( "Consultant" ). The COUNTY has selected the Consultant to perform the professional services set forth on Exhibit 1 (Scope of Work), attached to this Work Order and made part hereof by this reference.. The professional services will be performed by the Consultant for the fee schedule set forth in Exhibit 1 ( Fee Schedule), attached to this Work Order and made a part hereof by this reference. The Consultant will perform the professional services within the timeframe more particularly set forth in Exhibit 1 (Time Schedule); attached to this Work Order and made a part hereof by this reference all in accordance with the terms and provisions set forth in the Agreement. Pursuant to paragraph 1.4 of the Agreement, nothing contained in any Work Order shall conflict with the terms of the. Agreement and the terms of the Agreement shall . be deemed to be incorporated in each individual Work Order as if fully set forth herein. Polackwich, Sr„ County Attorney EXHIBIT 1 INDLWRIVER COUNTY UTILITIESaFDEP-PERMIT-RENEWALS . - - - - _ _-_ ___ ----_-- ------- i3'BLIE CI'RESS WW3`F AND ,CEI�1TRAb GENERAL The Indian River County Central Wastewater Treatment Facility (WWTF) Operating Permit (FLA010431 ) will expire on March 29, 2014 and. the Blue Cypress W WTF .Permit (FLAO10439) will expire on April 19, 2014, Rule 62-620: 335 of the Florida Administrative Code (F.A.C.) requires that all wastewater treatment facilities renew the FDEP Operating Permit every five years . Applications for permit renewal must be submitted to .the appropriate FDEP office six months prior to the expiration of the FDEP Permit.. Therefore, the renewal application for the Indian River County Central WWTF is due on or before September 29; 201' 3 and the Blue Cypress WWTF renewal application is due on or before October 19, 2013 . The following scope is provided to address these items. SCOPE OF SERVICES Task 1 — Site Inspection CONSULTANT will conduct the required site inspections to inspect the site and meet with the plant operators. The inspections will include an evaluation of the following:: 1 . The general condition of the facility i 2 . Component, system, . and process reliability standards as required by Rule 17- 600.400( 1 )(b), F.A.C. 3 . Component; system, andprocess operations 4. Hydraulic and organic loading 5 . Safety features 6. Operator certifications T Facility maintenance program 8 . Facility records 9. Sampling procedures 10 . Laboratory analysis procedures Task 2 — Tabulation and Analysis of Plant Operating Data CONSULTANT will tabulate and analyze, as required by .FDEP, historical plant operating data, which documents plant operating conditions for the duration of the current operating permit. Plant operating data will be provided by Indian River County Utilities (IRCU) staff as specified in the "Required Information to be Provided by County" section found within this Scope of Services. Task 3 — Preparation and Submittal of FDEP Permit Renewal Application CONSULTANT will prepare and provide to FDEP and IRCU, the following applications and forms necessary to obtain the facility' s required operating permit. Process Flow Diagram 2 . Site Plan 3. . Location and Vicinity Maps 4. Facilities Description Task 5 — Preparation of FDEP Required Capacity Analysis Report CONSULTANT will prepare a Capacity Analysis Report `in accordance with Rule. 17400:405, F.A.C. This report will document the permitted plant capacity, along with our analysis and . evaluation of the WWTF flows and current plant capacity. Task 6 _ Preparation of FDEP Required Operation - and Maintenance Performance Report CONSULTANT will prepare an Operation, Maintenance and Performance Report, in accordance - with Rule 17-600.735, F.A.C . This report will document the capability of wastewater treatment, reuse, and disposal. facilities to function as intended during the period for which the permit is _ . requested. SCHEDULE CONSULTANT will prepare the permit renewal application in it timely manner. The renewal application for the Indian River County Central WWTF and. all supporting documentations specified above will be submitted to FDEP on or before September 29, 2013 . The renewal application for the Indian River County Blue Cypress WWTF and all supporting documentations specified above will be submitted to FDEP on or before October 19, 2013 . COMPENSATION Compensation for IRCU Work Order No . 7 services described herein are detailed below for each facility and will be based on the Lurap Sum Method in accordance with Section 5 of the Agreement. The total estimated fee shall not exceed $24,250 without mutual agreement by the County and CONSULTANT. The compensation for each facility shall be as follows: Task 1 Blue Cypress WWTF FDEP Permit` Renewal $3 ,500 Task , 2 — Central. WWTF FDEP Permit Renewal $209750 The cost of permit fees has not been included in this proposal. It is anticipated that the permit application fee for the Blue Cypress WWTF will be $ 1 ,000 and the permit application fee for the Central WWTF will be $5,000. ADDITIONAL SERVICES When required by the COUNTY or the Contract Documents, where circumstances exist ' beyond the CONSULTANT 'S control, CONSULTANT shall provide or obtain from others; as circumstances require, those additional- services not listed as, part of the Work Order. CONSULTANT shall notify COUNTY promptly prior to commencing said Additional Services, and if agreed upon, will beL paid for by COUNTY in accordance with the Master Agreement. Professionals and vendors intended to provide additional services shall first be approved' by the COUNTY, in writing, by the COUNTY's. Project Manager. REOUIRED INFORMATION TO BE PROVIDED BY COUNTY The COUNTY shall provide the following information: • Last 5 years of DMRs. • Current operating permit • Residuals Dewatering Permit • Current Reclaimed Water. or Effluent Analysis Report (Central WWTF Only) • Current Effluent ,Disposal and Reclaimed Water Use Protocol (Central WWTF Only) • Current Reuse Feasibility Study (Central WWTF Only) • RPZ certification(s) • Flow meter calibration report(s) • Violations / Inspections from FDEP • List of Modifications to WWTF since the last permit renewal (2009) • Agreements with. Individual Users of Reclaimed Water (Central WWTF Only) • Current Reclaimed Water or Effluent Analysis Report, (Central WWTF Only) • Residual Hauler Agreement • Provide existing facility volumes (i.e. aeration basins, chlorine contact chamber, etc.) either from design plans or recent permit renewal plans • Current Service Area, Map Page 3 of
