevation cross sections within and 25 ' outside of right of
way at even 100 ' baseline stations and all distinct locations ( inverts , retention areas , swales ,
ditches etc . ) and changes in contour along the right of way corridor .
5 ) Field locate and record vertical ground elevation cross sections 100 ' each way along side streets ,
25 ' outside the existing rights of way at 50 ' intervals and all distinct locations ( inverts , retention
areas , swales , ditches etc . ) and changes in contour along the side street right of way .
6 ) Field locate and record vertical ground elevation cross sections 200 ' each way along County Road
510 , 25 ' outside the existing rights of way at 50 ' intervals and all distinct locations ( inverts , retention
areas , swales , ditches etc . ) and changes in contour along the side street right of way .
7 ) Field locate and record existing improvements and man made features within and 25 ' outside the
existing Right of Way and 100 ' in each direction along side streets .

8 ) Field locate and obtain west invert and structure size of all U . S . Highway 1 cross drains via
drainage outfalls , ( swales , ditches canals etc . ) . Tie to existing baseline as appropriate .
9 ) Process field data and prepare Topographic survey in accordance with Minimum Technical
Standards . Survey shall show Old Dixie right of way and baseline and related data . Prepare survey
set sheets , including cover sheet with location sketch , certifications , related title and project
number, notes , legend , abbreviations and plan view sheets . Prepare final deliverables .
Notes :
County to provided one hard copy and AutoCad digital file of Right of Way Map for Old Dixie Highway
within 10 days of notice to proceed . Phase submittals shall be provided for billing support .
Time of Completion and Deliverables :
10 ) Deliverables :
a ) Final review shall include one 100 % " Paper" copy be submitted prior to the Final Deliverables
for the project to ensure all previous comments have been addressed .
b ) FINAL — Five ( 5 ) paper " hardcopy" signed and sealed set and one ( 1 ) signed and
sealed

Compensation .
Payments shall be in accordance with the original Professional Surveying and Mapping/GIS Services
Agreement , Contract No . 1032 with the Indian River County.
